Gastric cancer was found in a patient in whom Menetrier's disease had been diagnosed 18 months before. In Menetrier's disease, incidence of cancer is as high as 14% of cases. But in only 5 cases of literature including the present one, has the time sequence been clearly documented. Metachronous tumours may be found 1 to 5 years after giant hypertrophic gastritis has been diagnozed. Such data make medical management, including anti-secretory drugs and periodic endoscopic survey, hazardous. We suggest that in documented Menetrier's disease elective total gastrectomy should still be considered as a treatment of choice.

TITLE: An evaluation of a direct access flexible fibreoptic sigmoidoscopy service.

During a 3 year period 146 general practitioners referred 630 patients to a direct access flexible sigmoidoscopy clinic. The yield was 53.3% with significant colonic or rectal pathology in 30%. Twenty six cancers, 4 Dukes' A, and 38 patients with symptomatic adenomatous polyps were detected. Five further cancers were detected by subsequent barium enemas. The service reduced delay in diagnosing colorectal pathology but did not reduce the number of barium enemas requested by general practitioners. It is suggested that where facilities are already available, such a service to investigate rectal bleeding in patients over 40 years is of benefit both to patients and general practitioners.

Oral cancer was studied in a population of 3435 which constituted all persons who were operated upon because of peptic ulcer in Gothenburg 1952-61. This population formed a data base which was coordinated with the National Cancer Register and the Register of Causes of Deaths in Sweden. The expected number of oral cancers was 7.7 and the observed number was 17 (P less than 0.01). In gerontologic population studies of the elderly in Gothenburg it was shown that 70-yr-old men with a history of peptic ulcer surgery more often were tobacco smokers, edentulous and more often had sideropenic and megaloblastic anemia and also had a lower bone mineral content.

In order to determine the best biochemical tests to detect cancer as the cause of ascites formation, ascitic fluid level of fibronectin, cholesterol, phospholipids and triglycerides were compared for 73 patients. The median values and the cut-off concentrations of these parameters were determined on 53 ascitic fluids without malignant cytology and 20 ascitic fluids characterized by a malignant cytology. The results showed that ascitic fluid fibronectin and cholesterol levels had a higher discriminating value than phospholipids and triglycerides to separate ascites caused by neoplasms. Cholesterol and fibronectin gave the same sensitivity and negative predictive value but cholesterol showed the highest specificity and positive predictive value.

An immunosuppressive factor derived from conditioned medium of a human renal cancer cell line, KU2, was analyzed, using mitogen-activated normal peripheral blood lymphocytes. Addition of conditioned medium to lymphocyte cultures resulted in suppression of [3H]-thymidine incorporation to DNA of lymphocytes in a dose-dependent manner. This effect was cytostatic and reversible. This factor inhibited lymphocyte activation stimulated by mitogen at an early stage, and it also suppressed Interleukin-2 production by activated lymphocytes. This factor was non-dialyzable and heat sensitive at 56 degrees C. DNA replication of the cells was necessary for the production of this factor.

In order to evaluate the possibility of an intensive radiotherapy of lung cancer in patients 70 years or older we compared the treatment results of 375 such patients with those, which we achieved in younger patients with the same kind of treatment. In non-small cell lung cancer radiation treatment with curative intent yielded at least equally good results for ages 70-75 years. Patients older than 75 years fared badly, however. After palliative irradiation the results are just as good as those in younger patients, even if the patients are older than 80 years.

Circulating levels of CA 125, the HMFG2 antigen and placental alkaline phosphatase were measured in patients with epithelial ovarian cancer. In 37 patients the antigens were assayed before operation and 161 follow-up samples from 41 patients were assayed at different times during treatment. These three human tumour-associated antigens were expressed independently of each other. Measurement of all three antigens, compared with measurement of CA 125 alone, resulted in a statistically significant improvement in the detection rate of patients with localized disease from 18% to 69%.

Bacterial and fungal contaminants have been identified in marijuana samples and thus are a potential risk factor in the immunocompromised patient using it as an antiemetic. We describe the development of an invasive pulmonary aspergillosis in a patient using illicitly obtained marijuana as an antiemetic during combination antitumor therapy for small cell lung cancer. Although this patient had multiple risk factors implicated in the development of invasive pulmonary aspergillosis, the infectious potential of inhaled marijuana must be recognized. Further study of this potential health risk in needed.

Such misconceptions are thus partly explained by the multistage nature of carcinogenesis, the absence of a clear understanding, and definition of the multiple factors which influence cancer induction. It is obvious that not all chemicals are carcinogenic and it is reasonable to accept that not all substances (including primary chemical carcinogens) that are found to enhance carcinogens in animals are necessarily carcinogenic to humans under conditions of usual human exposure.

A 10 year retrospective study of 134 patients in whom gallbladder cancer developed in El Paso County was undertaken. Improved survival was noted in younger, female, and Hispanic patients, which appears to be a unique demographic characteristic of gallbladder cancer in an area endemic for biliary calculi. These prolonged survival data may all be explained by early diagnosis. In addition, there is suggestive evidence that a multidisciplinary approach of operation plus adjuvant chemotherapy and radiotherapy results in greater length of survival.

The analgesic effect of eel-calcitonin (Elcitonin) was evaluated in 10 patients with metastatic bone lesions from urogenital cancer. Five patients had renal cell carcinoma, while the remaining 5 patients had prostatic cancer. Eel-calcitonin was injected intravenously to each patient at a dose of 80 units every day. The drug was markedly effective for 3 patients, effective for 4 patients and ineffective for 3 patients. The effect was better for prostatic cancer than renal cell carcinoma.

Four hundred and forty-one consecutive patients referred for endoscopy after a negative double-contrast barium meal were studied to determine the reliability of good quality radiology in excluding significant upper gastrointestinal pathology. Endoscopic abnormalities were detected in 23 patients (5.2%), nine of which were reporting errors. Duodenal ulcer was the commonest lesion missed radiologically, 50% of these patients either having a history of overt upper gastrointestinal bleeding, a family history or impressive symptomatic relief with H2-receptor antagonists. The three patients with radiologically undetected gastric cancer presented with classical symptoms; the radiological demonstration of gastric mucosal atrophy in this clinical context should alert the referring clinician to initiate urgent endoscopy.
