TITLE: Skin-tumor-initiating ability of benzo(a)pyrene-7,8-diol-9,10-epoxide (anti) when applied topically in tetrahydrofuran.

The skin-tumor-initiating abilities of various metabolites of benzo(a)pyrene (BP) were determined in mice by using a two-stage system of tumorigenesis. We previously reported that BP-7,8-dihydrodiol (+/- trans) was approximately as potent as BP, suggesting that it may be a proximate carcinogen, but the alleged ultimate carcinogen of BP [BP-7,8-dihydrodiol-9,10-epoxide (anti)] was a weak tumor initiator (Cancer Lett.2: 115, 1976). Because of its high reactivity, the tumor-initiating ability of the BP-7,8-dihydrodiol-9,10-epoxide (anti) was determined by using acetone, benzene, and tetrahydrofuran (THF) as the solvent vehicles. The 'diol-epoxide' of BP was found to be an effective tumor initiator when applied topically in THF. The effectiveness of the various vehicles for the 'diol-epoxide' was as follows: THF greater than benzene greater than acetone; however, acetone was the best solvent for BP tumor initiation. The BP-9,10-dihydrodiol and BP-3-hydroxy were found to be weak tumor initiators. BP-3-hydroxy was also tested for tumor-promoting ability and was found to be inactive in this capacity.

Since cellular and humoral immune mechanisms were shown to be involved in the pathogenesis of rheumatic diseases, efforts have been made to influence these systems therapeutically. Aside from suppression of the immune response, studies have been conducted to evaluate to what extent immunostimulation might be of value in the treatment of rheumatic diseases. Levamisole, chemically a simple synthetic agent recently shown to have immunorestorative capacities in anergic cancer patients, has been administered in several rheumatic diseases. The drug was administered either continuously or intermittently in a daily dose of 150 mg over several months. In about half of the rheumatoid arthritis patients a significant improvement was noted. Beneficial effects and sometimes even dramatic improvement lere observed in some patients with systemic lupus erythematosus, ankylosing spondylitis and Reiter's syndrome. Levamisole did not alter the course of psoriatic arthritis. Significant adverse reactions were leukopenia and allergic skin reactions. Other side effects were mild and did not require interruption of drug intake. The use of levamisole in rheumatic diseases is still experimental, but with accumulating experience it may provide a true improvement in control of rheumatic diseases.

Comparisons have been made of the DNA polymerases of normal human lung and cecum, primary carcinomas of human lung, breast, and cecum, and resting and regeneration rat liver. The picture for the normal human tissues is similar to the one for unstimulated rat liver, that for the human carcinomas resembles regenerating rat liver. The human tissues contain two polymerases with sedimentation coefficients of about 3 and 7 S, the enzymes are restricted to the nucleus, and the specific activities of the 7 S polymerase, but not of the 3 S enzyme, are elevated in the cancers. Just as with the regenerating rat liver polymerases, the 3 S activity of a bronchogenic carcinoma is unaffected by cytosine arabinoside 5'-triphosphate and only little reduced by novobiocin, whereas DNA synthesis by the 7 S enzyme is abolished by both compounds. A variety of other inhibitory agents have similar effects on the 7 S polymerases of the human carcinomas and regenerating rat liver.

Antisera raised in rabbits against glutaraldehyde-fixed human breast cancer cells contain antibodies to human cell membrane components, as determined by immunofluorescence. Adsorption of such antisera onto polymerized human serum, followed by acid elution, yields purified antibodies reacting with human cell surface antigens, indicating that membrane related antigens are present in the serum. The purified antibodies were radioiodinated and shown to bind to an immunoadsorbent prepared by entrapping in a polyacrylamide gel pleural exudate of breast cancer patients. The specificity of the binding was confirmed by inhibition experiments. Data are presented demonstrating that at least some of the antibodies reacting in this radioimmunoassay are directed against antigens related to cell surface components.

TITLE: In vivo and in vitro measurements of the relationship of human squamous carcinomas to herpes simplex virus tumor-associated antigens.

An additional 244 unfiltered sera have now been studied in a series of controlled, coded tests to determine the relationship of squamous carcinomas of the head and neck and cervix to the presence of complement-fixing antibodies to herpesvirus-tumor-associated antigens (HSV-TAA) in both tumor-bearing and cured patients. Ninety % of sera from patients with squamous carcinomas had antibodies to HSV-TAA, in contrast to 11% of sera from patients with nonsquamous cancers and 4% of sera from noraml individuals. The temporal relationship of Stage 1 laryngeal carcinomas suggests that HSV-TAA appearance precedes the immune defects. An in vitro correlate of the previously demonstrated specific delayed hypersensitivity reactions in controlled skin tests of squamous carcinoma patients with HSV-TAA is reported. In leukocyte migration inhibition tests, the migration indices after incubation with HSV-TAA of peripheral blood leukocytes from patients with squamous carcinoma (x = 0.847) were in definite contrast to migration indices seen for normal leukocytes (x = 1.037) and patients with nonsquamous solid cancers (x = 1.03). Thus, these polypeptides elicit both humoral antibody response and cell-mediated reactivity.

Eight of ten cases of colo-rectal cancer having positive radionuclide bone scans were observed in a Radiation Oncology Clinic in one year. Eight were referred to the institution after treatment elsewhere. In a series of fifty cases with their definitive therapy and evaluation at this institution, 33% of those with bone scans were positive. Review of recent literature shows that 50% of colo-rectal carcinoma have positive bone scans, usually following a multiple and widespread distribution including ribs, pelvis, vertebrae, skull or extremities. These studies indicate that bone scanning may greatly increase precision in staging of colo-rectal carcinoma and aid in the recognition of the truly localized primary lesion. The mechanism of spread of bone metastases in colo-rectal carcinoma was presumably through Batson's plexus.

The Western Cancer Study Group (WCSG) retrospectively reviewed chronic granulocytic leukemia (CGL) diagnosed during 1960 to 1974 from 22 institutions. In 100 cases, patients had positive findings for the Philadelphia chromosome (Ph'+). In 426 cases karyotyping was not done (Ph'?). Ten patients had no Ph' chromosome (Ph'-). The ten Ph'- patients ranged in age from 8 to 85 years and in survival from 1 to more than 125 months. Their small number and heterogeneity precluded statistical analysis, but this notable heterogeneity of the Ph'- group may well be typical. Busulfan prolonged survival in the Ph'+ and Ph'? patients. Females and younger patients tolerated their disease better than males and older patients. Survival studies without reference to the Ph' status, treatment, age at diagnosis, and sex are probably not valid.

The population of California and the San Francisco Bay Area has a number of ethnic components that differ in the frequency with which certain cancers occur. Mortality rates of California Janpanese and incidence data from the cancer reporting system of the Bay Area are analyzed for the cancer sites considered related to nutrition. The American Japanese of California are particularly amenable to epidemiological study because of the cancer data available from Japan for comparison with that of the California Japanese. The cancer rates occurring among the successive generations of the Japanese in California are compared to the rates in Japan and the white rates for the Bay Area. Gastric cancer rates undergo a stepwise reduction from the high rates in Japan to the intermediate rates of immigrant Japanese and the lower rates for the American born. Colon cancer rates are increased about equally in both generations of Japanese and are approaching the white rates. Cancers of the breast, uterine corpus, and ovary in women and the prostate for men are rapidly approaching the rates for the white population. Etiologies are continually being identified or indicated for a number of these cancers by epidemiological studies, and more specific identification of carcinogenic mechanisms should be possible by other disciplines.

Intravenous injections of urea-denatured ovalbumin (UD-OA) into OA-primed high responder mice suppressed the antibody response not only to the priming antigen but also to subsequent immunization with dinitrophenyl derivatives of OA (DNP-OA). The transfer of normal spleen cells or OA-primed spleen cells into UD-OA-treated animals did not restore the capacity of responding to DNP-OA to form anti-DNP IgE and IgG antibodies. The transfer of splenic T cell fraction from the UD-OA-treated animals into normal syngeneic mice diminished both IgE and IgG antibody responses of the recipients to DNP-OA. The B cell-rich fraction from the same donors failed to affect the anti-hapten antibody response and enhanced anti-cancer (OA) IgG antibody response of the recipients. It was also found that the transfer of T cell-rich fraction of OA-primed spleen cells failed to suppress antibody response of the recipients to DNP-OA. The results indicated that spleen cells of UD-OA-treated mice contained suppressor T cells which are distinct from helper cells. Suppressive activity of T cells in the UD-OA treated animals was specific for OA. The transfer of the T cell-rich fraction failed to suppress anti-DNP antibody response of the recipients to DNP-KLH.

TITLE: The chest radiograph in primary liver cancer: an analysis of 449 cases.

The value of plain chest radiography in the diagnosis of primary hepatocellular cancer (PHC) was assessed in 449 patients. An elevated right hemidiaphragm and pulmonary metastases were present in 11% and strongly suggest a diagnosis of PHC. In all, the right hemidiaphragm was raised in 30% of patients. Pulmonary metastases were present on admission in 19% of patients and appeared later in a further 6%. Secondary deposits were found in the lungs at necropsy in 52% of patients, compared with only 24% in whom there was radiological evidence of metastases during life. In 36% of patients chest radiographs were normal on admission.

One hundred and seventy seven pieces of normal or pathologic thyroid tissue from 17 patients were assayed for rRNA, tRNA and DNA content. The tRNA/DNA and rRNA/DNA ratios in pathologic tissue were statistically compared with the same ratios in normal tissue. In toxic adenoma (5 cases) and anoplastic cancer (2 cases), both ratios were increased. In cold nodules (9 cases), there was in increase of the tRNA/DNA ratio only in 1 case, of the rRNA/DNA ratio only in 4 cases, and of both ratios in 3 cases. In one case of a cold nodule in a Basedow's disease gland, there was no modification of these ratios. In Basedow's disease (3 cases), there was an increase of rRNA/DNA ratio only in one case.

On 170 patients with histologically proven carcinoma of the prostate scintigraphic studies of the skeleton using gamma camera and follow-up examinations were performed and compared with x-ray as well as serum alkaline and acid phosphatase. Osseous metastases in 47% had no radiological evidence and were only scintigraphically detectable. Positive scans were registered in 48% of the patients with prostatic cancer, 20% of them were positive due to metastases and 28% were false positive caused by osteoarthrotic and arthritic changes, sporadically by post-traumatic lesions and in 3 cases by Paget's disease. At the time of the initial diagnosis of prostatic cancer 21% of 159 patients studied scintigraphically had radiological or scan evidence of osseous metastases. Analyses corresponding stages of tumor revealed an unequivocal dependance of the frequency of metastases upon the extent of the primary tumor. The successful treatment is characterized by the decreased uptake of radioactivity primarily accumulated in skeletal metastases.

Some 1220 male and 2102 female cases of malignant neoplasms diagnosed histologically at the Department of Pathology of the Gadjah Mada University in Yogyakarta during the period 1970-73 were analysed. The most frequent tumour sites were among men: nasopharyngeal cancer, 21-8% and skin cancer, 17-6%. Among women the genital cancers were the most frequent with: cervix uteri, 25-7% chorionepithelioma, 3-7%, other uterus, 4-4% and ovary, 7-4%. Breast cancer comprised 17-0%, skin 9-6% and nasopharyngeal cancer 7-9%. Low frequencies were observed in both sexes for cancers of the gastro-intestinal tract and of the respiratory organs; previous reports of the rarity of gastric cancer were confirmed. The observed distribution is discussed in the light of possible biases, and compared with other material on the frequency of cancer from South East Asia.

The aim of adjuvant chemotherapy is the destruction of micrometastases after surgical removal of a malignant tumor. This treatment modality is gaining in importance in the light of experimental data and lcinical success in pediatric tumors. Results of ongoing studies in colo-rectal cancer show a marginal effect of prophylactic treatment with 5-fluorouracil. The treatment benefits in trials with historical controls are much greater than in studies with simultaneous controls. Use of historical controls is therefore of doubtful value. Ongoing trials use the combination of 5-fluorouracil and methyl-CCNU, which has been shown to double the remission rate in advanced gastrointestinal cancer. Adjuvant chemotherapy of colo-rectal cancer is still experimental and justified only in the framework of clinical trials.

TITLE: Human endometrial carcinomas serially transplanted in nude mice and established in continuous cell lines.

Three out of five human endometrial carcinomas were successfully grafted into nude mice (BALB/c/nu/nu). Two of these tumors could be maintained by serial transplantation. The morphological characteristics displayed by the grafted tumors were comparable to those of the original carcinomas. Permanent cell lines were established from these two tumors. Reinjection of cells grown in vitro into nude mice produced nodules of identical histology as compared to original solid transplants. The influence of medroxyprogesterone acetate on tumor growth in vivo and cell proliferation in vitro was studied. This hormonal treatment did not produce any significant effect on tumor cells, either in vitro or in vivo, for the two endometrial carcinomas. After medroxyprogesterone administration, a slight but non-significant growth inhibition of the tumor cells in vitro was observed and the tumor transplants in vivo did not appear to be influenced. The experiments illustrate the possible use of this model for testing potential anti-cancer agents.
