A personal series of 83 cases of carcinoma of transverse colon has been analysed, as the recent works of international literature. The low percentage of success of these cancers is justified by the anatomical conditions and the pathological findings. Particularly, this prognosis is made worse by the high percentage of multivisceral cancers, hepatic metastases and emergency cases. The operative technics are very different. Beside the typical resections (right or left hemicolectomy), there remains one more important place for segmental colectomy. At least the subtotal colectomy gets some more indications, especially in the high risk patients. Only 28% of the resected cases shows a 5 years survival rate. This percentage is nearly the same as most of the recents statistics.

TITLE: Electromyographic and nerve conduction findings in a patient with neuromyotonia, normocalcemic tetany and small-cell lung cancer.

A 57-year-old man with neuromyotonia and normocalcemic tetany as probable paraneoplastic symptoms of small-cell lung cancer was examined neurophysiologically. In EMG, spontaneous action potential generation was demonstrated in peripheral motor axons. There were also time-locked high-frequency discharges after some voluntarily activated motor unit potentials. After electrical stimulation of motor axons, the M-response as well as spinal F-response or H-reflex was often followed by a sequence of oscillating potentials which could last several hundred msec. There was no evident peripheral polyneuropathy. Tetany subsided when carbamazepine therapy was started. The only striking abnormality in extensive laboratory studies was an increase in the plasma noradrenaline value.

The determination of oestrogen receptors seems to be of prognostic value in the radically-operated breast cancer patient. Patients with negative receptor values exhibit early recurrence in a significantly higher percentage of cases than those with positive values. The prognostic implications of oestrogen receptor determination seem to be even higher than an assessment of axillary lymph node involvement. In metastatic breast carcinoma a correlation exists between response to hormonal therapy and receptor assay. The clinical implications for therapeutic management are discussed.

TITLE: A note on the use of nitrate fertilizers in a high-risk geographical area for stomach cancer.

Chile being the only country in the world producing natural fertilizers (NaNO3 and KNO3), it uses very large quantities of them for agricultural purposes. Since in early 1970 Zaldívar first related the use of fertilizers to stomach cancer mortality, it is of key interest to know the amount of nitrogen fertilizers used, expressed in tons of N, by province, for the period from 1945 to 69. Of a total of 420,750 tons of N for the nation, 238,950 tons were used in the high-risk provinces for stomach cancer (Colchagua, Curicó, Talca, Maule, Linares, Nuble, Concepción, Bio-Bio, Arauco, Malleco, Cautín). On the other hand, in the low-risk provinces (Tarapacá, Antofagasta, Atacama, Magallanes) only 2,550 tons were used. In Chile, the use of fertilizers increased from 37,550 (1945-50) to 185,050 (1965-69) tons of N.

TITLE: Cytological study of fluid from breast cysts.

A series of 215 patients with breast cysts have provided 348 specimens of cyst fluid for cytological diagnosis. Only one of these specimens was reported positive for malignant cells and this patient had a carcinoma. Nine specimens were reported as suspicious, but none of these patients had or subsequently developed carcinoma. In 338 specimens cytology was negative. A total of 178 patients (294 specimens) was available for follow-up. Of these 2 proved to have carcinomas on immediate follow-up, i.e. a lump presisted after aspiration, and a further 2 patients developed cancers in the same breast, one 4 and the other 7 years after aspiration. We therefore conclude that the routine submission of breast cyst fluid for cytology is wasteful of time and resources.

TITLE: The identification and measurement of a folate-binding protein in human serum by radioimmunoassay.

Antiserum raised in rabbits against the FBP obtained from CML cells, and the purified binder labeled with 125I, have been used for an RIA which can measure an immunologically similar protein in human serum. The concentration of the binding protein in normal serums ranged from 1.2 to 9.3 ng/ml, with a mean +/- S.E.M. of 3.8 +/- 0.4 ng/ml. Elevated values of the binder protein were measured in the serums from patients with folate deficiency, vitamin B12 deficiency, liver disease, uremia, myeloproliferative disease, chronic lymphocytic leukemia, and various types of cancer and in the serum from pregnant women. The concentration of the binder protein and the capacity of the serum to specifically bind isotopically labeled PGA correlated poorly, indicating that the binding protein concentration and degree of saturation by endogenous serum folate vary independently in many instances.

The authors studied the indices of the serumal ferrum (total ferrum of the blood serum, total ferrum-binding ability of the blood serum and transferrin saturation with ferrum) before and at different terms after the exfusion of 400 cc of blood for the autotransfusion in planned surgical interventions upon 30 cases of pulmonary cancer, 21 cases of carcinoma of the stomach and 16 orthopedic cases. The same study was carried out upon a control group of normal individuals after exfusion of 400 cc of donor blood. Characteristic changes of the serumal ferrum metabolism due to the loss of a certain amount of hemoglobin after the exfusion were found in differen patients and in normal individuals.

TITLE: Lung cancer in coastal Georgia: a death certificate analysis of occupation: brief communication.

A comparison of death certificate statements on occupation for 858 white males who lived in coastal Georgia counties and who died of lung cancer during 1961--74, and for 858 controls who were of the same age and county of residence revealed a twofold excess risk of lung cancer associated with the construction industry. No elevated risk of lung cancer was found for workers in the wood and paper industries in the urban areas of Savannah, Brunswick, or Waycross, but a threefold increase was uncovered for the remaining coastal counties where the wood and paper industry was the largest employer.

Three cases of alleged and feigned cancer are presented. The diagnostic and management problems presented are discussed through a review of the literature of feigned illness. This ranges across a spectrum from benign use of feigned or alleged symptoms, malingering, conversion reactions and hysterical manifestations to the severe and flamboyant clinical presentation of the Munchausen Syndrome. The cases presented mainly fit the criterion for malingering (conscious prevarication and simulation), although some elements of the Munchausen dynamics were present in two of the three. Also noteworthy was the fact that the "cancers" were presented to administration and lay people as an initial communication. The psychiatric consultation was with the lay people initially and the events of each episode were reconstructed through multiple sources of information including clinical interviews in two of the cases.

An analysis of 100 biopsy-proven lung cancers found by chance chest films and treated by resection have been collected over a 20 year period. Data relating to sex of patients, smoking habits, age groups, lung and lobar sites, cell type, treatment and survival are stated and discussed in relation to the findings of a known planned study for detecting early stage lung cancer by chest films. It is concluded that, if a person at lung cancer risk is genuinely interested in having regular checks to catch early, operable disease, then his chances are best with six-monthly chest films.

The concurrent combination therapy of radiation and Bleomycin was performed in a total of 175 cases of esophageal carcinoma. In 107 (63%) cases of them, in which the scheduled curative treatment was completed, yearly survival rates were 60% in one, 30% in two, 20% in three and 7% in five years. The local recurrence was noted in 60% and remote metastases were observed in 38% of them. These results were intimately correlated to tumor advancement and cancer type. From the results, it was concluded that this combination therapy should be valuable to prolong the life of a cancer patient, improving one to three year survival rates, and that the curable indications for this treatment should be T1 M0 and T2 M0 with tumorous or some ulcerous types of carcinoma.

Results of a comparison of the diagnostic efficacy of percutaneous needle biopsy of the parietal pleura with that of cytologic examination of pleural fluid sediment obtained concurrently from 166 patients with benign or cancerous pleural disorders are reported. Of 44 patients with confirmatory evidence of cancer involving the pleural surfaces, 43 had positive cytologic findings, whereas pleural biopsies were diagnostic in only 16 cases. Of a total of 122 patients with benign pleural diseases, cytology provided the diagnosis in two cases of eosinophilic pleurisy, and pleural biopsy contributed the diagnosis in four of ten cases of tuberculosis. The data indicate that percutaneous needle biopsy of the parietal pleura is less efficacious in the diagnosis of malignant pleural disease than is cytologic evaluation of the fluid sediment, whereas in the diagnosis of tuberculous pleurisy, pleural biopsy proved superior.

TITLE: Balloon catheter occlusion for cavernous carotid artery injury during transsphenoidal hypophysectomy. Case report.

During transsphenoidal hypophysectomy for pituitary ablation in a patient with disseminated breast cancer, brisk arterial hemorrhage occurred during separation of adhesions between the pituitary gland and the wall of the cavernous sinus. Hemorrhage was controlled by placement of a Prolo balloon catheter into the internal carotid artery (ICA) that occluded the site of hemorrhage. The patient experienced no neurological sequelae. The cervical ICA was easily exposed for insertion of this double-lumen catheter. With the image intensifier already in position, injection of contrast material through the arteriography lumen allowed precise localization of the site of injury and directed positioning of the balloon for control of the hemorrhage.

Total protein and sialic acid levels were determined in the supernatant of serum treated with perchloric acid. Patients with either localized or advanced metastatic malignancy have significantly elevated mean serum values. The highest levels occur in patients with lung, GI, GYN cancer, lymphoma and malignant melanoma. Patients with leukemia and multiple myeloma have slightly elevated values, but they were not significantly different from normal. Patients following curative surgery have normal values while patients in clinical remission following chemotherapy have elevated mean serum protein and NANA levels. Elevated values also occur in patients with benign tumors and 12% of patients with nonmalignant disease. Tumor cells appear to shed macromolecules which contribute to the observed elevation of serum protein and sialic acid levels.

TITLE: Effect of acute and chronic ethanol intake on metastasis formation in rat liver.

The effect of acute ethanol intoxication and prolonged intake of ethanol on metastasis formation in the liver was studied in the rat. The tumour cells used were derived from a pharmacologically induced colon carcinoma (NGW 1) and were injected intraportally. Acute ethanol intoxication at the time of injection was associated with a liver weight of 13.29 +/- 0.89 gm two weeks later, compared to 8.43 +/- 0.51 gm in controls. Daily intake of ethanol for eight weeks resulted in a mild reduction of platelets. The liver weight in two groups was 7.31 +/- 0.50 gm and 12.19 +/- 1.40 gm three weeks after intraportal injection of cancer cells as compared to 9.53 +/- 1.11 gm and 19.63 +/- 1.67 gm in controls. The results indirectly supported the hypothesis that acute ethanol intoxication causes increased platelet activity and confirmed earlier results showing the importance of platelets for metastasis formation.
