TITLE: The application of dose intensity to problems in chemotherapy of ovarian and endometrial cancer.

An attempt was made using dose intensity to resolve some of the conflicting issues regarding chemotherapy in ovarian cancer and endometrial cancer. Analyses were done to determine if dose intensity correlates with outcome of first-line and salvage chemotherapy of advanced ovarian cancer and chemotherapy of advanced endometrial cancer. The concept of dose intensity was used to analyze the relative contributions of individual drugs, such as cyclophosphamide, hexamethylmelamine, Adriamycin (Adria Laboratories, Columbus, OH) and cisplatin to outcome, and to suggest how such drugs should be used in combination.

TITLE: Statistical analysis of the surgical materials of stomach cancer in Japan.

Two thousand three hundred seventy gastric cancers from 2173 patients for which surgical resections were performed at the Tokyo University Hospital from 1967 to 1985, were analyzed. Cancers were classified into diffuse adenocarcinomas, intestinal adenocarcinomas, and an unclassified group. The proportional rate of the intestinal type was higher in the old than in the young, and higher in men than in women. The reverse was true of the diffuse type. The intestinal-diffuse ratio had increased recently, especially in patients with early cancers and in patients in their 50s and 60s, which suggested the efficacy of early screening methods for gastric cancers.

Local radiofrequency hyperthermia combined with chemotherapy was performed on five patients with pelvic recurrence of rectal cancer. Relief of pain was obtained in all four patients with uncontrolled pelvic pain due to recurrent rectal cancer. In some patients, CEA levels of peripheral blood were decreased and tumor necrosis was proved histologically. These results suggest that this combined therapy may be useful for patients with pelvic recurrence of rectal cancer.

Seventeen patients with metastatic renal cancer were treated with human lymphoblastoid cell-derived alpha interferon, 30 X 10(6) U/m2, which was given intravenously for 3 consecutive days of each week for 6 weeks. On the days of interferon treatment only, patients received prednisone, 15 mg t.i.d., in order to minimize toxicity. Of the 16 evaluable patients, one had a minor regression of disease, eight had stable metastatic disease, and seven progressed. Constitutional symptoms, such as fever, anorexia, and fatigue, were common. Debilitating degrees of malaise were a treatment-limiting factor.

We describe four patients with epithelial ovarian carcinoma and cerebral metastases. Cerebral metastases are still an infrequent occurrence in this malignancy but, as treatment for ovarian cancer produces more long-term survivors, such uncommon patterns of spread may become more frequent. Part of a future successful cure for this disorder may require the recognition of the central nervous system as a potential sanctuary site for metastatic disease.

TITLE: Inhibition of urethan-induced lung tumors in mice by dietary N-acetylcysteine.

The thiol N-acetylcysteine (NAC), a precursor of intracellular glutathione (GSH), efficiently prevented the induction of lung tumors in Swiss albino mice, when supplemented to the diet (0.2%) both before and after an i.p. injection of the carcinogen urethan (ethyl carbamate). Irrespective of urethan administration, NAC also significantly enhanced GSH S-transferase activity in liver preparations of the same animals. These data show that, under certain conditions, it is possible to prevent chemically induced cancer by increasing the levels of physiological trapping agents.

TITLE: Transient tumor marker elevation following chemotherapy for germ cell tumors of the testis.

A total of 22 adults with metastatic nonseminomatous germ cell tumor of the testis and raised pretreatment serum levels of the beta subunit of human chorionic gonadotropin (HCG) had serial monitoring of serum HCG by weekly assay following initiation of chemotherapy with bleomycin, etoposide, and cisplatin (BEP). A transient rise in HCG (the marker "surge") was detected in 9 patients (41%), of whom one relapsed. None of the other 13 patients have relapsed (follow-up, 24-60 months), and the BEP regimen overall leads to an 83.3% disease-free survival (Br J Cancer 47:613-619, 1983). The marker surge is not an adverse prognostic factor during initial chemotherapy.

We describe for the first time the specific determination of a tumor associated nucleic acid found in 50 of 57 sera from cancer patients. The novel tumor marker "RNA-proteolipid" described by us contains an mRNA of about 10 kilobases. We have established a test to determine this RNA by hybridization. A cDNA was obtained by reversed transcription from RNA using serum from a patient with Hodgkin's lymphoma. The cDNA was then ligated into a M13 phage and cloned in Escherichia coli. The phage DNA was sulfonated chemically and hybridized to RNA from neoplastic sera which was immobilized on nylon. The bound DNA was detected by an immunoassay.

TITLE: Completeness of chronic disease registration in general practice.

The completeness of chronic disease registration in four general practices was assessed by referring to the Hospital Activity Analysis. Overall, just over half (56%) of the 695 patients who had been discharged from hospital with a diagnosis of diabetes, cancer, myocardial infarction, epilepsy, hypertension, or thyroid disease were identified on the practice disease registers. Patients with diabetes were most likely to be identified (72%), and those who had had a myocardial infarct least likely (43%). If the standard of registration is to be improved general practitioners must be convinced of its value. The Hospital Activity Analysis might be used widely to audit and improve practice registers.

Reported is the case of 61-year-old male patient who developed an abscess, secondary to a ruptured retrocecal appendicitis, while undergoing effective chemotherapy for lung cancer. Patients undergoing cancer chemotherapy are especially susceptible to infectious complications because chemotherapy can cause myelosuppression. To our knowledge, has been no previous reported case of retrocecal appendicitis occurring during chemotherapy for the lung cancer. Retrocecal appendicitis is a dangerous form of complication occurring in patients being treated with chemotherapeutic drugs.

Data on fat and fibre content in the Singapore diet are presented. Food availability data show an upward trend in per capita fat and fibre supplies over the last 25 years. However, food availability is usually an overestimate of actual consumption. Data on food consumption levels show that fat intake in Singapore is in reality relatively lower than the corresponding intake in Western countries. Fibre intake is also lower than in the West. At the same time, the incidence of colorectal cancer is increasing. This provides an interesting opportunity for analytical studies to further elucidate the likely role of diet in the aetiology of colorectal cancer in Singapore.

TITLE: Adriamycin, cyclophosphamide and etoposide (ACE) in the treatment of small cell lung cancer.

Thirty-five small cell lung cancer (SCLC) patients were treated with combination chemotherapy including adriamycin, cyclophosphamide, etoposide (ACE). Out of 32 evaluable patients there were 21.9% complete responses and 53.1% partial responses with an overall median survival of 37 weeks (50 weeks for patients with limited disease and 34 weeks for patients with extended disease). Toxicity was generally well tolerated. In conclusion the ACE regimen results in being active and safe in the treatment of SCLC.
