Controlled clinical trials of therapy for childhood acute leukemia are reviewed in the Children's Cancer & Leukemia Group Studies 1972-1981. The mortality of acute lymphocytic leukemia in children has been reduced about one-half in the past 10 years. Continued progress depends on maintaining our objective of curing all children and in conceiving and testing new ideas that might contribute to cure. Nowadays, the end result of most importance is long-term leukemia-free survival, i.e., the freedom from relapse after off therapy. Advances in the childhood leukemia have paved the way for encouraging progress in the management of the much more common malignancies that affects adults.

Our original studies of the enkephalins were centered on behavioral stress and brain dopaminergic interactions. More recently we discovered the enkephalins to be immunomodulators as evidenced by their enhancement effects on lymphocyte blastogenesis in mice, increases in the sizes of the thymus or spleen in rodents, and prolongation of survival of BDF1 mice inoculated with attentuated L1210 cells. Finally, in studies of human blood samples from both normal volunteers and cancer patients, the enkephalins were demonstrated to stimulate active T cell rosettes and natural killer cell activities (in vitro). These studies support our hypothesis that, in stress, the enkephalins modulate the effects of steroid hormones on the immune system.

Relationship between irradiated dose and function preservation of the larynx in patients of laryngeal carcinoma (T1-T3) treated with radiotherapy were analysed and discussed. Local control rate for T1 and T2 lesion was highest in the dose range of 65-70 Gy. But in T3 lesion, especially of supraglottic cancer, the dose range to obtain highest local tumor control rate is above 76 Gy and higher than in case of T1 and T2 lesions. Laryngeal edema was seen in patients who were delivered the dose of 80 Gy. But total laryngectomy could be avoided by conservative therapy. Necessity of induction chemotherapy for advanced cases was also discussed.

TITLE: Crisis intervention: a group-therapy approach for hospitalized breast cancer patients.

The purpose of this study was to assess the effect of nursing intervention on the level of depression and the self-esteem of hospitalized breast cancer patients. The theoretical framework was based on crisis theory (Caplan 1964). The population of this study consisted of 18 female hospitalized breast cancer patients. Ten were in the control group and eight in the experimental group. Two self-administered instruments were selected for the pre- and post-testing of all clients. Nursing intervention was employed for the experimental group, over a 2-month period. The findings demonstrated that there was a change in the level of depression and improvement in self-esteem among the subjects in the experimental group after nursing intervention.

However, in 50% of the cases, alterations of the gonadotropins FSH and LH secretion occur. A relative hyperestrogenic state ensues. This has been proved to be involved in the genesis of uterus and breast dystrophies. Consequently, this method of contraceptive should not be offered to patients with benign breast disease, endometrial or myometrial hyperplasia, in premenopausal women or those women with a hisotyr of dysovulation, and in women with a high risk of genital cancer. If generalizations can be drawn from these two sessions, we would suggest that evidence can be seen in support of certain principles for the chemotherapy of breast cancer. These principles are: Combination chemotherapy. Doxorubicin-containing combinations are superior to others in terms of improved remission rates. In the past decade, combination chemotherapy has resulted in significant improvement of survival in patients with disseminated breast cancer, and with this approach a small fraction of patients with metastatic disease have remained in extended complete remission. Dose of drugs. Drug combinations utilized at lower doses result in lower response rates. Drug dose rate. When chemotherapy is interrupted for treatment of local disease in inflammatory carcinoma of the breast, the survival rate of patients is lower compared to the subgroups of patients where there is no interruption in the treatment.

In search of perspectives pertaining to the selection of invasive procedures for evaluating patients with lung cancer, data from other centers were considered, and our recent consecutive experience with 40 lung cancer patients was reviewed. Cost estimates for the least and the most complete use of invasive procedures were done. An average of three procedures per patient was used. Information judged beneficial was obtained from 94 for 120 procedures (78 percent). By retrospective analysis, 15 procedures (0.38 per patient) provided no useful information. An approach to the use of invasive procedures in the management of patients with suspected lung cancer is proposed.

Peroxisomes (microbodies) have been localized in the cytoplasm of various cells, mainly the hepatocytes and the renal tubular cells. Various enzymes were found in the granular matrix and sometimes in the nucleus ; they were involved in the production and the degradation of hydrogen peroxide, in regulation of the energetic processes of the cell, and in the oxidation of fatty acids. Several xenobiotics and drugs such as hypolipidemic agents clofibrate-like, have induced in rodents a proliferation of peroxisomes and frequently, the appearance of malignant tumors, in men, the precise role of peroxisomes in the physiology of the cell, their link with the hypolipidemic action of some drugs and the development of cancer remain to be established.

The authors described clinicomorphological correlations in the evaluation of the efficacy of preoperative radiotherapy combined with metronidazole (MZ) in laryngeal cancer patients. A study was made of the tumors from 61 laryngeal cancer patients following radiation exposure at a summary dose of 20 and 32 Gy with and without MZ. Histological change of a tumor confirmed a radiosensitizing effect of MZ. However the MZ effect was not equally noticeable in tumor central and peripheral parts. Microscopic foci of the affected epithelium of the larynx and gland mucosa, muscular cells and vessels were revealed. MZ produces a stimulating effect on collagen production by fibroblasts.

TITLE: Mortality among miners receiving workmen's compensation for silicosis in Ontario: 1940-1975.

We investigated mortality among 1,190 Ontario miners who received Workmen's Compensation awards for silicosis from 1940 through 1975. In comparison with the general population of Ontario these men had elevated all-cause mortality rates, with deaths attributed to nonmalignant respiratory diseases and tuberculosis being primarily responsible. The group of miners receiving their compensation awards between 1940 and 1959 has experienced more than twice as many lung cancer deaths as expected while men receiving compensation awards after 1959 have had lung cancer rates similar to the general population. It is concluded that silicosis is not a benign disease and that efforts must be continued to prevent its occurrence.

TITLE: [Regression of adenomas in Gardner's syndrome induced by systemic chemotherapy]

A 31-year-old male was admitted to our hospital with Gardner's syndrome and sigmoid colon cancer. Palliative resection (sigmoid colectomy) was performed due to hepatic and lymph node metastasis. Systemic chemotherapy with MMC, 5-FU and PSK was started postoperatively. Barium enema study on the 23rd successive post-operative day and fiberscopic study on the 134th post-operative day showed regression of the size and number of the polyps in the remaining colon and rectum. We suggest that the administration of anticancer drugs may be useful in the treatment of familial polyposis or Gardner's syndrome which have been treated with only surgical therapy.

Fifty-three patients with documented primary biliary carcinoma were studied with computed tomography. Twenty-six patients had gallbladder carcinoma and 27 patients had carcinoma of the biliary ductal system. Ninety percent of patients with gallbladder cancer had an intraluminal mass. Local invasion into the liver was common. The majority of patients with biliary ductal carcinoma had dilated bile ducts, while only 50% of patients with gallbladder cancer had biliary ductal dilatation. The most common location of tumor involving the extrahepatic biliary ductal system was the distal common bile duct. This occurred in eight patients out of 27, or 30% of the cases.

Localized metastatic foci of the colon from gastric cancer have been reported in only a few cases. Tow cases of gastric cancer were reported. One relapsed in the form of a localized lesion at the sigmoid colon (Case 1) and the other at the rectum and the transverse colon (Case 2) after curative resection of gastric cancer. From the results of clinical examinations obtained from these cases, it may be reasonable to postulate that peritoneal dissemination and/or lymphatic invasion of cancer was the main cause of solitary metastasis to the large intestine.
