TITLE: Photoradiation therapy in early gastrointestinal cancer.

Endoscopic photodynamic therapy has been used in the treatment of 19 cases of upper gastrointestinal cancer of which six were superficial esophageal and 13 were early gastric cancer. Six patients subsequently underwent surgical resection. Residual tumors were found in the resected specimens of one esophageal carcinoma and in the two early gastric carcinomas. Technical problems resulted in one failure. Follow-up ranged from 4 months to 3 years and 11 months with no tumor recurrence in either the operated or unoperated patients. Other delivery systems are currently under investigation.

The correlation of clinical, echo and morphological findings demonstrated ultrasound potentialities in the differential diagnosis of thyroid cancer. Ultrasound permitted the detection of nodes up to 3 cm in diameter. False-negative results were obtained in 6.5% of cases, there were no true false positive results. Adenomas are often characterized by mixed echogenicity of a node and hypoechogenic lining around it. Chronic nonspecific thyroid diseases are characterized by inhomogeneous echogenicity of the organ. Thyroid cancer is characterized by hypoechogenicity of a node with its inhomogeneous structure and irregular contours as if consisting of several nodules. The probability of malignancy grows with an increase in the number of these features combining in a particular patient.

TITLE: An unusual presentation of colon cancer.

A rare case of asymptomatic cancer of the ascending colon presenting as a subcutaneous groin mass is reported. At the time of right hemicolectomy with resection of the groin mass there was no sign of incontinuity spread or lymph blockage. Sequential determination of carcinoembryonic antigen levels (CEA) in serum reflected the reduction in tumor mass after surgery as well as subsequent recurrences of disease in the groin and in the lung after 2 years. Immunoperoxidase staining of the primary tumor and the metastases showed strong positivity for CEA confirming the origin of the serum CEA. There have been no signs of liver involvement. The route of spread of this unusual metastasis is not known.

TITLE: Prolonged survival without treatment in a patient with primary hepatocellular carcinoma.

The proportion of primary hepatocellular carcinomas which are technically operable with hope of cure is small. Recommended therapy ranges from aggressive therapy to a nihilistic approach. There is little data available on the natural history of untreated tumours and we report such a case. A review of the literature regarding survival with primary hepatocellular carcinoma shows that the prognosis of patients with this tumour may vary considerably. Such variability has important implications for those involved in the design and interpretation of trials of treatment for hepatocellular cancer.

The occurrence of high cadmium levels in the digestive gland (hepatopancreas) of the crab Cancer pagurus is reviewed and its possible significance to human health is discussed. Previous attempts to isolate this cadmium in its native form are briefly reviewed and the present method used in this laboratory to isolate crab cadmium metallothionein is described. Quantitative amino acid analysis of the purified protein suggests that it has a total of 55 residues, including 19 Cys, and a molecular weight of approximately 5800. This yields an extinction coefficient at 220 nm for the protein in HCl, pH 1.5, of 50,000 and at 250 nm for the native cadmium protein in Tris-HCl, pH 8.4, of 92,000.

TITLE: [Primary diffused infiltrating (linitis plastica) type of colonic cancer--case report]

The primary diffused infiltrating type of colonic cancer is characteristically rare, and has a high rate of malignancy. A 54-year-old Japanese man was admitted to our hospital complaining of abdominal pain and decreased stools. Sigmoid colonic cancer was suspected as a result of the Ba-enema and colonic fiberscopy examination, but the biopsy results were negative. The sigmoid colon was resected extensively. Histopathologically, the cancer was diagnosed as being a signet ring-cell carcinoma of the sigmoid colon. Although the patient received postoperative chemotherapy, he died of carcinomata peritonitis 175 days after surgery.

Progesterone receptors (PR) from human breast cancer cells have been purified, and used as antigens to generate anti-PR monoclonal antibodies. Immunologic and in situ photoaffinity labeling methods were then used to study receptor structure, and we conclude that native human PR consist of two independent 8S receptors: one 8S subtype contains B-proteins (120,000 kDa) and the other contains A-proteins (94,000 kDa).

Breast parenchymal patterns have been assessed by the method of Wolfe in a prospective study in Guernsey. Women with P2 or DY grades had approximately four times the risk of developing breast cancer compared with those with N1 or P1 grades. Age, weight, parity and age at birth of first child are all related to the distribution of mammographic patterns. The combination of these variables with Wolfe grades may identify subsets of the population at very high risk.

Recent reviews of the management of hilar biliary cancer suggest that radical resection is the preferred treatment option. It seems true that most of the long term survivors are treated this way, but it is equally true that most patients receive palliative rather than curative treatment. Before we go too far in our advocacy of radical treatment, we need to develop methods for quantifying palliation because that is in truth what we usually achieve. A method of quantifying palliation is described. Surgeons are encouraged to use methods of this kind in order to allow valid comparison of the results of different methods of treatment.

Four patients with proven ovarian cancer were found to have single lesions in the liver parenchyma after completing chemotherapy. They otherwise appeared to be in complete clinical remission. Percutaneous needle biopsy was sometimes misleading and dangerous. Laparoscopy and laparotomy were required for optimum safe diagnosis, staging, and treatment. Definitive resection appeared to be beneficial but usually only as a palliative form of debulking. Subsequent adjuvant combination chemotherapy may sometimes improve the duration of useful remissions. Unusual sites of metastatic ovarian cancer appear to be increasing in frequency. Successful management of late recurrences in the liver is possible, provided the diagnosis is immediately suspected and one utilizes both definitive surgery and chemotherapy.

Chest radiography is the basic imaging method in lung cancer. It allows early detection of peripheral and, with lower sensitivity, detection of central neoplasms. The subsequent staging procedure includes other imaging tools, first of all computed tomography (CT) for a noninvasive estimate of the local and regional extent. Because of high sensitivity and lower specificity for mediastinal disease, positive CT scans suggesting inoperability ask for cytological verification whereas negative studies justify surgery unless other contraindications exist. During and after treatment, chest radiography again affords a morphological follow up. Systematic comparison of the current with recent and early post-treatment studies is a prerequisite for detecting tiny signs of recurrent disease and for differentiating them from iatrogenic changes and infection.

A comparison has been made between the absorption of oral medroxyprogesterone acetate (MPA) in an aqueous suspension preparation and in syrup form. Plasma drug profiles were measured after a single administration of the two formulations in 17 advanced cancer patients. On average the standard form (aqueous suspension) gave peak levels which were lower than the syrup mixture. However, the wide intersubject spread in MPA plasma levels observed in both groups did not allow any statistical significance to be assigned to this difference.

Fifty-two patients with thyroid epithelial cell cancer were studied for evidence of association with human leukocyte antigens (HLA). Twenty-eight patients (53.8%) and 19.4% of 160 controls were HLA-DR1-positive, conferring a relative risk of 4.85 (chi 2 = 21.3, P less than 0.0001). HLA-DR1 was increased in all histologic types of thyroid cancer. Interestingly 10 of 12 patients with metastatic disease were DR1-positive compared to 18 of 41 patients without metastases (relative risk = 6.1, chi 2 = 4.7, P less than 0.05). This study suggests that major histocompatibility complex-linked gene(s) determine susceptibility to and the biologic behavior of thyroid cancer.

Despite frequent cancer metastases to the thyroid gland demonstrated at autopsy, finding of thyroid metastasis in clinical practice is rare. Malignancies most commonly metastasizing to the thyroid are malignant melanoma and carcinoma of the breast, kidney, and lung. Very little is known about thyroid metastases caused by sarcomas. Of these, malignant fibrous histiocytoma (MFH) is one of the most common in adults, with a high rate of metastases. A case of clinically significant thyroid metastasis from primary MFH involving the thigh visualized on I-123 thyroid imaging is reported.
