TITLE: Vindesine (VDS) monochemotherapy for non-small cell lung cancer: a report of 45 cases.

Fifty-three patients with non-small cell lung cancer were treated with vindesine. Of the 45 evaluable patients, 11 (24%) had a partial response. Responses were all evident within 6 weeks. Median duration of response was 8 weeks from documentation. Median survival was not improved significantly among responders (P less than 0.10, two-tailed test of significance). The most frequent toxic effect was leukopenia. The most troublesome toxic effect was peripheral neuropathy, with patients greater than 60 years old experiencing this more frequently.

TITLE: Blood pressure distribution in responders and initial non-responders in a population screening study.

The distribution of diastolic blood pressure (DBP) was compared between those who responded to a first written request to attend at their general practitioner's surgery for screening for hypertension and those who need more intensive effort encouraging them to attend. After excluding 25% of the population whose current address could not be traced, 92% of the remainder were eventually seen and screened. There was no association between diastolic blood pressure and effort needed to bring the patient to screening, and this applied to all patients both with and without a history of hypertension. Thus it appears that unlike screening for cervical cancer, screening for hypertension may not selectively tend to miss the groups most at risk.

By the use of a simplistic concept of the structure of heterochromatin, one can derive a formulation for the mechanism by which the phenomenon of V-type position effect can repress genic activity. This notion would explain how the Bar locus and other position effect examples cause genic repression. The mechanism, when extended, may also explain the process of cancer induction by hereditary means. In its most concise form the theory states that heterochromatin has a stabilized structure through special proteins, such as phosphorylated species that allows it the capacity to induce new domains of a "pseudo-heterochromatic" conformation to juxta-positional euchromatic segments for short distances by virtue of electrostatic and hydrogen bond forces.

Complications after definitive radiotherapy of prostatic cancer occur in 3 to 30% of cases. Most common is radiation injury of the urethra, the bladder or the rectum. Definitive radiation therapy of carcinoma of the prostata after staging lymphadenectomy has a considerable risk of lymphedema of the lower extremities. Two typical cases are used to illustrate the permanent damage done to the quality of life of the particular patient. The complication rate is decreased if only the prostate is irradiated without radiation of the regional lymphnodes. Interstitial radiation therapy using 125-Iodine-seeds yields comparable results with a tenfold decrease in the complication rate.

Fibroblast strains from 6 patients with ataxia telangiectasia (A-T) were found to be markedly hypersensitive to the cytotoxic action of the tumor promoter phorbol-12-myristate-13-acetate (PMA), their D37 values being 5 times lower than those of two normal controls. Two A-T heterozygous strains were slightly hypersensitive to PMA, while a third one showed normal sensitivity. It is concluded that the DNA lesion which is critical in A-T cells is an important component of the damage caused by PMA-induced free radicals and may play a role both in the tumor-promoting activity of PMA and the cancer proneness of A-T patients.

Frequency of CNS involvement in 407 non-resected and 227 resected lung cancer were 29 and 5% in small cell, 26 and 10% in adenocarcinoma, 23 and 18% in large cell, and 10 and 5% in squamous cell carcinoma. High risk factors overlapped with factors contributing long-term survival in small cell carcinoma. In resected adenocarcinoma, frequency was highest in poorly differentiated type. Differences were not significant in response rates of brain irradiation according to histologic type and to the dose ranging 3000 to 5000 rads. Survival time after the brain surgery was better than irradiation in patients whose primary lung tumor were resected and was worse in non-resected.

Twenty-five patients with involvement of the meninges by disseminated cancers were treated weekly with intraventricular thiotepa (10 mg) and methotrexate (10 mg) and with radiation therapy. Twenty-two of the 25 patients were clinically improved or stable. Malignant cells were cleared from the cerebrospinal fluid in 76% of the patients. Myelosuppression was the dose-limiting toxic effect, and neurotoxicity occurred in three patients. Combination chemotherapy employing thiotepa and methotrexate is a safe and effective treatment for neoplastic meningitis.

Urinary levels of the glucuronides of androsterone, aetiocholanolone, 5 alpha-androstane-3 alpha, 17 beta-diol and 5 beta-androstane-3 alpha, 17 beta-diol, were determined in twenty-two-post menopausal women both in spring and in autumn--winter. In sixteen women dehydroepiandrosterone sulphate plasma levels were also measured. Plasma DHAS levels as well as urinary metabolite excretion values were significantly higher in autumn--winter than in spring. This seasonal variability should be taken into account in long-term studies involving adrenal hormone levels, excretion or metabolism. Moreover this seasonal variability might be of relevance for the cyclical growth rate of hormone dependent tumours such as breast cancer.

Aclacinomycin-A was evaluated in a phase II trial for advanced head and neck cancer. A weekly infusion of 65 mg/m2 was used. Eighteen patients were entered. Fifteen patients were evaluable for response and toxicity. One was evaluable for toxicity only and two died before completion of a full course of therapy. One patient without previous chemotherapy achieved a PR in a neck node for a short time. Six patients had stable disease with a median to progression of 6 weeks. Major toxicity encountered was bone marrow suppression. Aclacinomycin-A appears to have no activity in squamous cancer of the head and neck.

The effects of chronic administration of cyclophosphamide and procarbazine on testicular function in the rat were examined. Cyclophosphamide produced disruption of the normal spermatogenic architecture that was dose and time dependent. Total ablation of the germinal epithelium was not achieved. Procarbazine produced more specific testicular damage. Multiple weekly injections of 100-200 mg/kg of procarbazine caused complete destruction of the spermatogenic cells, with no effect on Sertoli cells. Treated animals remained infertile for more than 4 months and showed no histologic evidence of recovery. This model may be useful for examining possible mechanisms to prevent spermatogenic damage associated with cancer chemotherapy in man.

Surveys of natural radiation indoors permit improved estimates to be made of exposures nationally and of regional variations. In the UK, there are clear indications that individual doses from radon decay products can be more than two orders of magnitude above the national average. A reliable risk factor for lung cancer allows the risks to individuals to be determined, and some members of the public are seen to be exposed to relatively high risks. The feasibility of reducing such risks requires a knowledge of the costs and effectiveness of remedial and preventive measures. There are also social and administrative considerations. The form of standards and the means of implementating them require careful consideration. A systematic approach to these issues is discussed in this paper.

Site-specific cancer mortality data for 20 United States cities have been abstracted from United States Government publications to explore further the hypothesis that fluoridation of water supplies causes cancer. Changes in mortality (standardised for age, sex, and ethnic group) between 1958-62 and 1968-72 in 10 fluoridated and 10 non-fluoridated United States cities have been examined. Of the seven sites (or groups of sites) examined, for only one is there a statistically significant difference between the fluoridated and non-fluoridated cities with respect to average change in mortality. This difference, for cancers of the genital organs, favours the fluoridated cities. These data do not provide evidence of a positive association between fluoridation of water supplies and cancer of any of the sites considered.

We developed a new surgical technique to manually resect the thoracic esophagus without thoracotomy and employed it in 31 patients with cancer of the esophagus who were considered poor risks for resection by traditional thoracotomy. Postoperative complications consisted of left hemothorax in seven patients, hoarseness in five, sternal osteomyelitis in three, a perforated trachea in one, and cardiorenal failure in three patients. No severe pulmonary complications occurred, and there were no hospital deaths. The long-term results of this surgical treatment have not been assessed.

Twenty-eight patients with inoperable or metastatic gastric cancer were treated with combination chemotherapy using 5-fluorouracil, adriamycin and mitomycin-C (FAM). Twenty-one patients had measurable disease parameters and 57 per cent of these experienced a major regression of disease following chemotherapy. The median survival of the responding patients was 13.2 months compared with 6.8 months in the group who did not achieve a major response. These results demonstrate the sensitivity of gastric carcinoma to this drug combination.

The usefulness of T antigen in the diagnosis of lung cancer (LCA), including early, was assessed by determining the in vitro delayed type hypersensitivity response to T(DTHR-T), and by measuring with a solid phase immunofluorescent assay the serum anti-T IgM response. Sensitivity of DTHR-T was 89% for 73 patients with LCA including 8/9 with Stage T1N0M0 disease, overall specificity was 95% for 212 healthy persons and those with non-CA disease. The humoral immune assay detected 31/35 (89%) LCA patients including 4/5 Stage T1N0M0 patients. Overall specificity was greater than 90% among 116 persons without CA.
