TITLE: Vindesine (VDS) monochemotherapy for non-small cell lung cancer: a report of 45 cases.

Fifty-three patients with non-small cell lung cancer were treated with vindesine. Of the 45 evaluable patients, 11 (24%) had a partial response. Responses were all evident within 6 weeks. Median duration of response was 8 weeks from documentation. Median survival was not improved significantly among responders (P less than 0.10, two-tailed test of significance). The most frequent toxic effect was leukopenia. The most troublesome toxic effect was peripheral neuropathy, with patients greater than 60 years old experiencing this more frequently.

TITLE: Photoradiation therapy in early gastrointestinal cancer.

Endoscopic photodynamic therapy has been used in the treatment of 19 cases of upper gastrointestinal cancer of which six were superficial esophageal and 13 were early gastric cancer. Six patients subsequently underwent surgical resection. Residual tumors were found in the resected specimens of one esophageal carcinoma and in the two early gastric carcinomas. Technical problems resulted in one failure. Follow-up ranged from 4 months to 3 years and 11 months with no tumor recurrence in either the operated or unoperated patients. Other delivery systems are currently under investigation.

The correlation of clinical, echo and morphological findings demonstrated ultrasound potentialities in the differential diagnosis of thyroid cancer. Ultrasound permitted the detection of nodes up to 3 cm in diameter. False-negative results were obtained in 6.5% of cases, there were no true false positive results. Adenomas are often characterized by mixed echogenicity of a node and hypoechogenic lining around it. Chronic nonspecific thyroid diseases are characterized by inhomogeneous echogenicity of the organ. Thyroid cancer is characterized by hypoechogenicity of a node with its inhomogeneous structure and irregular contours as if consisting of several nodules. The probability of malignancy grows with an increase in the number of these features combining in a particular patient.

The distribution of diastolic blood pressure (DBP) was compared between those who responded to a first written request to attend at their general practitioner's surgery for screening for hypertension and those who need more intensive effort encouraging them to attend. After excluding 25% of the population whose current address could not be traced, 92% of the remainder were eventually seen and screened. There was no association between diastolic blood pressure and effort needed to bring the patient to screening, and this applied to all patients both with and without a history of hypertension. Thus it appears that unlike screening for cervical cancer, screening for hypertension may not selectively tend to miss the groups most at risk.

By the use of a simplistic concept of the structure of heterochromatin, one can derive a formulation for the mechanism by which the phenomenon of V-type position effect can repress genic activity. This notion would explain how the Bar locus and other position effect examples cause genic repression. The mechanism, when extended, may also explain the process of cancer induction by hereditary means. In its most concise form the theory states that heterochromatin has a stabilized structure through special proteins, such as phosphorylated species that allows it the capacity to induce new domains of a "pseudo-heterochromatic" conformation to juxta-positional euchromatic segments for short distances by virtue of electrostatic and hydrogen bond forces.

TITLE: An unusual presentation of colon cancer.

A rare case of asymptomatic cancer of the ascending colon presenting as a subcutaneous groin mass is reported. At the time of right hemicolectomy with resection of the groin mass there was no sign of incontinuity spread or lymph blockage. Sequential determination of carcinoembryonic antigen levels (CEA) in serum reflected the reduction in tumor mass after surgery as well as subsequent recurrences of disease in the groin and in the lung after 2 years. Immunoperoxidase staining of the primary tumor and the metastases showed strong positivity for CEA confirming the origin of the serum CEA. There have been no signs of liver involvement. The route of spread of this unusual metastasis is not known.

Complications after definitive radiotherapy of prostatic cancer occur in 3 to 30% of cases. Most common is radiation injury of the urethra, the bladder or the rectum. Definitive radiation therapy of carcinoma of the prostata after staging lymphadenectomy has a considerable risk of lymphedema of the lower extremities. Two typical cases are used to illustrate the permanent damage done to the quality of life of the particular patient. The complication rate is decreased if only the prostate is irradiated without radiation of the regional lymphnodes. Interstitial radiation therapy using 125-Iodine-seeds yields comparable results with a tenfold decrease in the complication rate.

Fibroblast strains from 6 patients with ataxia telangiectasia (A-T) were found to be markedly hypersensitive to the cytotoxic action of the tumor promoter phorbol-12-myristate-13-acetate (PMA), their D37 values being 5 times lower than those of two normal controls. Two A-T heterozygous strains were slightly hypersensitive to PMA, while a third one showed normal sensitivity. It is concluded that the DNA lesion which is critical in A-T cells is an important component of the damage caused by PMA-induced free radicals and may play a role both in the tumor-promoting activity of PMA and the cancer proneness of A-T patients.

The proportion of primary hepatocellular carcinomas which are technically operable with hope of cure is small. Recommended therapy ranges from aggressive therapy to a nihilistic approach. There is little data available on the natural history of untreated tumours and we report such a case. A review of the literature regarding survival with primary hepatocellular carcinoma shows that the prognosis of patients with this tumour may vary considerably. Such variability has important implications for those involved in the design and interpretation of trials of treatment for hepatocellular cancer.

The occurrence of high cadmium levels in the digestive gland (hepatopancreas) of the crab Cancer pagurus is reviewed and its possible significance to human health is discussed. Previous attempts to isolate this cadmium in its native form are briefly reviewed and the present method used in this laboratory to isolate crab cadmium metallothionein is described. Quantitative amino acid analysis of the purified protein suggests that it has a total of 55 residues, including 19 Cys, and a molecular weight of approximately 5800. This yields an extinction coefficient at 220 nm for the protein in HCl, pH 1.5, of 50,000 and at 250 nm for the native cadmium protein in Tris-HCl, pH 8.4, of 92,000.

Frequency of CNS involvement in 407 non-resected and 227 resected lung cancer were 29 and 5% in small cell, 26 and 10% in adenocarcinoma, 23 and 18% in large cell, and 10 and 5% in squamous cell carcinoma. High risk factors overlapped with factors contributing long-term survival in small cell carcinoma. In resected adenocarcinoma, frequency was highest in poorly differentiated type. Differences were not significant in response rates of brain irradiation according to histologic type and to the dose ranging 3000 to 5000 rads. Survival time after the brain surgery was better than irradiation in patients whose primary lung tumor were resected and was worse in non-resected.

The primary diffused infiltrating type of colonic cancer is characteristically rare, and has a high rate of malignancy. A 54-year-old Japanese man was admitted to our hospital complaining of abdominal pain and decreased stools. Sigmoid colonic cancer was suspected as a result of the Ba-enema and colonic fiberscopy examination, but the biopsy results were negative. The sigmoid colon was resected extensively. Histopathologically, the cancer was diagnosed as being a signet ring-cell carcinoma of the sigmoid colon. Although the patient received postoperative chemotherapy, he died of carcinomata peritonitis 175 days after surgery.

Progesterone receptors (PR) from human breast cancer cells have been purified, and used as antigens to generate anti-PR monoclonal antibodies. Immunologic and in situ photoaffinity labeling methods were then used to study receptor structure, and we conclude that native human PR consist of two independent 8S receptors: one 8S subtype contains B-proteins (120,000 kDa) and the other contains A-proteins (94,000 kDa).

Twenty-five patients with involvement of the meninges by disseminated cancers were treated weekly with intraventricular thiotepa (10 mg) and methotrexate (10 mg) and with radiation therapy. Twenty-two of the 25 patients were clinically improved or stable. Malignant cells were cleared from the cerebrospinal fluid in 76% of the patients. Myelosuppression was the dose-limiting toxic effect, and neurotoxicity occurred in three patients. Combination chemotherapy employing thiotepa and methotrexate is a safe and effective treatment for neoplastic meningitis.

TITLE: A prospective study of mammographic parenchymal patterns and risk of breast cancer.

Breast parenchymal patterns have been assessed by the method of Wolfe in a prospective study in Guernsey. Women with P2 or DY grades had approximately four times the risk of developing breast cancer compared with those with N1 or P1 grades. Age, weight, parity and age at birth of first child are all related to the distribution of mammographic patterns. The combination of these variables with Wolfe grades may identify subsets of the population at very high risk.
