TITLE: Symptomatic choroidal metastases in breast cancer.

A series of 21 consecutive patients with carcinoma of the breast and symptomatic choroidal metastases was reviewed. All patients were symptomatic of choroidal metastases at presentation. Most patients had disease progression at other sites at the time of diagnosis. The incidence of cerebral metastases was not increased in this group compared to that in patients with metastatic breast cancer at other sites. Radiotherapy using a direct anterior field proved effective palliation for this disease site. Of the 32 eyes treated, serial visual acuity was documented in 23. Seventeen showed objective improvement and six showed stabilisation. The median duration of improvement was 11.5 months.

Logistic regression analyses, using information from the New York State Tumor Registry and area-level social class indicators, demonstrated that, in New York City, older Black, lower class women in public hospitals were 3.75 and 2.54 times more likely to have late stage breast or cervical cancer, respectively, than were younger White, high social class women in non-public hospitals.

In order to establish an effective cancer chemotherapy, theoretical analyses of chemoresistance factors in recurrent patients and possible approaches to curative chemotherapy were undertaken. Multidrug resistance of tumor cells in recurrent patients may hamper the development of new antitumor agents. Thus, we developed a new screening method (FACS method) which employs fresh clinical specimens. By this method, TAS-103 was found to be far more effective than 10 standard drugs and to be one of the most effective drugs among 20 or more new antitumor agents under study. Development of several effective drugs, such as TAS-103, might lead to a curative cancer chemotherapy.

TITLE: [Results of radiation and chemoradiation therapy in esophageal cancer]

Eight therapeutic variants were worked out for 602 patients with esophageal cancer: 61 patients entered in the control group. The latter received routine radiation therapy. The results of therapy of 496 patients were studied. The most promising method was chemoradiotherapy using multifractionation of a daily dose of irradiation, the 4-year survival rate of 20% being achieved. For combination with radiation therapy the following schemes of chemotherapeutic agents were used bleomycin-methotrexate-prospidine, bleomycin-adriamycin-methotrexate, dactinomycin-methotrexate-prospidine, methotrexate-prospidine. Intensive detoxication therapy with parenteral feeding is recommended to decrease the frequency of complications and to improve therapy tolerance.

TITLE: Enlarged lymph nodes: malignant or not?

Prospective controlled data of lymph node evaluation by endoscopic ultrasound in esophageal, gastric and pancreatic cancer is presented. Lymph node pattern, changes of boundaries and echogenicity were considered. Preoperative findings of endoscopic ultrasound were classified according to TNM staging. Only those cases with subsequent histologic examination of the resected specimen were entered into the study. Sensitivity of endoscopic ultrasonography (EUS) was 90% for esophageal and 87% for gastric cancer. The specificity was 72% and 88%, respectively. In spite of the overall satisfactory results of EUS in evaluation of lymph nodes, further improvement in detection and differentiation of benign and malignant nodes is required.

Cell survival is normally mediated by factors in the extracellular environment, whereas genetic changes that constitutively activate intracellular survival pathways often occur in cancer. It is suggested that a Ras/Raf/MAP kinase-dependent pathway is critical for cell survival. Apoptosis results from loss of these survival factors or deregulation of survival pathways. If protein kinase cascades mediate survival, then it is likely that phosphatases mediate apoptosis. Potential targets for dephosphorylation include regulators of ion homeostasis as these have been implicated in the regulation of endonucleases associated with apoptosis. Survival factors also modulate anticancer drug response and understanding these pathways may improve therapy.

TITLE: [Results of tamoxifen treatment in patients with advanced breast cancer]

The results of advanced breast cancer therapy with tamoxifen have been evaluated. The objective response to the drug has been achieved in 32 (31.7%) patients, and a 2-year survival in 52% of cases. A correlation between the localization of lesions and response rate has been found. The best results have been achieved in localized lesions and lymph nodes metastases. Low toxicity of the drug enables achievement of results comparable to those seen in all types of the hormonal therapy, except androgens.

Using Avidn-biotin-peroxidase complex (ABC) immunohistochemical method, 68 gastric cancer specimens and 20 non-cancerous stomach specimens were examined for ER. In 38 of 68 gastric cancer specimens, ER concentrations were also biochemically determined by Dextran-coated charcoal (DCC) assay. The results showed that ER was absent in noncancerous stomach tissues. ER was present in 21 (30.9%) of gastric cancers. The ER positive cases were chiefly poorly differentiated gastric cancers. There were no significant statistical differences in the patient's age or sex, size of primary tumor and the ER positive rates. The results indicate that endocrine therapy might be useful for the ER positive gastric cancer patients.

We have started a multicentre trial to study the possibilities of first-trimester maternal serum screening for fetal chromosomal abnormalities. Maternal blood samples were obtained before 13 weeks of gestation. We present the preliminary results of the first 950 patients on alpha-fetoprotein (AFP). Results on cancer antigen 125 (CA 125) in Down's syndrome and normal pregnancies are also presented. We conclude that the results on AFP are promising and that CA 125 might be predictive for fetal Down's syndrome.

A 75-year-old male was admitted one year after surgery for advanced gastric cancer. He was diagnosed as having multiple liver metastases with elevated serum CEA level. TITLE: High birthweight as a predictor of prostate cancer risk.

In a follow-up study of a cohort of 50-year-old men who were born in 1913 and were living in Gothenburg, Sweden, we found an association between birthweight and prostate cancer. Of 366 men with known birthweight, there were 21 patients with prostate cancer. The incidence is about five times higher in the highest quartile of birthweight than in other birthweight groups. The findings indicate that aspects of the pre- and perinatal period may affect the risk of subsequent prostate cancer.

Mammographic detection and analysis of microcalcification is a key component in the detection of early breast cancer. A casting configuration or granular morphology has a strong association with malignancy. These are relatively easily recognised, but with the proliferation of mammographic screening, other varieties of calcification are being encountered which may pose diagnostic dilemmas. This case report describes the findings in a patient with Pseudoxanthoma Elasticum (PXE) who presented in this way. To our knowledge, this has not previously been documented. Such calcification is not usually visible on conventional radiographs.
