Anne Mullens used a recent fellowship provided by the Atkinson Foundation to take an in-depth look at euthanasia in the Netherlands. During her time in Holland, she discussed the issue with doctors who support and oppose euthanasia. She accompanied a doctor as he visited a patient who was dying of cancer and was beginning to consider the possibility of euthanasia. She talked to a nonphysician who is adamantly opposed to euthanasia and carries a card stating that. She visited a hospital in Amsterdam that has received requests from foreigners seeking euthansia. Mullens offers a comprehensive look at an issue that continues to provoke strong feelings among Canadian physicians and patients.

The incidence of polymyalgia rheumatica and temporal arteritis is increasing, mainly in elderly people. The risk of cancer during a lifetime is high in patients with positive biopsy of the temporal artery, but polymyalgia rheumatica and temporal arteritis are not to be considered as paraneoplastic syndromes. Temporal arteritis can appear in patients under treatment for polymyalgia rheumatica, and polymyalgia rheumatica can be accompanied by arthritis. Various new aspects of diagnoses and treatment are discussed.

This survey of 517 Australian nurses suggests that they have some of the same educational needs as nurses in North America, such as how to assess pain intensity and how to select a dose of an opioid analgesic that is appropriate for the individual patient.

TITLE: Quality of life in lung cancer surgical adjuvant trials.

The Functional Living Index-Cancer (FLIC) was administered to 438 patients in the Lung Cancer Study Group on whom long-term follow-up was available in 1993. Across all trials, the total FLIC score was predictive for survival even when corrected for extent of disease, although individual items on the FLIC were not. There was no significant impact of a short course of chemotherapy on quality of life. The FLIC is a reliable means of assessing quality of life in lung cancer surgical adjuvant trials.

Cathepsin B is a lysosomal cysteine protease of the papain family. In a previous randomized clinical trial against curatively resected gastric cancer, we compared the effect of immunochemotherapy to chemotherapy and obtained significantly better survival and disease-free survival time (DFS) in a combined therapy group. Although DFS was analysed with a conventional approach in which the onset of relapse was defined as the diagnosis of relapse, we considered it necessary to re-analyse the data and compare the results of an interval-censored approach with the conventional approach, in order to examine whether the conclusion might be altered depending on the approach.

Immunotoxins and chimeric toxins are hybrid molecules constituted of antibodies, growth factor or cytokines coupled to peptide toxins. They are designed to selectively eliminate tumor cells. Some of these chimera have been shown to induce complete tumor regressions of human tumor xenografts in immunodeficient mice. In clinical trials, higher anti tumor response were observed in lymphoma, brain tumor, breast and colon cancers. Problems arose with normal tissue toxicity and the production of neutralising antibodies. Should the latest recombinant toxins conceived by rationale designed, solved these problems, chimeric toxins would be an alternative approach to target tumor cells and vascular endothelial cells in solid tumors.

In contrast to cervical cancer, no routine mass screening programs for the early detection of endometrial cancer have been organized. All of the methods used by cytologists for studying endometrial changes have limitations. Cervicovaginal smears allow diagnosis of, at best, 60% to 70% of carcinomas of the endometrium. Intrauterine sampling is another possible method. It consists of direct aspiration of uterine cavity, scraping or brushing of the mucosa or uterine washing. Any method of endometrial sampling is imperfect, even dilatation and curettage has been shown to miss endometrial carcinoma. The lack of an effective, inexpensive and easy-to-use sampling method is the reason for the nonexistent decline in the incidence of invasive endometrial carcinoma.

Mean glandular dose limits in mammography are defined in terms of a "standard breast". However, accrediting bodies do not always use the same standard breast. In this study, dose was measured following the guidelines of the National Program for the Early Detection of Breast Cancer (NPEDBC) and the Royal Australasian College of Radiologists (RACR). The RACR dose was found to be 32% less than the NPEDBC dose. This highlights the need for breast characteristics to be included in any statement of mean glandular dose.
