TITLE: [Primary liver cancer coincident with schistosomiasis japonica]

The etiologic relationship of a parasitic liver disease to primary liver cancer has long been debated, and reported is a case that has been encountered of a primary liver cancer coincident with Schistosomiasis japonica. A fifty-nine year-old-man was diagnosed as having a primary liver cancer complicated with liver cirrhosis. A posterior segmentectomy was performed and a microscopic examination revealed a primary liver cancer with multiple ovae of Schistosoma japonicum in the fibrous stroma of the portal spaces. Discussed is the etiologic relationship of Schistosomiasis japonica to the primary liver cancer.

A case of recurrent squamous cell carcinoma under a skin flap as confirmed and delineated by magnetic resonance imaging (MRI) is described. This report illustrates a potentially new indication for MRI in clinical dermatology, namely, to define skin cancer recurrences under skin flaps and grafts and to determine the extent of underlying tissue involvement. The cost-effectiveness of MRI and the use of gadolinium, an MRI contrast agent, in the evaluation of skin tumors are also addressed.

Transplantable rat tumours (sarcoma M-1, sarcoma 45, alveolar liver cancer PC-1 and Pliss' lymphosarcoma) were used to study chlorin e6 accumulation in tumours and its photodynamic effect. Tumours were irradiated by krypton ion laser light (647 and 676 nm; 90 J cm-2) 15 min and 24 h after chlorin e6 injection at doses of 5 and 10 mg kg-1. The relationship between some morphological peculiarities of these tumour strains, photosensitizer accumulation in tumours and their response to the photodynamic treatment is discussed.

Interleukin-2 (IL-2) immunotherapy resulted in the development of inflammatory arthritis in three male patients with metastatic cancer. Two patients developed a clinical picture consistent with rheumatoid arthritis. A third patient with a remote history of Reiter's syndrome developed a recrudescence of an inflammatory arthritis after treatment with IL-2. The clinical, laboratory, and histologic data on the patients and the potential pathophysiologic mechanisms are discussed.

Although colorectal cancer is the second leading cause of death in the US, the majority of diagnoses are made at an advanced stage, and screening detects 70% at an early stage, controversy has existed because of a lack of experimental controlled trial evidence showing a decrease in mortality. Indirect and now direct evidence supports the current screening guidelines of the American Cancer Society and the National Cancer Institute as reasonable and should be continued.

The comparison of blood levels of oestradiol, testosterone, prolactin, and sex-hormone binding globulin (SHBG) was made of 3250 rural Chinese and 300 British women, aged 35 to 64. To reduce the number of assays performed the blood samples were combined so as to form 390 and 30 pools, respectively. The Chinese had significantly less oestradiol and testosterone. Prolactin levels were similar in both races. SHBG was significantly lower in postmenopausal British women. In the Chinese women, testosterone was positively and prolactin negatively correlated with breast cancer mortality.

This quasi-prospective study of 2163 women attending a breast-screening clinic for a routine medical check-up indicates that certain types of coping strategies and personality dispositions predispose some women to an increased risk of developing breast cancer following the occurrence of a major life-event such as bereavement or other loss-related event. Regular exposure to stress situations appears to reduce the risk of a malignancy; experiencing a single, major life event was found to be potentially much more damaging, particularly if the individual was unable to externalize her emotions and obtain appropriate help and counselling.

A new method of transrectal biopsy of the prostate using an Iowa trumpet is introduced. This method allows the urologist to biopsy what he or she palpates during a standard digital rectal examination. The methodology of using this biopsy guide is explained. Among 66 patients with a palpable prostatic abnormality 31 cancers were diagnosed with this new method, while only 15 cancers could be diagnosed with standard ultrasound guided biopsy techniques. However, of these 31 cancers 8 would have been missed without the aid of ultrasound. No unusual complications were noted with this new biopsy method.

204 pts with breast cancer were compared to 208 pts of similar age, with pathologies other than cancer, to assess the frequency of biological and social factors related to breast cancer, in Chilean females. The pts with breast cancer were in the 40 to 65 year old group, had a higher frequency of AB blood group, lower pregnancy and delivery rates and later pregnancies. Age of menarche and menopause, prolonged breast feeding, oral contraceptives and nutritional state were not related to a higher frequency of breast cancer. Further studies that include the analysis of previous breast pathology, diabetes, psychosomatic factors, alcohol and caffeine consumption, tobacco and others are suggested.

Endometriosis is rarely encountered by the general surgeon but may on occasion mimic general surgical disease. Although usually diagnosed easily because of cyclic symptoms and typical physical findings, six atypical cases are presented. These cases were diagnosed as appendicitis (2), subcutaneous mass (2), pelvic wall malignancy, and lower gastrointestinal hemorrhage. Diagnosis was ultimately made by laparotomy or excisional biopsy in five cases and by fine needle aspiration in one. Resection of the lesion, if feasible, is often curative. If resection is not feasible, therapy depends on the severity of the symptoms, and the patient's desire for fertility. Recognition of the typical appearance of an endometrioma may avoid mistaking it for cancer.

A new investigative modality, cervicography, has been advocated for cervical screening. In the first 51 patients referred for colposcopy because of an abnormal cervicogram, none had invasive cancer and 75% had preinvasive cancer. The cervicogram appears superior to cytology but inferior to colposcopy in the detection of cervical pathology. Based on the available evidence, however, cervicography cannot be recommended for universal screening. It may have a role in the follow-up of patients with a mildly abnormal cervical smear, but the optimum management remains early referral for colposcopy.
