In 306 patients, fiberpanendoscopy and radiology were performed within a time interval of 7 days or less. In 196 cases, radiology was performed before endoscopy and in 110 cases after endoscopy. In cases with histologically unproven divergent findings, follow-up studies were performed. In the diagnosis of gastric ulcer and gastric cancer, endoscopy with biopsy was more accurate than radiology. In the diagnosis of hiatal hernia, radiology was more accurate. Important additional findings difficult or impossible to observe with endoscopy were present in 36% of the radiological examinations. Thus, the first examination should be endoscopy when ulcer or carcinoma is suspected. In patients with uncharacteristic epigastric symptoms, radiology should be performed first.

Retrospective data on dietary habits, employment history and tobacco use were obtained from 569 bladder cancer patients and 1025 age-matched controls admitted to Roswell Park Memorial Institute. Sex-adjusted relative risks revealed increases in risk for lower levels of an index of vitamin A intake. A similar pattern of risk elevation was associated with infrequent milk and carrot intake. Some elevation of risk was found for heavy coffee drinking but the apparent protective effect for milk consumption was not found to be a spurious result of lower coffee intake. Neither was the role of vitamin A explained by its relationship with smoking or employment in high risk occupations. Some association of bladder cancer with infrequent consumption of cruciferous vegetables was also observed. The findings of this investigation are consistent with tumor inhibition by retinoids in animal studies and the low risk associated with vitamin A in epidemiologic studies of lung cancer.

A total of 134 patients suspected of having pancreatic cancer were given preoperative ultrasonic examinations. A total of 54 cases had a final diagnosis of pancreatic cancer histologically confirmed. The diagnostic sensitivity and specificity of the ultrasonographies were 76% and 91% or 87% and 74%, depending on whether doubtful results were called abnormal or normal. "Doubtful" results were defined as those that stated that pancreatic cancer was "possible" or "could not be excluded." When these results were divided into abnormal and normal groups on the basis of the patients' preoperative serum carcinoembryonic antigen and pancreatic oncofetal antigen levels, the sensitivity and specificity of the combined tests may prove to be a more accurate means of screening patients for pancreatic cancer than ultrasonography alone.

Incidence of colorectal cancer in Israel was studied in 1035 patients who were divided by ethno-geography into a Euro-American group and an Afro-Asian group. The majority of patients were in the age groups over 50 years, with a markedly higher incidence rate of disease in the Euro-American group. In the younger age group the incidence rate of the disease was significantly higher in the Afro-Asian group. These findings, if verified by others, must lead to revision of our present concepts of epidemiology of colorectal cancer.

This study investigated a patient's ego strength (Es) at the time of an initial cancer diagnosis and its relationship, over time, to mood disturbance, vulnerability, self-reported physical symptom totals, current concerns, coping strategies, and effectiveness in the resolution of problems. The subjects were 163 newly diagnosed male and female cancer patients representing five primary tumor sites. All patients were seen for an initial evaluation, at which time they completed Barron's Es scale, the Profile of Mood States, the Inventory of Current Concerns, and a semistructured interview. Ratings on patient vulnerability, coping strategies, and problem resolutions were made at each of the five follow-ups. Results showed that psychosocial adaptation to cancer was related to a patient's ego strength. Es correlated positively with a patient's use of effective coping strategies. The concept of Es and problems with Es assessment were discussed within a cognitive-ego analytic frame of reference.

Thirty-five patients with Stage I carcinoma of the lung were tested postoperatively to assess lymphoproliferative responses. Depressed lymphocyte proliferation (LP) responses to alloantaigen in the mixed leukocyte culture (MLC) as measured by thee relative proliferation index (RPI) were associated with a significantly shorter disease-free interval. In this group of patients, the immunologic responses predicted subsequent clinical course better than the TNM classification or the histological type of the tumor, and therefore this procedure appears promising for improved staging of patients with early stages of lung cancer (stage I lung cancer and T1N0M0). The depressed response to alloantaigen was a more sensitive discriminator of disease recurrence than PHA alone or even conbined with PHA.

Of 2,141 patients with scleroderma who were seen at the Mayo Clinic, Rochester, Minn, between 1959 and 1975, 78 had 87 internal malignancies. The relative frequency of the types of cancers was similar to that for the general population. The increased numbers of patients with breast and uterine carcinomas were consistent with the female preponderance in scleroderma. Contrary to previous reports, carcinoma of the lung was not the most frequent malignancy associated with scleroderma. The lymphoma-leukemia malignancies were second only to breast carcinoma in frequency, comprising about 17% of the malignancies. Both conditions developed within a three-year period in 68% (45/66) of our patients affected with systemic scleroderma, and this subgroup comprised a high mortality group. This study is not an epidemiologic one in the strictest sense, but it is a review of the experience of the Mayo Clinic with patients who have had both scleroderma and an internal malignancy.

Cutaneous metastases are seldom encountered in digestive cancers. The mechanism of their formation explains their low rate. The metastases spread mainly by the lymphatic system (especially in diffuse, spotty or pseudo-elephantiasic forms and in regional forms), however numerous lymph node filters found through out the lymphatic system limit the progression of neoplasic cells. On the contrary, metastases most often spread by retrograde involvement due to blocking of the lymphatic system. As for hematogenous metastases (generalized nodular forms or aberrant localizations), they are observed even less frequently than lymphatic ones; metastatic embolisms may effectively become blocked in capillaries, especially in the lungs and the liver, which constitutes a double barrier before the skin may be involved.

The cellular immune response has been reported to be impaired in patients with renal cancer and to be increased by removal of the tumor bulk. T lymphocyte levels in the vein of the neoplastic kidney were investigated in 4 patients to assess the presence of a T lymphocyte inhibiting factor produced by the tumor. T lymphocyte levels resulted in significantly decreased tumorous venous blood, whether compared to arterial, contralateral or peripheral blood. A similar result was not found in 10 control patients. Some hypotheses on the nature of this T lymphocyte inhibition are discussed.

TITLE: Testicular function after combined chemotherapy for metastatic testicular cancer.

In 10 patients cured for metastatic testicular cancer by combination chemotherapy serum hormone levels and serum agglutinating antibodies were analysed 12 to 35 months after discontinuation of the treatment. Together with these examinations sperm analysis was done. All patients had increased levels of follicle stimulating hormone (FSH). Serum testosterone was usually in the lower part of the normal range (below 20 nmol/l). In eight patients the serum agglutinating antibodies were normal, while two patients had increased levels. In all patients azoospermia was observed, indicating long-lasting infertility in patients testicular cancer treated by combination chemotherapy. The possible importance of cryopreservation prior to start of chemotherapy is discussed.

Carcinoembryonic antigen and activities of glucosephosphate isomerase (EC 5.3.1.9), gamma-glutamyltransferase (EC 2.3.2.2), and lactate dehydrogenase (EC 1.1.1.27) were measured in aqueous extracts of fetal, normal adult, and malignant human colon tissues. Fetal colon, as well as primary and metastatic colon tumor tissue, showed higher activities of these analytes than did normal adult human colon. Liver metastases of colon cancer gave the highest values, normal adult human colon the lowest. Statistically, these differences were more striking in the case of carcinoembryonic antigen and glucosephosphate isomerase than for gamma-glutamyltransferase or lactate dehydrogenase. In contrast to the other markers, gamma-glutamyltransferase activity was lower in fetal organs than in normal adult colon and colon tumors. These results are consistent with earlier observations that activities of these markers are significantly increased in the blood of patients with metastatic colon cancer.

Synchronous dual primary lung carcinomas were found in 8 of 482 patients with lung cancer, an incidence of 1.6 percent. Of the 8 cases, 7 were unilateral and one bilateral. Three showed dissimilar histologic patterns occurring in combinations of squamous-cell carcinoma and adenocarcinoma. In 5 cases, two separate tumors were noted with identical histologic patterns in combinations of squamous-cell, giant-cell, large-cell, adeno-. and undifferentiated carcinoma. The patients were heavy smokers and this possibly may have been a factor in the development of carcinoma at more than one site.

Much has been written about working with dying patients. To evaluate counseling, 120 terminally ill cancer patients were randomly assigned to counseling or no counseling and studied before random assignment and at 1, 3, 6, 9, and 12 months on quality of life variables (alienation, depression, locus of control, life satisfaction, self-esteem) as well as functional status and survival. Counseled patients changed significantly in comparison to controls and in a favorable direction by 3 months. The purpose here was to see if older and younger cancer patients differed at baseline and if outcomes of counseling differed by age. Patients under age 60 were compared with those 60 and over. There was no multivariate difference at baseline but univariate differences of more disability and less life satisfaction in the older group. Overall, response to therapy was similar in old and young, with both improving.
