Anne Mullens used a recent fellowship provided by the Atkinson Foundation to take an in-depth look at euthanasia in the Netherlands. During her time in Holland, she discussed the issue with doctors who support and oppose euthanasia. She accompanied a doctor as he visited a patient who was dying of cancer and was beginning to consider the possibility of euthanasia. She talked to a nonphysician who is adamantly opposed to euthanasia and carries a card stating that. She visited a hospital in Amsterdam that has received requests from foreigners seeking euthansia. Mullens offers a comprehensive look at an issue that continues to provoke strong feelings among Canadian physicians and patients.

The incidence of polymyalgia rheumatica and temporal arteritis is increasing, mainly in elderly people. The risk of cancer during a lifetime is high in patients with positive biopsy of the temporal artery, but polymyalgia rheumatica and temporal arteritis are not to be considered as paraneoplastic syndromes. Temporal arteritis can appear in patients under treatment for polymyalgia rheumatica, and polymyalgia rheumatica can be accompanied by arthritis. Various new aspects of diagnoses and treatment are discussed.

The etiologic relationship of a parasitic liver disease to primary liver cancer has long been debated, and reported is a case that has been encountered of a primary liver cancer coincident with Schistosomiasis japonica. A fifty-nine year-old-man was diagnosed as having a primary liver cancer complicated with liver cirrhosis. A posterior segmentectomy was performed and a microscopic examination revealed a primary liver cancer with multiple ovae of Schistosoma japonicum in the fibrous stroma of the portal spaces. Discussed is the etiologic relationship of Schistosomiasis japonica to the primary liver cancer.

For example, the World Health Organization states that unrelieved cancer pain is an international problem. Lack of education of health care professionals, including nurses, is frequently cited as a major reason for under-treatment of pain. This survey of 517 Australian nurses suggests that they have some of the same educational needs as nurses in North America, such as how to assess pain intensity and how to select a dose of an opioid analgesic that is appropriate for the individual patient.

The Functional Living Index-Cancer (FLIC) was administered to 438 patients in the Lung Cancer Study Group on whom long-term follow-up was available in 1993. Across all trials, the total FLIC score was predictive for survival even when corrected for extent of disease, although individual items on the FLIC were not. There was no significant impact of a short course of chemotherapy on quality of life. The FLIC is a reliable means of assessing quality of life in lung cancer surgical adjuvant trials.

Cathepsin B is a lysosomal cysteine protease of the papain family. Transplantable rat tumours (sarcoma M-1, sarcoma 45, alveolar liver cancer PC-1 and Pliss' lymphosarcoma) were used to study chlorin e6 accumulation in tumours and its photodynamic effect. Tumours were irradiated by krypton ion laser light (647 and 676 nm; 90 J cm-2) 15 min and 24 h after chlorin e6 injection at doses of 5 and 10 mg kg-1. The relationship between some morphological peculiarities of these tumour strains, photosensitizer accumulation in tumours and their response to the photodynamic treatment is discussed.

Interleukin-2 (IL-2) immunotherapy resulted in the development of inflammatory arthritis in three male patients with metastatic cancer. Two patients developed a clinical picture consistent with rheumatoid arthritis. A third patient with a remote history of Reiter's syndrome developed a recrudescence of an inflammatory arthritis after treatment with IL-2. The clinical, laboratory, and histologic data on the patients and the potential pathophysiologic mechanisms are discussed.

Although colorectal cancer is the second leading cause of death in the US, the majority of diagnoses are made at an advanced stage, and screening detects 70% at an early stage, controversy has existed because of a lack of experimental controlled trial evidence showing a decrease in mortality. Indirect and now direct evidence supports the current screening guidelines of the American Cancer Society and the National Cancer Institute as reasonable and should be continued.

In a previous randomized clinical trial against curatively resected gastric cancer, we compared the effect of immunochemotherapy to chemotherapy and obtained significantly better survival and disease-free survival time (DFS) in a combined therapy group. Although DFS was analysed with a conventional approach in which the onset of relapse was defined as the diagnosis of relapse, we considered it necessary to re-analyse the data and compare the results of an interval-censored approach with the conventional approach, in order to examine whether the conclusion might be altered depending on the approach.

The comparison of blood levels of oestradiol, testosterone, prolactin, and sex-hormone binding globulin (SHBG) was made of 3250 rural Chinese and 300 British women, aged 35 to 64. To reduce the number of assays performed the blood samples were combined so as to form 390 and 30 pools, respectively. The Chinese had significantly less oestradiol and testosterone. Prolactin levels were similar in both races. SHBG was significantly lower in postmenopausal British women. In the Chinese women, testosterone was positively and prolactin negatively correlated with breast cancer mortality.
