A man with large-cell carcinoma of the lung, cerebral meningioma, occult adenocarcinoma of the prostate, and follicular adenoma of the thyroid developed symptomatic, rapidly progressive hypereosinophilia with abnormalities of eosinophil ultrastructure and bone marrow karyotype (45,X,15q22-). Although the patient's eosinophilia defied strict classification as idiopathic hypereosinophilic syndrome (HES), simple tumor-associated eosinophilia, or eosinophilic leukemia, it appeared to be incited by the lung cancer and quickly acquired malignant independence. The family had an excess of prostate cancer and lymphoproliferative neoplasms.

Fine-needle aspiration in conjunction with a transrectal core biopsy was performed on a 58-year-old man who presented with a prostate nodule. The aspirate contained many cell clusters that were negative for prostatic acid phosphatase and cytologically appeared to be basal cells. The entity of basal cell hyperplasia of the prostate is discussed with reference to its appearance in aspiration cytologic examination. Emphasis is placed upon the possibility that these cells may be mistaken for cancer cells.

TITLE: Elevated growth hormone levels in sera from breast cancer patients.

The concentrations of growth hormone (GH) and prolactin (PRL) in the serum of 42 breast cancer patients were determined by radioimmunoassay (RIA). Forty percent of the patients had elevated GH levels while only 17% had elevated PRL levels. These findings suggest a relationship between GH and breast cancer; a weaker correlation exists between PRL and this malignancy. In addition, total lactogens in the serum were measured by a bioassay (BA). The BA/RIA (GH + PRL) ratio was greater in the breast cancer patients than the controls, indicating that variant forms of the hormones with higher than normal biological activity might be present.

All Alaskan Native patients diagnosed with leukemia, lymphoma, multiple myeloma, and related cancers during the time period 1969-83 were identified. Of 72 biopsy-confirmed cases, lymphoma was diagnosed in 37 (including mycosis fungoides in 5), leukemia in 22, multiple myeloma in 11, and other lymphoreticular cancers in 2. Compared to the rates for U.S. whites, incidence rates were low for leukemia and lymphoma (especially Hodgkin's disease), but not for multiple myeloma in males. There was an apparent increase over time in lymphomas in males, while no increase occurred in females, nor for leukemia or multiple myeloma in either sex.

The primary diffused infiltrating type of colonic cancer is characteristically rare, and has a high rate of malignancy. A 54-year-old Japanese man was admitted to our hospital complaining of abdominal pain and decreased stools. Sigmoid colonic cancer was suspected as a result of the Ba-enema and colonic fiberscopy examination, but the biopsy results were negative. The sigmoid colon was resected extensively. Histopathologically, the cancer was diagnosed as being a signet ring-cell carcinoma of the sigmoid colon. Although the patient received postoperative chemotherapy, he died of carcinomata peritonitis 175 days after surgery.

This report is a retrospective analysis of 62 patients treated for early glottic carcinoma during the six-year period of January 1982 through December 1987. The authors report that the cancer has been controlled in 100% of patients. For invasive stage 1 tumors the radiation cure rate is 42 of 45 patients or 93%, and voice quality after treatment was usually normal. Surgically treated tumors are usually confined to one vocal cord. Primary surgery with laser or cord stripping ultimately led to tumor control and voice preservation in all eleven patients treated although two required salvage with radiotherapy and mild but acceptable hoarseness was a usual sequel.

In a case control study of the prevalence of palmar keratoses in 69 bladder cancer patients, 66 lung cancer patients, and 218 hospital controls palmar keratoses were more common in the cases, especially among those with cancer of the bladder. Above the age of 50 years, 87% of bladder cancer patients and 71% of lung cancer patients but only 36% of the controls had one or more keratoses. The prevalence increased with age and keratoses were more common in males. No features could be found which distinguished the keratoses in cases from those in controls.

Androsterone and aetiocholanolone were measured in urine specimens obtained from 218 women with early breast cancer. Patients who excreted less than the median amount of these steroids had a significantly higher recurrence rate (P less than 0.005) than patients excreting more than the median value. The relationship between androgen excretion and recurrence is stronger in pre-menopausal women that in the post-menopausal group. The androgen assays are still associated with recurrence rates when patients are stratified by pathological stage, but no clear relationship was found with histological grade.

Two cases of pre-invasive in situ carcinoma of the pancreas treated by surgery are reported. In each case the resection was carried out because of the probable diagnosis of invasive carcinoma of the pancreas. In situ carcinoma of the pancreas is not included in standard clinical classifications of pancreatic cancer. This is an important omission because of the prognostic implications of this diagnosis, and because of the difficulties in interpreting fine needle aspiration cytology or histology on needle biopsy in such cases.

High dose MAP (medroxyprogesterone acetate) and TAM (tamoxifen) were administered orally to 31 postmenopausal patients with advanced breast cancer. CR (complete remission) was achieved in 1 (3%) and PR (partial remission) in 11 (32%) patients for a median duration of 12 months. The treatment was well tolerated and in no instance was interrupted because of toxicity. This study does not support the hypothesis that the combination of MAP and TAM gives better results than the single agents.

Clinical staging and morphological grading are very important for treatment of prostatic carcinoma. In 1980 a German pathological-urological study group of prostate cancer completed a new cytological grading system. This system has grades I-III of malignancy, based on six cellular criteria, whose relevance for prognosis was shown by cellkinetical investigations. The present study demonstrates less "undergrading" from transrectal fine needle biopsy and cytological diagnosis than from punch-biopsy and histological diagnosis. The cytological grade correlates well with the local extent of the tumor and metastases to pelvic lymph nodes. Despite of the wellknown problems with morphological grading on a biopsy it gives us very important information.

Twenty-five patients with metastatic breast cancer in progression after prior chemotherapy +/- hormonotherapy were treated with etoposide 50 mg/m2 i.v. days 1 to 5 every 21 days and mitomycin-C 10 mg/m2 i.v. day 1 every 42 days. A partial response (PR) occurred in 10 patients with an overall response rate of 40% (47% when only the 21 patients evaluable after 3 courses or more were considered). The median duration of PR was 10.5 months (range 3-31). The soft tissue metastatic sites were the most responsive. Toxicity was mild.

TITLE: Health status in chronic disease: a comparative study of rheumatoid arthritis.

A self-administered questionnaire has been used to measure and compare health status in rheumatoid arthritis (RA) and 5 other chronic diseases. Subjects with RA, hypertension, cancer, diabetes, cardiac disease, or pulmonary disease were assessed along 11 aspects of health status. Results showed significant differences in health status across the 6 diseases, particularly for physical function and pain. The psychological status measures were similar. A summary estimate indicated that RA and pulmonary diseases produced poorer health status than the others. This study confirms the severe negative health status associated with chronic RA.

Forty-three patients with evaluable metastatic breast cancer refractory to hormonal agents and extensive combination chemotherapy including doxorubicin were treated with mitomycin, 20 mg/m2 intravenously every 6 weeks. There were five partial responses (12%) and three minor responses (7%), with a mean time to progression of 5 months and 3.5 months, respectively. Thrombocytopenia was the major dose-limiting toxicity, and myelosuppression was cumulative. Cardiac dysfunction was observed in 12% of patients. Mitomycin had some antitumor activity in this group of metastatic breast cancer patients refractory to extensive combination chemotherapy including doxorubicin.

It has recently been reported that a series of testis cancers shows a temporal cycle in birth dates with a peak in certain months. This observation has been tested by an examination of a larger series covering all testis cancers diagnosed in the years 1971-84 in England and Wales, in men born in 1940 or later. Limited evidence was found of a 4-monthly cycle, but this was due to a 2-monthly cycle shown by teratomas in men born in the years 1945-49. No evidence was found for such a cycle outside this period, nor for a peak in any particular month.
