Nulliparous women have a greater risk of developing breast cancer than women who have borne children, but so far no functional differences in breast tissue have been reported between parous women and nulliparae. Macroscopically and histologically normal breast tissue was obtained from 74 women of reproductive age during biopsy of benign breast lesions and was examined for the presence of plasma cells by immunfluorescence. Immunoglobulin synthesis was detected by an in-vitro culture technique. Synthesis of IgA was detected in 81% of specimans of IgG in 45%, and of IgM in 3%. IgA synthesis much more intense than IgG or IgM synthesis. Plasma cells containing IgA were seen in 71% of the specimens examined, and 88% of specimens had deposits of IgA in the ductules. The findings were not significantly incluenced by the nature of the condition necessitating biopsy or by oral contraception. Nulliparous women showed no cyclical changes, but among parous women IgA synthesis was more intense during luteal phase of the menstrual cycle. This suggests that after the first pregnancy the breast is more sensitive to progesterone.

A man with large-cell carcinoma of the lung, cerebral meningioma, occult adenocarcinoma of the prostate, and follicular adenoma of the thyroid developed symptomatic, rapidly progressive hypereosinophilia with abnormalities of eosinophil ultrastructure and bone marrow karyotype (45,X,15q22-). Although the patient's eosinophilia defied strict classification as idiopathic hypereosinophilic syndrome (HES), simple tumor-associated eosinophilia, or eosinophilic leukemia, it appeared to be incited by the lung cancer and quickly acquired malignant independence. The family had an excess of prostate cancer and lymphoproliferative neoplasms.

Fine-needle aspiration in conjunction with a transrectal core biopsy was performed on a 58-year-old man who presented with a prostate nodule. The aspirate contained many cell clusters that were negative for prostatic acid phosphatase and cytologically appeared to be basal cells. The entity of basal cell hyperplasia of the prostate is discussed with reference to its appearance in aspiration cytologic examination. Emphasis is placed upon the possibility that these cells may be mistaken for cancer cells.

In 306 patients, fiberpanendoscopy and radiology were performed within a time interval of 7 days or less. In 196 cases, radiology was performed before endoscopy and in 110 cases after endoscopy. In cases with histologically unproven divergent findings, follow-up studies were performed. In the diagnosis of gastric ulcer and gastric cancer, endoscopy with biopsy was more accurate than radiology. In the diagnosis of hiatal hernia, radiology was more accurate. Important additional findings difficult or impossible to observe with endoscopy were present in 36% of the radiological examinations. Thus, the first examination should be endoscopy when ulcer or carcinoma is suspected. In patients with uncharacteristic epigastric symptoms, radiology should be performed first.

There are certain problems associated with investigation of the geographic variation in the occurrence of cancer of the uterine corpus. Mortality rates are especially unsuitable for this because of the large proportion of deaths from uterine cancer which are not specified as to whether they originated in the cervix or the corpus. Incidence rates may be somewhat inflated if cytology screening is being extensively used in a population of women, but the designation between corpus and cervix is nearly complete. Incidence rates compiled by the International Union Against Cancer indicate much variation in the frequency of this cancer and that of the ovary and breast, but not with cervical cancer. In the United States southern white women tend to have cancers of the uterine corpus, breast, and ovary less frequently and of the cervix more frequently than women of the north and west. American born Japanese women are developing breast cancer nearly as frequently in California as white women, which may also be true for ovarian cancer. Uterine corpus cancer may be increasing but the population is still too young, since this cancer reaches its highest rates at older ages. All these cancers are less frequent in Japan.

Retrospective data on dietary habits, employment history and tobacco use were obtained from 569 bladder cancer patients and 1025 age-matched controls admitted to Roswell Park Memorial Institute. Sex-adjusted relative risks revealed increases in risk for lower levels of an index of vitamin A intake. A similar pattern of risk elevation was associated with infrequent milk and carrot intake. Some elevation of risk was found for heavy coffee drinking but the apparent protective effect for milk consumption was not found to be a spurious result of lower coffee intake. Neither was the role of vitamin A explained by its relationship with smoking or employment in high risk occupations. Some association of bladder cancer with infrequent consumption of cruciferous vegetables was also observed. The findings of this investigation are consistent with tumor inhibition by retinoids in animal studies and the low risk associated with vitamin A in epidemiologic studies of lung cancer.

The concentrations of growth hormone (GH) and prolactin (PRL) in the serum of 42 breast cancer patients were determined by radioimmunoassay (RIA). Forty percent of the patients had elevated GH levels while only 17% had elevated PRL levels. These findings suggest a relationship between GH and breast cancer; a weaker correlation exists between PRL and this malignancy. In addition, total lactogens in the serum were measured by a bioassay (BA). The BA/RIA (GH + PRL) ratio was greater in the breast cancer patients than the controls, indicating that variant forms of the hormones with higher than normal biological activity might be present.

A total of 134 patients suspected of having pancreatic cancer were given preoperative ultrasonic examinations. A total of 54 cases had a final diagnosis of pancreatic cancer histologically confirmed. The diagnostic sensitivity and specificity of the ultrasonographies were 76% and 91% or 87% and 74%, depending on whether doubtful results were called abnormal or normal. "Doubtful" results were defined as those that stated that pancreatic cancer was "possible" or "could not be excluded." When these results were divided into abnormal and normal groups on the basis of the patients' preoperative serum carcinoembryonic antigen and pancreatic oncofetal antigen levels, the sensitivity and specificity of the combined tests may prove to be a more accurate means of screening patients for pancreatic cancer than ultrasonography alone.

Incidence of colorectal cancer in Israel was studied in 1035 patients who were divided by ethno-geography into a Euro-American group and an Afro-Asian group. The majority of patients were in the age groups over 50 years, with a markedly higher incidence rate of disease in the Euro-American group. In the younger age group the incidence rate of the disease was significantly higher in the Afro-Asian group. These findings, if verified by others, must lead to revision of our present concepts of epidemiology of colorectal cancer.

All Alaskan Native patients diagnosed with leukemia, lymphoma, multiple myeloma, and related cancers during the time period 1969-83 were identified. Of 72 biopsy-confirmed cases, lymphoma was diagnosed in 37 (including mycosis fungoides in 5), leukemia in 22, multiple myeloma in 11, and other lymphoreticular cancers in 2. Compared to the rates for U.S. whites, incidence rates were low for leukemia and lymphoma (especially Hodgkin's disease), but not for multiple myeloma in males. There was an apparent increase over time in lymphomas in males, while no increase occurred in females, nor for leukemia or multiple myeloma in either sex.

The primary diffused infiltrating type of colonic cancer is characteristically rare, and has a high rate of malignancy. A 54-year-old Japanese man was admitted to our hospital complaining of abdominal pain and decreased stools. Sigmoid colonic cancer was suspected as a result of the Ba-enema and colonic fiberscopy examination, but the biopsy results were negative. The sigmoid colon was resected extensively. Histopathologically, the cancer was diagnosed as being a signet ring-cell carcinoma of the sigmoid colon. Although the patient received postoperative chemotherapy, he died of carcinomata peritonitis 175 days after surgery.

This report is a retrospective analysis of 62 patients treated for early glottic carcinoma during the six-year period of January 1982 through December 1987. The authors report that the cancer has been controlled in 100% of patients. For invasive stage 1 tumors the radiation cure rate is 42 of 45 patients or 93%, and voice quality after treatment was usually normal. Surgically treated tumors are usually confined to one vocal cord. Primary surgery with laser or cord stripping ultimately led to tumor control and voice preservation in all eleven patients treated although two required salvage with radiotherapy and mild but acceptable hoarseness was a usual sequel.

In a case control study of the prevalence of palmar keratoses in 69 bladder cancer patients, 66 lung cancer patients, and 218 hospital controls palmar keratoses were more common in the cases, especially among those with cancer of the bladder. Above the age of 50 years, 87% of bladder cancer patients and 71% of lung cancer patients but only 36% of the controls had one or more keratoses. The prevalence increased with age and keratoses were more common in males. No features could be found which distinguished the keratoses in cases from those in controls.

TITLE: The management of febrile episodes in-neutropenic cancer.

The management of 33 febrile episodes in neutropenic patients suffering from acute leukaemia and other cancers is described. In 32 out of 33 episodes, the fever subsided. One patient died of cerebral haemorrhage while infected. The most common clinical sites of infection were, in order of frequency, the chest, the throat and the skin. Positive cultures were obtained in about half the episodes, the most common site being the throat followed by blood and sputum. The most common organisms isolated were Gram negative. Eighty-four per cent of febrile episodes occurred with neutrophil counts of less than 500/mm3, and in the majority of these, less than 100 neutrophils per cubic millmetre. All patients were reverse barrier nursed and on becoming febrile were given 48 hours of parenteral antibiotic therapy. The most common antibiotic combination used was gentamicin and cephalothin. At 48 hours, granulocyte transfusion and a third antibiotic were added to the regimen of patients not responding. The percentage response to antibiotics alone, in this series, at 58% was similar to that of other series, but the mortality experienced was lower.
