Germ cell tumors are relatively uncommon childhood tumors. Several interesting characteristics distinguish them from other tumors. (1) Abnormal migration of primordial germ cells results in the frequent occurrence of extragonadal germ cell tumors in children. (2) Serum markers exist to allow evaluation of extent of resection and development of recurrent tumor. (3) The introduction of modern chemotherapy including cisplatin, etoposide, and bleomycin has significantly improved survival of these children. Optimal therapy includes complete surgical resection, accurate histological examination, and selective use of chemotherapy. Mature and immature teratomas at any site and stage I testicular cancers are managed by observation alone, whereas all other tumors require chemotherapy.

Apoptosis is now recognized as an important process in tissue homeostasis. In malignancy, mutations in apoptotic programs may promote tumor progression as well as reduce the efficacy of cancer therapy. Recent studies identify the product of the p53 tumor-suppressor gene as an important regulator of apoptosis in tumor cells. At the same time, clinical studies implicate p53 mutations in pleiotropic resistance to cytotoxic cancer therapy. Together, these observations suggest that inactivation of p53 promotes resistance to anticancer agents by attenuating apoptosis. This view identifies p53 as a potential drug target and suggests several strategies for therapeutic intervention.

Women who smoke cigarettes are at increased risk for lung cancer, chronic obstructive pulmonary disease, and complications of oral contraceptive use. During pregnancy, cigarette smoking increases the risks for a low birthweight infant and infant mortality. A national health objective for the year 2000 is to reduce cigarette smoking among women of reproductive age (i.e., 18-44 years) to a prevalence of no more than 12% (objective 3.4h)(1). This goal is substantially lower than the estimated baseline prevalence of 29% measured by CDC's 1987 National Health Interview Survey (NHIS). To characterize recent trends in cigarette smoking and monitor progress toward the year 2000 objective, data from the NHIS for 1987 through 1992 were analyzed for women aged 18-44 years.

The life expectancy of patients with inoperable esophageal neoplasms is very limited. Therefore, palliative strategies should be immediately effective in relieving dysphagia. The efficiency of different methods so far available for palliation is discussed. In inoperable localized cancers, combined chemotherapy and radiation therapy are more effective than irradiation alone. Patients with more advanced carcinomas will benefit from the development of self-expanding metal stents. These stents can be placed quickly, are safe and show low morbidity rates.

TITLE: Amputation neuroma following radical neck dissection--report of 3 cases.

Amputation neuroma occurred in three cases among 111 cases of oral cancer patients after radical neck dissection. All of three cases appeared as a small nodule at superior neck around the carotid artery with accompanying tenderness. The lesions were located in continuity with the proximal end of nerve at excision. Although the incidence of amputation neuroma is low, critical examination is required to distinguish this from recurrent tumor after cancer surgery.

Moderate sized cancers of the upper third of the ear can be widely excised, including cartilage, leaving the posterior skin intact. This avoids a large wedge excision and primary closure which leaves a significant defect. The unsupported soft tissue of the upper ear following excision of the cancer can be reconstructed by rotating into position a composite flap of skin and cartilage from the middle third of the ear. This composite flap supports the superior helix. The tissue defects of the flap donor site and the area superior to the flap inset can be closed with thick split grafts from the post-auricular area.

Mutations of the p53 tumor-suppressor gene are the most common genetic alterations in human cancer, found in approximately 50% of all tumors. The importance of p53 in human cancer attracts attention in molecular studies dealing with the pathogenesis, diagnosis and prognosis in tumor pathology. This review summarizes the current understanding of p53 both on the genetic and protein level. Frequency and spectrum of somatic p53 mutations in the carcinogenesis of breast cancer, colorectal cancer, gastric cancer, hepatocellular carcinoma, squamous-cell carcinoma of the skin and malignant melanoma are discussed including our own investigations and studies published in the literature.

Surgical treatment of transitional bladder tumours is based on liberal resection of the bladder. After cystectomy, reconstitution of the urinary tract is required for urinary excretion and patient comfort. The bladder can be replaced by a segment of intestine detached from the digestive tract. Another mean is urinary diversion at the abdominal wall by a direct or continent fistula. The patient should be seen 6 weeks after surgery to assess the quality of the surgical assemblage, the absence of distension of the upper urinary tract and the degree of patient comfort. Subsequently, the patient should be seen every 6 months. Early treatment of bladder cancer leads to cure at 5 years in 50 to 75% of cases.

The prominent desmoplastic or stromal reaction seen in many invasive carcinomas suggests that stromal cells play a role in cancer pathogenesis. Investigations based on cell typing, using antibodies to cytoskeletal constituents, have revealed that most tumors contain various types of fibroblasts. Stromal cells with myofibroblastic differentiation features are the predominant cell type at the periphery of epithelial tumors. These tumor-activated fibroblasts play a major role in tumor development and spread, affecting the proliferation, differentiation, invasion or regression of cancer cells. This review considers the events inducing the different fibroblastic responses and the role of tumor-activated fibroblasts in both tumor development and anti-cancer treatments.
