TITLE: Mammography in symptomatic breast disease.
PUBDATE: 19890401
The contribution of mammography to the diagnosis of breast cancer was examined in 5080 patients with various breast symptoms. There were 562 breast cancers within this group. The sensitivity of mammography was 88%. Only 18 cancers were detected by mammography alone, and of these 3 were in the other breast. 7 of the remaining 15 cancers had clinical signs which would have been pursued to open biopsy in the absence of mammography. Clinical examination is of paramount importance in the management of patients with symptomatic breast disease. Mammography is most useful when applied in specific situations rather than to screen every patient with breast symptoms.

TITLE: Surgical management of brain metastases: how aggressive should we be?
PUBDATE: 19890601
Treatment of cancer metastatic to the brain is dictated by both the need to achieve immediate short-term palliation and the desire for durable symptom-free remission. Surgical resection of brain metastases makes available tissue for diagnosis and experimental study, permits prompt relief of neurological signs and symptoms secondary to tumor mass effect, enhances the potential for sustained local tumor control in those patients with solitary disease, and allows greater therapeutic flexibility by obviating the need for cranial radiation in some situations and by expanding patient access to alternative experimental protocols or systemic chemotherapy.

TITLE: Declining breast cancer mortality among young American women.
PUBDATE: 19870301
Changes in age-specific breast cancer mortality rates among white females in the United States during 1950-80 were shown to be correlated with changes in patterns of childbearing in early adulthood. However, for the most recent 5-year period among women below age 40 years, small declines in breast cancer mortality were observed in the late 1970's, despite a predicted increase following delays in childbearing that began in the 1960's and despite evidence of a rising incidence of the cancer. Although correlation analyses have inherent limitations, the findings raise the possibility that recent changes in the detection and management of breast cancer have contributed to a lowered mortality from this cancer among young American women.

The state of the major mediastinal veins was studied in 113 patients with central cancer of the right lung upper lobe with metastases to the regional lymph nodes. Routine angiography was performed in 92 patients, digital subtraction angiography--in 21 patients. Vascularization was shown to be more common in and adenocarcinoma. The authors identified 2 groups of angiographic signs: marked and doubtful ones. They have been of opinion that only the presence of marked signs can be a contraindication for surgical intervention. The informative value of routine angiography was compared with that of digital subtraction angiography.

TITLE: The incidence of cancer following total hip replacement.
PUBDATE: 19880801
We have studied the incidence of tumours at remote sites following total hip replacement: 1,358 individuals have been followed up for 14,286 person-years after operation. In the decade following implantation the incidence of tumours of the lymphatic and haemopoietic systems was significantly greater, and that of cancer of the breast, colon, and rectum, significantly less than expected. Whilst the association might be due in part to an effect of the prosthetic implants, other mechanisms, particularly drug therapy, require consideration.

PUBDATE: 19870901
Total bladder replacement with tubular sigmoid colon and detubularized ileocecal bowel segments was performed on 17 patients after cystoprostatectomy for bladder cancer. There were few complications and patient acceptance was excellent. Daytime continence was achieved in most patients but voiding patterns were superior with detubularized segments. However, enuresis was common with both segment types. Total urinary continence (day and nighttime continence) was achieved in 7 patients with an artificial urinary sphincter that was activated only at night. Total bladder replacement is an acceptable form of urinary diversion after cystoprostatectomy in appropriate patients.

PUBDATE: 19891101
A 14-year-old patient who was eventually found to have Gardner syndrome initially presented at the age of 3 years with a desmoid tumor involving the scalp. A careful review of the family history revealed a high incidence of colonic cancer, which prompted endoscopic evaluation of the patient. The discovery of adenomatous polyps in the colon confirmed the diagnosis of Gardner syndrome. In patients with hard or soft tissue tumors, the possibility of Gardner syndrome should be kept in mind, and a thorough family history taken. Early diagnosis may prevent malignant transformation of colonic polyps.

TITLE: A unifying concept of the aetiology of breast cancer.
PUBDATE: 19880501
A number of risk factors for breast cancer are considered jointly in one pathogenetic framework which relates to the formation of pre-cancerous lesions. Energy-rich diet during puberty and adolescence enhances the occurrence of pre-cancerous lesions in the breast. This process is counteracted by full-term pregnancies, and the earlier they come the fewer the number of such lesions. Energy-rich diet later in life contributes to the occurrence of obesity which, after menopause, enhances the growth of existing subclinical and clinical breast cancer. In both periods of life the nutritional factor exerts its effect through endocrine mechanisms in which oestrogens play a major part.

PUBDATE: 19891201
A nucleoid sedimentation technique was developed and used for analysis of DNA repair capacity in 40 members from 7 cancer families. Chromosome aberration rate (CAR) and chromosome fragile sites (CFS) were assayed simultaneously for comparison. It was shown that 32 members (80%) had a decreased DNA repair capacity and 34 (85%) had increased CAR as well as an apparent increase of CFS. 28 members (70%) showed both decreases of DNA repair capacity and increases of CAR. The importance of these criteria in heritable susceptibility to cancer is discussed.

A clinicopathological study of 9 cases presenting axillary mass alone was reported. Histological diagnosis revealed 3 occult breast cancers, 2 apocrine gland adenocarcinomas of the skin, a carcinoma and a fibroadenoma in the axillary accessory breast, a neurogenic tumor in the axilla, and a benign lymphadenopathy. There were no clinical characteristics helpful in preoperative differential diagnosis. As the axilla includes the main lymphatic routes from the breast, the axillary tail of the breast and accessory breast, breast-associated tumors should be kept in mind in the management of patients with axillary mass as a sole clinical feature.

In 27 cases of prostatic cancer, the histopathologic prostatic picture at the time of autopsy was compared with that in biopsy and that of metastatic lesions at autopsy. A change in the grade between in biopsy and in autopsy was confirmed in 4 cases (14.8%). A change in the grade between the prostate and metastatic lesion at the time of autopsy was confirmed in 3 cases (11.1%). The cases that had a change in the grade, had a common tendency: being older, having a longer clinical course than the no grade-change group, changing to a higher grade.

PUBDATE: 19891001
Excising cancer in successive layers and examining the entire underside of each layer in the microscope by the systematic use of frozen sections ensure eradication of the neoplasm, including its "silent" outgrowths, and this is accomplished with the maximal sparing of normal tissue. Fresh-tissue micrographic surgery is used for most cancers, but the fixed-tissue technique is safer for melanomas because all incisions are made through fixed (killed) tissues, avoiding the danger of disseminating highly transplantable melanoma cells from transecting silent outgrowths or clinically invisible satellites. Although the essential concept was very simple, more than a half century of continuing development was required to bring the method to its present status; further progress is an exciting prospect.

The analgesia for patients suffering from incurable tumors is a major problem. The parenteral administration of opioids causes disagreable side effects. The development of completely implantable devices which permit the epidural administration of morphine offers higher quality analgesia with less side effects. Being easy to use, these devices allow cancer patients to remain at home, whereas otherwise they would need to be hospitalized for effective treatment of pain.

TITLE: Pharmacokinetics of PTT-119 in man.
PUBDATE: 19880401
The pharmacokinetics of PTT-119, a new alkylating agent, was studied in 8 advanced cancer patients. PTT-119 disappeared rapidly from plasma after administration at a dose of 3 mg/kg by i.v. bolus injection. The HPLC method shows plasma levels of m-bis (dichloroethyl)amino-phenyl-L-alanine which is the major metabolite of PPT-119. Elimination of the drug from plasma can be described by a one-compartment model. Mean values of 77.8 min for the half-life, 510.8 ml/min for the total plasma clearance and 0.69 l/kg for the volume of distribution were found.
