TITLE: Prognostic value of Helix pomatia in breast cancer. International (Ludwig) Breast Cancer Study Group.
PUBDATE: 19930701
Six hundred and eighty-four primary breast cancers from the International (Ludwig) Breast Cancer Study Group (IBCSG) were studied for Helix pomatia lectin (HPA) binding. There was a weak correlation between lymph node-positive and HPA positive (P = 0.04). In our series there was a large advantage in disease-free survival (DFS) and overall survival (OS) for node-negative patients (P < 0.0001 DFS and OS). However, there was no such advantage for HPA-negative patients (P = 0.23 DFS and P = 0.32 OS). We conclude that in this randomised patient group HPA is of no clinical predictive value.

TITLE: Induction of covalent DNA adducts in rodents by tamoxifen.
PUBDATE: 19920301
The antiestrogen tamoxifen, increasingly used as adjuvant treatment for breast cancer, has been found to covalently modify DNA of rodents. For instance, the liver DNA of female Sprague-Dawley rats treated with a single injection of tamoxifen contained two DNA adducts. Four additional DNA adducts were formed and adduct concentrations increased 5- 7- and 10-15-fold after three and six tamoxifen injections, respectively, from levels observed after a single dose. The accumulation of DNA adducts with repeated administrations of tamoxifen to rodents may make this drug a poor choice for the chronic preventative treatment of breast cancer.

PUBDATE: 19970101
The epidemic of breast cancer and the availability of tools to increase early detection and decrease mortality require care and diligence on the part of every physician directly caring for patients. With the widespread implementation of managed care, primary care physicians are in the best position to recommend breast cancer screening. Educational programs to emphasize the primary care physician's role in breast cancer prevention and detection, to improve physical examination skills, and to implement office systems to facilitate screening should be available to primary care physicians, and may have a major impact on breast cancer mortality in the future.

PUBDATE: 19900301
Mucins are heavily glycosylated proteins that are produced in excessive amounts in breast cancers and other adenocarcinomas. These proteins are potent immunogens; indeed, most monoclonal antibodies raised against extracts of breast cancer cells or tumors are directed toward a single family of mucins. The anti-mucin antibodies currently available are valuable diagnostic aids for detecting advanced stages of breast cancer. To improve the sensitivity of these antibodies so that they might be suitable for use in screening and early diagnosis, tumor imaging, and therapy of breast cancer, recent studies have focused on identifying the precise epitopes they recognize.

PUBDATE: 19950701
No clear role has yet been defined for the role of total parenteral nutrition in the management of the cancer patient, and this subject remains an area of controversy. Peri-operative nutritional support, if given for at least 10 days, may reduce morbidity and mortality in some patients, although this subgroup has not been fully characterized. There is no evidence that parenteral nutritional support benefits patients undergoing chemotherapy or radiotherapy in terms of an increased tumor response rate or prolongation of survival.

PUBDATE: 19920701
Sera of patients with chronic hepatitis and chronic haemodialysed uraemia were tested for both Hepatitis B virus and Hepatitis Delta virus markers from 1985. Hepatitis Delta virus seroconversion was found in 10 of 88 haemodialysed patients and anti-Delta-IgG sero-positivity in 11 of 108 chronic hepatitis patients. The clinical course of Delta superinfections in haemodialysed patients was as follows: 3 infections were symptomless, 7 patients had acute hepatitis, two of the latter group died because of fulminant hepatitis. Of the 11 HBsAg and anti-Delta-IgG positive chronic hepatitis patients 3 had primary hepatocellular cancer.

TITLE: Mammary gland mass and breast cancer risk.
PUBDATE: 19921101
Evidence indicates that early life events and conditions, possibly extending to the intrauterine stages of life, and including energy restriction in early life, affect the risk of breast cancer. The mechanism of this effect is likely to be through a reduction in mammary gland mass and, inferentially, the total number of ductal stem cells. The evidence derives from epidemiologic and animal studies. It can explain certain epidemiologic findings that cannot be accounted for by more established breast cancer risk factors, including the more frequent occurrence of breast cancer in the left breast and the higher incidence of this disease among caucasian women than among Asian women in Asia.

Investigation of certain indices of immunity in 52 patients with lung cancer subjected to X-ray irradiation at stages of the surgical treatment under conditions of various kinds of anesthesia have shown that preoperative radiation therapy with the dose of 30 gr resulted in a considerable suppression of immunity indices at the preoperative period and aggravated immune depression following operative procedures. At the same time immune depressive reactions were found to depend on the kind of anesthesia.

PUBDATE: 19970201
Germ cell tumors are relatively uncommon childhood tumors. Several interesting characteristics distinguish them from other tumors. (1) Abnormal migration of primordial germ cells results in the frequent occurrence of extragonadal germ cell tumors in children. (2) Serum markers exist to allow evaluation of extent of resection and development of recurrent tumor. (3) The introduction of modern chemotherapy including cisplatin, etoposide, and bleomycin has significantly improved survival of these children. Optimal therapy includes complete surgical resection, accurate histological examination, and selective use of chemotherapy. Mature and immature teratomas at any site and stage I testicular cancers are managed by observation alone, whereas all other tumors require chemotherapy.

PUBDATE: 19910801
By using thymocytes of mice as target cells in bioassay, IL-2 production activity was investigated in 24 lung cancer patients, 24 noncancerous pulmonary lesions as well as 20 normal individuals. Fourteen cancer patients were reexamined after the first course of chemotherapy. This research demonstrated that: 1. PBL capable of IL-2 production in lung cancer patients were rare in comparison with benign disease or normal control. 2. IL-2 activity was markedly enhanced by chemotherapy. It is suggested that IL-2 is a good indicator for estimating the efficacy of treatment of lung cancer.

Not long ago, cancer was generally looked on as a lethal disease. In the last decades, recovery chances for various forms of cancer have increased spectacularly. And research is still going on. Various new therapeutical techniques have been implemented recently or will be in the near future. In my paper, I will first briefly mention some achievements of the past 25 years. Then I will discuss some methods that have been developed recently, or that are still in an experimental stage. I will also concentrate on aspects that are of interest to the radiographer, and that may influence daily routine and workload.

PUBDATE: 19901201
Colorectal cancer is a major public health problem in Western industrialized countries. It is estimated that more than a quarter of a million people are affected by this disease each year. The population-adjusted yearly incidence rate has remained constant at approximately 40 to 50 cases per 100,000. About 75% of the individuals with these cancers have a primary surgical resection with hope of complete tumor eradication. Despite the high resectability rate and a general improvement in surgical therapy, nearly half of all patients with colorectal cancer still die of metastatic tumor. Major progress has been made in various aspects of colorectal cancer over the last few years, including molecular biology, screening and adjuvant therapy.

PUBDATE: 19951101
Apoptosis is now recognized as an important process in tissue homeostasis. In malignancy, mutations in apoptotic programs may promote tumor progression as well as reduce the efficacy of cancer therapy. Recent studies identify the product of the p53 tumor-suppressor gene as an important regulator of apoptosis in tumor cells. At the same time, clinical studies implicate p53 mutations in pleiotropic resistance to cytotoxic cancer therapy. Together, these observations suggest that inactivation of p53 promotes resistance to anticancer agents by attenuating apoptosis. This view identifies p53 as a potential drug target and suggests several strategies for therapeutic intervention.

TITLE: Cigarette smoking among women of reproductive age--United States, 1987-1992.
PUBDATE: 19941101
Women who smoke cigarettes are at increased risk for lung cancer, chronic obstructive pulmonary disease, and complications of oral contraceptive use. During pregnancy, cigarette smoking increases the risks for a low birthweight infant and infant mortality. A national health objective for the year 2000 is to reduce cigarette smoking among women of reproductive age (i.e., 18-44 years) to a prevalence of no more than 12% (objective 3.4h)(1). This goal is substantially lower than the estimated baseline prevalence of 29% measured by CDC's 1987 National Health Interview Survey (NHIS). To characterize recent trends in cigarette smoking and monitor progress toward the year 2000 objective, data from the NHIS for 1987 through 1992 were analyzed for women aged 18-44 years.

In this extensive review the author discusses the incidence of breast cancer, screening methods, the theoretic radiation risk, the mammographic signs of suspicious lesions and the value of ultrasonography and stereotactic fine-needle biopsy. The emphasis is on the close relation between radiology, the physical examination, surgery and pathological studies. Up-to-date information about controversial topics is provided, and a therapeutic scheme is suggested.
