TITLE: Mammography in symptomatic breast disease.

The contribution of mammography to the diagnosis of breast cancer was examined in 5080 patients with various breast symptoms. There were 562 breast cancers within this group. The sensitivity of mammography was 88%. Only 18 cancers were detected by mammography alone, and of these 3 were in the other breast. 7 of the remaining 15 cancers had clinical signs which would have been pursued to open biopsy in the absence of mammography. Clinical examination is of paramount importance in the management of patients with symptomatic breast disease. Mammography is most useful when applied in specific situations rather than to screen every patient with breast symptoms.

Breast cancer is known to metastasize to all organs of the human body, and its manifestations are protean. It is almost impossible to predict which organ system will be invaded. Since 1950, there are seven autopsy series of patients with breast carcinoma published in the English literature. All data came from American hospitals (total of 2,147 patients), and each report studied more than 100 patients with breast cancer. The reported incidence of involvement of different organ sites are presented in Table I. Although the data came from different institutions and spanned over 35 years (1943-1977), the findings regarding metastatic involvement of specific organs are fairly similar.

We histologically examined 17 lesions from minute gastric cancer (less than 5 mm in diameter). Flat or IIb type was frequently seen in minute cancer (47%); the depressed type was predominant in ordinary early gastric cancer (72%). Differentiated adenocarcinoma was noted more often in minute cancer (88%) than in ordinary early gastric cancer (61%) or advanced cancer (45%). Differentiated type minute cancer was frequently associated with severe atrophic gastritis (93%) and intestinal metaplasia (73%). There was no evidence for the preexistence of chronic peptic ulcer, because no fibrosis was present in the gastric wall beneath the minute cancerous lesions.

TITLE: Declining breast cancer mortality among young American women.

Changes in age-specific breast cancer mortality rates among white females in the United States during 1950-80 were shown to be correlated with changes in patterns of childbearing in early adulthood. However, for the most recent 5-year period among women below age 40 years, small declines in breast cancer mortality were observed in the late 1970's, despite a predicted increase following delays in childbearing that began in the 1960's and despite evidence of a rising incidence of the cancer. Although correlation analyses have inherent limitations, the findings raise the possibility that recent changes in the detection and management of breast cancer have contributed to a lowered mortality from this cancer among young American women.

The state of the major mediastinal veins was studied in 113 patients with central cancer of the right lung upper lobe with metastases to the regional lymph nodes. Routine angiography was performed in 92 patients, digital subtraction angiography--in 21 patients. Vascularization was shown to be more common in and adenocarcinoma. The authors identified 2 groups of angiographic signs: marked and doubtful ones. They have been of opinion that only the presence of marked signs can be a contraindication for surgical intervention. The informative value of routine angiography was compared with that of digital subtraction angiography.

In adult rats, the intracerebroventricular injection of alpha-difluoromethylornithine (DFMO), a polyamine antimetabolite which specifically inhibits ornithine-decarboxylase, induces a typical convulsive syndrome (ED50 = 100 micrograms/rat) and death (LD50 = 300 micrograms/rat). Histological lesions are seen only with the highest doses and are essentially restricted to the Ammon's horn neurones. Since DFMO is currently undergoing clinical trials for use in cancer chemotherapy, including brain tumours, its CNS toxicity should be carefully considered.

The activities of serum acid ribonuclease (RNase) were determined in patients with malignant neoplasm or with renal failure. The levels were markedly increased in myelogenous leukemia and renal failure, and only slightly increased in solid cancers, lymphoid malignancies and multiple myeloma. These increases correlated significantly with serum LDH activity in myelogenous leukemia and with creatinine levels in other malignancies or renal failure. The acid RNase content of granulocytes was 22.7-fold higher than that of lymphocytes. The increase of serum acid RNase may suggest an increased granulocyte destruction in myelogenous leukemia and a reduced glomerular filtration in other malignant neoplasms and renal failure.

TITLE: Cancer pain relief using chronic morphine infusion. Early experience with a programmable implanted drug pump.

Fourteen patients were implanted with drug pumps to provide chronic epidural or intrathecal morphine to relieve pain due to cancer. A new programmable pump was used in seven of the patients and a constant infusion device was used in the other seven patients. Results, judged by subjective pain reports (on a 0 to 10 scale), decrease in oral narcotics, and change in activity level, were excellent in eight patients, good in five patients, and poor in one patient. The programmable device has the obvious advantage of being able to vary dose according to patient need and requires less frequent refilling. Four programmable pump failures occurred, two requiring replacement.

We have studied the incidence of tumours at remote sites following total hip replacement: 1,358 individuals have been followed up for 14,286 person-years after operation. In the decade following implantation the incidence of tumours of the lymphatic and haemopoietic systems was significantly greater, and that of cancer of the breast, colon, and rectum, significantly less than expected. Whilst the association might be due in part to an effect of the prosthetic implants, other mechanisms, particularly drug therapy, require consideration.

Total bladder replacement with tubular sigmoid colon and detubularized ileocecal bowel segments was performed on 17 patients after cystoprostatectomy for bladder cancer. There were few complications and patient acceptance was excellent. Daytime continence was achieved in most patients but voiding patterns were superior with detubularized segments. However, enuresis was common with both segment types. Total urinary continence (day and nighttime continence) was achieved in 7 patients with an artificial urinary sphincter that was activated only at night. Total bladder replacement is an acceptable form of urinary diversion after cystoprostatectomy in appropriate patients.

A 14-year-old patient who was eventually found to have Gardner syndrome initially presented at the age of 3 years with a desmoid tumor involving the scalp. A careful review of the family history revealed a high incidence of colonic cancer, which prompted endoscopic evaluation of the patient. The discovery of adenomatous polyps in the colon confirmed the diagnosis of Gardner syndrome. In patients with hard or soft tissue tumors, the possibility of Gardner syndrome should be kept in mind, and a thorough family history taken. Early diagnosis may prevent malignant transformation of colonic polyps.

A number of risk factors for breast cancer are considered jointly in one pathogenetic framework which relates to the formation of pre-cancerous lesions. Energy-rich diet during puberty and adolescence enhances the occurrence of pre-cancerous lesions in the breast. This process is counteracted by full-term pregnancies, and the earlier they come the fewer the number of such lesions. Energy-rich diet later in life contributes to the occurrence of obesity which, after menopause, enhances the growth of existing subclinical and clinical breast cancer. In both periods of life the nutritional factor exerts its effect through endocrine mechanisms in which oestrogens play a major part.

The content of fibrin fibrinogen splitting products (FSP), radioimmune trypsin, C-peptide and carbohydrate antigen (CA) 19-9 in the blood of 82 patients with acute pancreatitis (edematous and hemorrhagic), and chronic recurrent pancreatitis at the stage of exacerbation, 42 patients with chronic pancreatitis, 34 patients with cancer of the pancreas (stages III-IV) and 22 healthy persons were studied. Results indicate a high diagnostic value of determination FSP, trypsin and C-peptide in patients with acute pancreatitis and chronic recurring pancreatitis at the stage of exacerbation, trypsin and C-peptide in patients with chronic pancreatitis associated with severe exocrine insufficiency of the pancreas, KA 19-9 in patients with cancer of the pancreas.
