TITLE: Involvement of platelets in tumour angiogenesis?
PUBDATE: 19981101
Preclinical and clinical research show that tumour growth is dependent on angiogenesis. Activation of the coagulation cascade is commonly found in patients with cancer. We propose that platelets contribute to tumour-induced angiogenesis. The basis of our hypothesis is that platelets are a rich source of stimulators and inhibitors of angiogenesis and their interaction with the endothelium. Presumably, the antithrombotic state of normal endothelium is disturbed by endothelial stimuli derived from tumour cells. This hypothesis may explain the suggested clinical benefits of anticoagulants in cancer and implies that targeting of platelet interaction with tumour vasculature will inhibit angiogenesis.

Clinical pathways are useful tools in providing quality care while decreasing the cost of that care. Pathways help to facilitate managed care, identify patient and family educational needs, encourage multidisciplinary communication, and expedite patient discharge. This article discusses a clinical pathway for ovarian cancer surgery.

PUBDATE: 20001001
Checkpoints are mechanisms that regulate progression through the cell cycle insuring that each step takes place only once and in the right sequence. Mutations of checkpoint proteins are frequent in all types of cancer as defects in cell cycle control can lead to genetic instability. This review will focus on three major areas of cell cycle transition control, with particular attention to the alterations found in human cancer. These areas include the G1/S transition, where most cancer-related defects occur, the G2/M checkpoint and its activation in response to DNA damage, and the spindle checkpoint.

Morphine is recommended as the strong opioid of choice by the World Health Organization (WHO) for cancer pain management. It is associated with delay in gastro-intestinal transit; as a result, patients may experience marked constipation. We describe a case of intestinal pseudo-obstruction associated with the use of oral morphine in a terminally ill cancer patient.

The Women's Health Alliance Intervention Study is a community-based intervention study designed to determine if county-based coalitions can increase breast and cervical cancer screening rates among women aged 40 and older in four rural Wisconsin counties. This article describes the intervention process and demonstrates the feasibility of rural volunteer coalitions to promote breast and cervical cancer screening in communities. An accompanying article in this issue describes the effectiveness of these interventions.

PUBDATE: 20011001
In 2000, we reported that there is no reliable evidence that screening for breast cancer reduces mortality. As we discuss here, a Cochrane review has now confirmed and strengthened our previous findings. The review also shows that breast-cancer mortality is a misleading outcome measure. Finally, we use data supplemental to those in the Cochrane review to show that screening leads to more aggressive treatment.

TITLE: Decrease in concentration of free catechins in tea over time determined by micellar electrokinetic chromatography.
PUBDATE: 20010701
Cancer chemoprevention is a new and important medical science and much interest has been focused on catechins, not only for their antioxidant activity, but also because of their known antimutagenic and antitumorigenic properties. Green tea and black tea, which are among the most popular beverages consumed worldwide, contain many different catechins. Due to the instability of catechins in solutions with neutral or basic pH values the concentrations of catechins in tea decrease in time. In this presentation we used micellar electrokinetic chromatography to determine the real concentration of catechins between 0 and 60 min after the tea was brewed.

Our experience of arterial infusion chemotherapy combined with regional hyperthermia in the treatment of non-resectable pancreatic cancer was presented. A patient with cancer in the pancreatic head with accompanying extensive metastasis in both hepatic lobes was treated by sub selective aortic infusion of 5-FU and MMC with microwave hyperthermia. Both the cancer in the pancreatic head and the liver metastasis showed complete remission for 16 years.

Hepatitis C (HCV) can cause liver damage or liver cancer, and it is doubtful that antiviral drugs alone will be able to cure HCV. Because of the limitations of antiviral drugs, researchers are studying ways to enhance the immune system's ability to fight HCV. The current standard therapy for treating HCV is to use Interferon-alpha, although this may change to a combination of Interferon-alpha and Ribavirin. However, Ribavirin can have detrimental effects on bone marrow and may not be tolerated by some patients. Results of a study are presented.
