TITLE: Chemotherapy of hormonally unresponsive prostatic carcinoma.
PUBDATE: 19840501
The national Prostatic Cancer Project has evaluated single and combination chemotherapeutic agents for the treatment of patients with advanced prostatic cancer that has been unresponsive to hormonal therapy. Although regression rates have been modest, a number of drugs have shown some efficacy and are now undergoing testing in combination. Current trials of the LH-RH agonists and the antiandrogen flutamide are in progress. These trials have demonstrated a role for chemotherapy in the treatment of the patient who has failed hormonal therapy.

TITLE: Effects of mechlorethamine (HN2, nitrogen mustard) on UV-induced carcinogenesis in hairless mouse skin.
PUBDATE: 19840201
The influence of mechlorethamine (HN2, nitrogen mustard) on UV-induced carcinogenesis was examined in the hairless mouse skin in vivo. Noncarcinogenic amounts of topically applied HN2 and carcinogenic levels of UVB energy were used in the study. The HN2 applications significantly accelerated the appearance and growth of cutaneous tumors in this study. Thus HN2 acted as either a promoter or cocarcinogen for UV-induced cancer formation. Although the mechanism of this effect was not established, combinations of UVB radiation and HN2 topical therapy pointed to an increased incidence of cutaneous tumors in human skin.

TITLE: Lactic acidosis: a metabolic complication of extensive metastatic cancer.
PUBDATE: 19830501
Lactic acidosis is a rare metabolic complication of cancer. An analysis of the cases reported in the English and French literature shows that all the patients have extensive neoplastic disease. Metastatic hepatic lesions are present in the large majority of cases, suggesting that alteration of liver function is part of the clinical picture. Chemotherapy against the neoplastic disease is the only effective treatment of this type of lactic acidosis.

Lung cancer of the right upper lobe not infrequently invades the right lower trachea. A tracheobronchoplastic procedure is described that makes it possible to reconstruct the tracheal defect after excision of the growth, while preserving the uninvolved lobes of the right lung.

PUBDATE: 19850201
Insoluble extracts of human bronchogenic squamous cell carcinoma were solubilized by desoxycholate. After fractionation, the soluble extracts were used to prepare antisera in guinea pigs. These antisera, after being highly absorbed with homogenates of several normal organ, serum and carcinoembryonic antigen, were reactive only with squamous cell carcinoma. There was no reactivity to fetal lung, normal adult organs or other cancers.

Transphenoidal ablation of the pituitary gland by the injection of alcohol is used for the relief of wide-spread cancer pain. A technique is described in which a cryoprobe is passed through a transphenoidal needle and a number of lesions produced within the gland. It is suggested that this method of pituitary ablation may prove to be as effective but accompanied by fewer complications than the injection of alcohol.

PUBDATE: 19831101
This paper describes the working of an oncology nurses coping group which functioned under the guidance of a consultation-liaison psychiatrist at the Toronto General Hospital. Nurses were helped to deal more effectively with the many and varied stresses which they face in treating patients with cancer. As a result ward atmosphere, patient care and inter-staff communication improved. The success of the group was due to a number of factors that included the high motivation of the staff, the high level of stress on the ward, the support of the head nurse and the consistent relationship of the consulting psychiatrist. A series of cases are presented to illustrate these points.

PUBDATE: 19820501
We have conducted a broad phase II clinical trial of chlorozotocin in 74 patients including 28 with malignant melanoma, 18 with breast cancer, nine with non-Hodgkin's lymphoma, six with nonseminomatous testicular cancer, five with ovarian cancer, four with sarcoma, three with non-beta islet cell carcinoma of the pancreas, and one with anaplastic carcinoma of the thyroid. Objective responses were noted only in 15% of the patients with melanoma and in 11% of the patients with non-Hodgkin's lymphoma. Significant leukopenia and thrombocytopenia were observed only in previously treated patients. Chlorozotocin does not appear to offer clinically significant advantages over other currently available nitrosoureas.

Twenty-three patients with gallbladder and cholangiocarcinoma were treated with m-AMSA at doses of 60-120 mg/m2 i.v. repeated at 4-week intervals. Toxicity was primarily hematologic. Partial responses occurred in 1/12 patients with gallbladder cancer and 1/11 patients with cholangiocarcinoma. The activity of m-AMSA in these neoplasms appears similar to that seen in hepatomas.

The intraductal secretin test is an important diagnostic study. It enables the physician to determine the pancreatic secretory function in patients with known pancreatitis and to confirm the diagnosis of pancreatitis in many patients with indeterminate upper abdominal pain in whom ERCP and other diagnostic studies are normal. The IDST also provides the endoscopist and biochemist a new means to establish discriminating tests in differential diagnosis of pancreatic cancer and pancreatitis and to study the physiology of pancreatic secretion.

Retinoids--vitamin A and its analogues--have been recognized for more than half a century to be potent agents in the control of cellular differentiation and proliferation. Only recently, however, have there been concerted efforts to exploit this observation, with particular emphasis on cancer prevention. Experimental findings of tumor inhibition and suppression are reviewed.

TITLE: Current status of ultrasonic tissue characterization in breast cancer.
PUBDATE: 19841201
Early detection and correct diagnosis of breast cancer is necessary and an important determinant of prognosis because of its high incidence, affecting 1 of 13 women in western countries. The recent rapid increase of breast cancer in Japan is due primarily to westernized dietary habits. Clinical sonography, used in screening to detect breast cancer, is increasingly being used as it offers a high degree of diagnostic accuracy and no physical hazards such as x-ray mammography. Ultrasonic tissue characterization study is an important factor in determining diagnostic criteria for breast cancer and to develop new ultrasonic equipment in clinical breast echography. Therefore the current status of ultrasonic tissue characterization is reviewed, high-lighting breast cancer.

TITLE: A study of gastrointestinal cancer-associated antigen (GICA) in human fetal organs.
PUBDATE: 19830901
The gastrointestinal cancer-associated antigen (GICA) characterized by 1116 NS 19-9 monoclonal antibody was studied in human fetal organs by immunohistology and immunofixation on nitrocellulose sheets. It was found in all the gastrointestinal tracts of fetuses and newborns. Other fetal organs, except biliary and pancreatic ducts, were negative. Immunohistological data and enzymatic studies led us to conclude that GICA is predominantly a mucin-type glycoprotein in fetal organs.
