This is a report on 64 patients, who were operated on because of gastric ulcer situated near the cardia. We found the best results after the so-called step-like resection, which is preferable to selective vagotomy with excision of the ulcer and pyloroplasty because of the special conditions of the secretion of gastric juice in the case of high ulcers. Because of the danger of the development of a cancer in a B-II-stomach we prefer the B-I-anastomosis. For ulcers with stenosis of the esophagus a cardiofundectomy should be performed. Ulcers high on the greater curvature and in the fundus should be excised and a distal gastrectomy (B-I) should follow. The sole excision of the ulcer or the operation according to Kelling-Madlener are not recommended.

TITLE: Inappropriate production of collagen and prolyl hydroxylase by human breast cancer cells in vivo.

Thirty-two scirrhous cancers of breast have been examined to determine the origin of the collagen stroma in these tumours. Employing two immunohistochemical techniques it has been shown that the malignant epithelial cells in 30 of these tumours contain not only collagen but also prolyl hydroxylase, a key enzyme in collagen biosynthesis. Neither this enzyme nor collagen was detectable in the spindle cells in the stroma of these tumours. Neither the epithelium in normal breast, that in fibrocystic disease and in fibroadenomata, nor the malignant epithelium in two medullary cancers of breast contained either collagen or prolyl hydroxylase. These results strongly suggest that the malignant epithelium of scirrhous breast cancers produces its own collagen stroma and that the scirrhous reaction in these tumours is not a host response to tumour invasion. The production of collagen and prolyl hydroxylase by breast cancer cells (of the scirrhous type) therefore represents another example of inappropriate protein production by a human tumour.

178 patients of both sexes operated on for benign conditions of the mamilla between 1957 and 1967 were reviewed and the results are reported. The most frequent condition was sclerocystic mastosis, followed by benign cancer and forms with chronic inflammatory aetiology. The most affected sex, obviously, was the female. At follow-up, on average 9 yrs after operation, 19 female patients reported the reappearance of benign neoformations of the mamilla while carcinoma had occurred in a further 7 patients.

Platelet transfusions are of unquestionably proven benefit for the correction of thrombocytopenia or functional platelet disorders, and they have allowed for more intensive antineoplastic therapy. With the advent of blood component therapy most modern blood banks now have the capabilities for supplying at least limited quantities of platelets. Refinements in procurement methods will inevitably lead to a greater supply of platelets and the establishment of larger transfusion programs. These programs will need to incorporate facilities for platelet storage, recruitment of suitable donors, selection of special donors for refractory patients, and methods for quality control. As antineoplastic therapy becomes more aggressive, such transfusion programs will become an integral part of the operation of cancer treatment centers.

Among 16,394 necropsies during the 12-year period, 1958-1969 (representing about 60% of all persons who died in Malmö during this period, 747 cases of bronchial cancer were found in 744 individuals. There were considerable differences in frequency of different histologic types with regard to sex. Two main classes have therefore been distinguished: Group I, comprising squamous epithelial cancer and anaplastic cancer (including oat-cell cancer), which was predominant among men; and Group II, comprising adenocarcinoma, bronchiolar carcinoma, carcinoid and bronchial gland tumour, which was of more or less equal frequency in both sexes. Compared with other malignant tumours, bronchial carcinoma had a strong tendency to metastasise to the adrenals, brain, kidneys and skeleton. Bronchial cancer did not differ from other tumours with regard to lymphogenous spread but had a stronger tendency to spread via the blood stream. Squamous epithelial cancer showed the strongest tendency to remain confined to the thoracic cavity.

A series of 121 patients with carcinoma of the lower oral cavity seen at the British Columbia Cancer Institute during the years 1963 to 1969 has been reviewed. We have evidence of improved treatment results over a previous series of patients treated between 1953 and 1962. The results are as satisfactory as other reported treatment policies in which either surgery or radiation is used exclusively. Careful follow-up of all patients is essential. When recurrence or metastatic disease is recognized early, further treatment can still result in cure. The criteria for case selection of the individual patients are outlined.

Frequent findings are peptic ulcers, suture granulomas, and reflux esophagitis. Carcinomas in the stump develop increasingly from the 10th to 15th postoperative years. In the resected stomach, the pylorocardial expansion type of gastritis predominates. Atrophy is not an obligatory finding. Foveolar hyperplasia is typical, predominantly in the region of the anastomosis.

Ten patients with advanced or recurrent ovarian cancer were treated with a combination of chemotherapy and active specific immunotherapy after tumour stasis had been induced. They were inoculated with irradiated allogeneic cryopreserved tumour cells and B.C.G. once monthly in addition to receiving conventional chemotherapy. The overall duration of "remission", median survival, and projected 24-month actuarial survival in the patients receiving immunotherapy were apparently better than in a retrospectively matched control group treated by chemotherapy alone.

Peripheral blood leukocytes (PBL) from breast cancer patients with early or localized cancer fail to adhere to glass in the presence of breast cancer extract. The same leukocytes do not react to unrelated tumour extracts. Enrichment and depletion of certain PBL populations from patients with apparently localized breast cancer indicated that the indicator and/or reactive cell manifesting non-adherence in the presence of appropriate tumour antigen was phagocytic, glass adherent in the absence of tumour antigen and had cell surface Fc-receptors. The cell involved, therefore, appears to be the circulating monocyte. These results show that in the tube LAI assay, the peripheral blood monocyte appears to react directly with the tumour antigen resulting in a loss of its property of adherence to glass.

Reconstruction after mandbulectomy for cancer presents a major problem in head and neck surgery. This study reports the experiences with a planned staged method of reconstruction that aims at both cosmetic and functional rehabilitation and that is based on a close cooperation between surgeon and prosthodontist. Stabilization of the remaining mandibular fragments by means of intermaxillary fixation at the time the tumor is resected, delayed bone grafting, and preprosthetic surgery to allow the use of a functional denture are the high points of the method. The results in 58 patients are discussed.

Because of the greater life expectancy of women, the length of the post-menopausal period has also been constantly increasing. Since there is no generally accepted definition of the late post-menopause and senium in women, we submit a suggestion of how these periods of life may be deliminated. During the years 1960--1969 6662 female patients aged 60 years or more, were operated on in 17 different Westberliner departments of gynecology. Of these 3.9% (i.e. 261 cases) were major gynecological operations on women of 80 years or more: 155 were malignant and 37 benign tumors, 59 were due to displacement of the uterus (11 cases of minor descended uterus and 48 cases of fully prolapsed uterus) and 10 other diseases. The postoperative death rate was 13.4% (36 cases). In spite of such mortality these major operations were generally considered necessary and beneficial in women of this age group. Among other reasons, cancer-screening examinations of old women should be done more regularly than at present because of the multi-morbidity occurring at this age.

12 patients with cancer arising from a cirrhosis of the liver and 8 patients with regenerative nodes in a cirrhotic liver have been examined with scintigraphy, serial scintigraphy, angiography and biopsy. The results show that defects in the parenchyma of the liver can be shown with relative certainty using conventional scintigraphy with radiocolloids. But nothing can be said about their nature. Because of the simplicity of this technique and the minor claims on the patient scintigraphy should be the first diagnostic step. If angiography is contra-indicated serial scintigraphy may further elucidate the effects. When, however, angiography can be done, it will offer the best chance of differentiating between cancer and regenerative nodes on the basis of cirrhosis.

Carcinoma in situ is defined as the early stage of cancer and must therefore be initiated by an as yet unknown carcinogen(s). Progression of the lesion to invasive carcinoma is reported to occur in a high proportion of nontreated cases. Reserve cell proliferations are frequently associated with both dysplasia and carcinoma in situ, and it is suggested that these are the cells from which both lesions arise. Dysplasia may result from both carcinogenic and noncarcinogenic stimuli. Since dysplasia usually either regresses or remains stabilized over a prolonged period, it is suggested that it is more frequently associated with noncarcinogenic stimuli. Microinvasive carcinoma is limited to lesions with no more than 5 mm. of stromal invasion as measured from the surface. Confluence of growth and lymphatic-like space invasion should not interdict the diagnosis. Microinvasive carcinoma thus defined rarely gives rise to lymph node metastasis or eventuates in death. The diagnosis cannot be made from punch biopsy specimens. Only if pathologists adhere to a standard nomenclature can follow-up studies be used successfully to identify the natural behavior of each type of lesion in this spectrum.

TITLE: Ascites form of a human cancer serially transplantable in nude mice.

Human adenocarcinoma cells injected into the peritoneal cavities of BALB/c nude mice (nu/nu) induced ascites carcinoma. The inoculant was obtained from subcutaneous tumors produced in nude mice by an injection of ascites cells from a patient with carcinomatous peritonitis caused by mucinous adenocarcinoma of the stomach. An ascitic fluid began to accumulate 45 days after inoculation and reached the maximum volume within 120 days. Dispersed stomach cancer cells in the ascites could be serially transplanted in nude mice in an ascites form. The morphology of these cells was similar to that of the original cells in the ascitic fluid of a patient with carcinomatous peritonitis.
