Radioimmunodetection is a nuclear medicine technique that depends on in vivo detection of localization of antibodies and antibody forms carrying radioactivity for the purpose of diagnosis in patients with cancer. Current methods take advantage of tracers suitable for high resolution gamma-camera imaging, such as 99mTc and 111In, for common tumors, such as colon and lung cancers. In addition, tracers such as 125I have been used for the intraoperative detection of metastatic deposits. These methods detect from 75 to 90% of metastatic deposits with high specificity, and typically contribute important diagnostic information, even in 25-40% of patients with occult disease.

A case of adenocarcinoma of the splenic flexure during pregnancy is reported. A 27-year-old woman was admitted at 34 weeks of pregnancy with abdominal pain and vomiting. 36 hours after normal delivery an emergency exploratory laparotomy was performed for intestinal obstruction. A huge tumor of the splenic flexure with retroperitoneal extension was found. She died 1 year later of metastatic disease. Delay in diagnosis and complications of colonic cancer are more prevalent in pregnancy. While colonic carcinoma during pregnancy is extremely rare, it should be included in the differential diagnosis of colonic symptoms. The 24th case of this type is reported.

Myelosuppression is the most common toxicity associated with the administration of dose-intensive cytotoxic chemotherapy. The basic understanding of neutrophil biology and the physiology of chemotherapy-induced neutropenia has advanced tremendously in the past 2 decades. Concordantly, the ability to reduce the morbidity associated with neutropenia has improved. Adjunctive cytokine and progenitor cell support of hematologic recovery after myelosuppressive therapy have proved to be models of translational research and have led to novel therapeutic initiatives for patients with cancer and hematologic malignancies. In this article, fundamental aspects of neutrophil production are discussed, and the clinical development of hematopoietic cytokines active on cells of the leukocyte lineages is presented.

Dysplasia and cancer of the cervix may be related to human papillomavirus infection. To determine the presence of cancer in cases of genital warts we have been using a technique, androscopy, for examining male genitals with magnified lens previously sprayed with white vinegar (5% acetic acid). This paper explores the methods of using colposcopy and androscopy on both sexual partners for a thorough examination of genitals and the anorectal area. It also discusses treatment advantages of the CO2 laser. In our investigation plain, pedunculated warts and a giant condyloma (Buschke-Loewenstein tumor) were found and treated with CO2 laser surgery.

Although laparoscopic techniques in gastric surgery have become an important part of upper gastrointestinal surgery, they are mainly utilized for organ preserving procedures. A standardized technique for laparoscopic gastrectomy has not yet been described in detail. The purpose of this study was to develop a standardized technique for laparoscopic gastrectomy with reconstruction by a stapled termino-lateral esophago-je-junostomy and Roux-en-Y-anastomosis in a cadaver model. The resected stomach was removed through an enlarged trocar incision. The described technique has been successfully performed in one patient with early gastric cancer.

Colorectal cancer screening using immunochemical fecal occult blood tests has been performed supported by legislation since 1992 in Japan. Approximately 3.6 million persons were screened and 5,400 cases of colorectal cancer were found in 1993. The effectiveness of the screening program has been evaluated by several observational studies. It has been strongly suggested that risk of dying of colorectal cancer would be reduced by 60% for those screened annually by the program. Japanese screening program today seems to be promising in terms of the mortality reduction, however, issues such as limitation of capacity for diagnostic work-ups should be resolved.

The sensitivity of contrast-enhanced breast MR imaging for malignancy is close to 100%, an improvement over traditional methods of cancer detection. The reported specificity of breast MR imaging, however, ranges from approximately 37% to 97%. This range of values is the result of differences in technique and diagnostic criteria. The variation in biologic activity and vascularity of benign and malignant lesions, as well as normal tissue, also complicates MR imaging diagnosis. A review of breast MR imaging research with an emphasis on specificity issues is also presented.

We assessed immunohistochemical expression of p185 and the proliferating cell nuclear antigen (PCNA) in 68 patients undergoing surgery for non-small cell lung cancer. Adenocarcinoma of the lung was the histological type whose expression of p185 at levels exceeding 10% was greater to a statistically significant degree. Epidermoid carcinoma, on the other hand, was associated with the highest PCNA indices. Although these 2 markers had no prognostic value in the survival study, it must be pointed out that the longest survivors were patients with p185 expression less than 10% in neoplastic cells.

The topoisomerase I inhibitors are an exciting new class of antineoplastic agents currently under clinical development. Analogues of camptothecin with improved toxicity profiles and antitumor activity included CPT-11 and topotecan. CPT-11 has demonstrated activity against a variety of tumor types, particularly colon and lung cancer. Early results with topotecan against ovarian and lung cancer are also encouraging. Combination trials with other antineoplastic agents including cisplatin and etoposide, and early clinical trials with new topoisomerase I inhibitors, such as 9-aminocamptothecin, are underway.

Prostate-specific antigen increasingly is being used as a surrogate end point in trials of new agents in patients with hormone refractory prostate cancer. This article reviews data that support this marker as a surrogate end point and the contradictory data reported recently for trials of suramin. These contrasting views may originate in the different mechanisms of actions of the agents studied. These data suggest that a decline in prostate-specific antigen of at least 50% from baseline may be an important predictor of survival for patients receiving cytotoxic therapy.

The observation that a number of genetic mutations can result in neoplastic transformation has led many investigators to speculate that gene therapy may represent a useful approach to treat cancer. Conceptually, this application of gene therapy seems quite simple: to treat cancer, restore the correctly regulated expression of the needed tumor suppressor genes inside tumor cells and revert the transformed phenotype of such cells. However, such regulated expression has been difficult to achieve in practice. Here we describe recent efforts at such restoration via a novel approach to gene therapy that involves ribozyme-mediated repair of mutant RNA transcripts.
