Encouraged by reports of radioimmunoimaging of colorectal carcinomas and by examining an immunohistochemical report on resected pancreas cancer tissues, we studied the diagnostic potential of radioimmunoimaging with the radioiodinelabeled monoclonal antibody to the surface antigen of a pancreas cancer cell line. A monoclonal antibody (MoAb; HC-1) to a human pancreas cancer cell line (HGC25)5 was labeled with radioiodine and injected into athymic nude mice implanted with human pancreas cancer cells. Antibody HC-1 was cleared from the circulation and accumulated significantly in the implanted tumor sites.

The case history of a 27 year old male ex-intravenous drug user who presented with haemoptysis and indistinct lung shadowing is reviewed. During a 3 week period of investigation his lung shadowing increased dramatically and it became apparent that he had metastases from chorionic gonadotropin-producing testis cancer which had undergone spontaneous regression of the primary tumour. Unfortunately by the time he was referred for treatment he was in respiratory failure and died 48 hours after treatment started. The critical importance of the urinary pregnancy test (available in most casualty departments to diagnose ectopic pregnancy) in accelerating diagnosis and treatment of such a case is highlighted.

The authors report about a series of 22 papillary carcinomas of the thyroid gland. Among the operated patients, 6 underwent loboisthmectomy, 9 total thyroidectomy. For papillary carcinoma, a frequent cancer (1.3% of all cancers) dominating in women, the survival rate is virtually the same if the lesion is treated with maximal or conservative surgery. The results obtained with minimalist conservative procedures (decrease in morbidity, no recurrence) reinforces the authors' attitude, even more so as it corresponds to the data found in the literature.

A case was described in which the brachiocephalic artery ran transversely in front of the fourth tracheal ring. The abnormal course of this artery may present a possible hazard in the event of a tracheotomy. In this case the anomalous artery was accompanied by a shift of the trachea to the right and of the esophagus to the left. As a consequence, the left recurrent laryngeal nerve ascended in front of the esophagus. The anomalous nerve course may present the possibility of injury to the nerve in the event of a curative operation for esophageal cancer.

Inhibitors of ribonucleotide reductase are potential antiproliferative agents, since they deplete cells from DNA precursors. Substrate nucleoside analogues, carrying azido groups at the base moiety, are shown to have strong cytostatic properties, as measured by the inhibition of the incorporation of thymidine into DNA. One compound, 8-azidoadenosine, inhibits CDP reduction in cytosolic extracts from cancer cells. The corresponding diphosphate behaves as a substrate for ribonucleotide reductase while the triphosphate is an allosteric effector.

Compound 6b was concentrated in vitro preferentially in HT-29 human colon carcinoma cells compared to V79 Chinese hamster lung fibroblasts and showed selective retention in PaCa-2 human pancreatic cancer cells grown as solid tumor xenografts in the nude mouse.

A survey to determine demographics, prostate cancer screening practices, and prostate cancer-related knowledge and beliefs was administered to over 1,700 participants at five sites during Prostate Cancer Awareness Week (1991) screening events. Findings are presented by site since significant differences in demographics existed. Results suggest that screenings conducted at the major medical centers attract primarily white males, a number of whom already practice adequate secondary prevention. Thus, if optimal benefit is sought through mass prostate cancer screening, innovative strategies to reach populations that are currently underserved and at risk are necessary.

Molecular biology has identified new receptors and ligands which are deregulated in diseases such as cancer and autoimmune conditions and which provide rational targets for therapeutic intervention. Advances in instrumentation and methodology make it possible to screen large numbers of samples in simple receptor-ligand binding assays in the search for drug candidates. Caution must be exercised in the interpretation of data derived from such assays. This is particularly pertinent to the recently characterized receptors, such as the cytokine receptors, as we do not fully understand the relationship between the receptor type and the linkage of receptors to the appropriate or inappropriate second messenger systems that are used in the experimental screening protocols and the disease state.

Bacillus Calmette-Guerin (BCG) is the most effective agent currently available to treat superficial bladder cancer. However, this form of therapy is not without potential serious or fatal complications. In addition to the potentially toxic systemic side effects attributed to hematogenous absorption of the bacillus, direct upper tract seeding may occur in patients with vesicoureteral reflux. We report on a patient treated with intravesical BCG for bladder cancer in whom unilateral necrotizing granulomatous pyelonephritis developed. Although severe, this complication is rare and we conclude that reflux is not a contraindication for intravesical BCG therapy.

Although blastomycosis is prevalent in the North American continent, it occurs only sporadically in Africa. We describe a 42 yr old patient who complained of intermittent cough and haemoptysis. Clinical findings were strongly suggestive of lung cancer. The diagnosis of pulmonary blastomycosis was made at thoracotomy. This rather unusual disease in our areas caused a considerable delay in securing the diagnosis and we suggest that this infection may be found elsewhere in Africa and the distribution may be wider than has previously been suspected.
