TITLE: Sexual and urinary dysfunction following surgery for sigmoid colon cancer.

We investigated sexual and urinary dysfunction following surgery for sigmoid colon cancer. Thirteen (46.4 per cent) of the 28 male patients with a normal sexual life prior to surgery could no longer ejaculate, but all could maintain erection. With the extension of lymph node dissection, the incidence of a disappearance of ejaculation tended to increase. In particular, in patients undergoing an extended lymph node dissection, the incidence was 53.8 per cent. Urinary dysfunction occurred in 7 (10.3 per cent) of the 68 patients (37 men and 31 women), but was slight to mild. There were no significant differences between the extent of lymph node dissection and urinary dysfunction. In surgery for sigmoid colon cancer, care should be taken to preserve the hypogastric nerves.

TITLE: Treatment of early localized breast cancer in elderly patients by Tamoxifen.

Twenty-seven early localized breast cancer cases in elderly patients, 21 stage I and six stage II, were treated with Tamoxifen only. In 15 cases there was complete radiological remission and in seven cases partial remission or growth arrest. Five cases did not respond to treatment. Tamoxifen seemed to be a useful alternative treatment for this group of patients. Possibly tumour mass and growth rate may affect the response to antioestrogen treatment.

We are promoting a project of preventive medicine for prostatic diseases. As the primary prevention (prevention of morbidity), high risk groups of prostatic cancer were determined by a case-control study. The results are being introduced thoroughly to the people. As the secondary prevention (prevention of development), an original mass screening system using a special mobile unit with two sets of transrectal ultrasonic scanner has been developed. The detection rate of benign prostatic hypertrophy and prostatic cancer in males over 55 years of age was approximately 30% and 0.6%, respectively.

Nontraumatic Clostridium septicum infections may present as either septicemia or as metastatic myonecrosis. Most of these infections occur in debilitated patients with diabetes who are receiving cancer chemotherapy. The majority have a hematologic abnormality or a carcinoma of the colon. Usually there is an ulcerative lesion of the gastrointestinal tract that serves as the portal of entry. While most of these patients die from overwhelming sepsis, our patient was debrided early and treated promptly with high-dose penicillin therapy as well as hyperbaric oxygen therapy. He fully recovered from C. septicum gas gangrene and underwent resection of a recurrent colonic cancer.

Secondary tumors were noted in 8 out of 675 patients with Hodgkin's disease during remission following treatment. The following diseases were diagnosed:acute myeloblastic leukemia (1), lung cancer (2), gastric cancer (3), cervical cancer (1), and basalioma (2). The incidence of secondary tumors among patients with Hodgkin's disease turned out to be much higher than in general population. Complications were more frequently observed in men after combined therapy (radio- and drug therapy).

We investigated the present incidence of lung cancer and its distribution by histologic type in Nagasaki City using Nagasaki Tumor Registry data from the years 1973-1977. The risk of radiogenic lung cancer did not increase significantly in either sex. The difference in relative distribution by histologic type between the exposure and non-exposure groups was not significant. Further analyses should be carried out on the risk present for individuals who were young at the time the bomb was dropped and the effect of exposure distance on the incidence and histologic types of lung cancer in A-bomb survivors.

Cohort mortality between 1875 and 1975 in Japan from cancers of the oesophagus, tongue, and larynx were examined. Death rates were compared with per capita alcohol and tobacco use which are major aetiological agents for these tumours in Westernized countries. Cohort mortality from cancer of the oesophagus remains high for both sexes in a country with historically low alcohol consumption. More consistent with this low drinking pattern are the rates for cancers of the tongue which remain among the lowest in the world. Cancers of the larynx, which have been associated with tobacco and alcohol, declined among successive cohorts, which was in contrast to the sharp cohort increases for lung cancers and tobacco use in Japan during the same period.

TITLE: Intraoperative localization of occult breast tumours.

Mammographic examination of the clinically normal breast can identify some breast masses before they become palpable. In order to remove them successfully, a surgeon requires the help of some localization procedure. The only method recommended to date has been preoperative needle localization under radiologic control by a radiologist. A technique of intraoperative needle localization by the surgeon without radiologic control has been developed at St. Michael's Hospital, Toronto. The method saves time, avoids complications and is very accurate. Because of the possibility that the mass is malignant, the excisional biopsy is performed as a definitive lumpectomy. Of 112 consecutive occult tumours so localized, only 3 required removal at a second operation. Twenty-six (23%) of the lesions proved to be cancer (16% invasive, 7% noninvasive).

The results of combined radiotherapy of corpus uteri cancer in 54 patients with Stages I-III of the disease using an Agat-B remote afterloading apparatus were analysed. The therapeutic efficacy, frequency and severity of radiation complications were assessed. Direct clinical cure was achieved in 52 (96.3%) women. Of the 52 followed-up patients 43 (82.7%) have been alive for 1 year, after 3 yrs. 20 (70.4%) out of the 27 followed-up patients. It is possible to infer from the above data that the results of this therapeutic method are quite satisfactory.

Linear accelerators play an important role in the treatment of some cancer patients. These accelerators produce high-energy X-rays (in the megavolt (MV) range) and the effective penetration of the X-rays depends on the energy. The percentage depth dose at the central axis gives an indication of effective penetration. An attempt was made to derive formulae for the percentage depth dose for 8 and 16 MV X-rays in tissue. The formulae can be used for treatment planning purposes and also for calculating the integral absorbed dose. A further important application of the formulae is the calculation of the central absorbed dose for two opposing beams at non-standard treatment distances.

Pharyngoesophageal reconstruction continues to be a problem in the management of cancer involving the hypopharynx. In our experience, the use of deltopectoral and pectoralis major myocutaneous flaps for total reconstruction has been disappointing. We report four cases of immediate near-total reconstruction using a quilted , skin-grafted pectoralis major muscle flap. All cases had received full dosage preoperative irradiation. A thin-walled pharynx was created that has allowed early development of a good esophageal voice. There has been no stricture formation. All patients have experienced no difficulty in establishing a good oral intake.

Plasma concentrations of morphine were studied in 16 patients with chronic pain due to advanced cancer, treated with regular doses of morphine chloride by the oral route. Doses ranged from 20 to 125 mg every 4 h and a significant, positive, linear correlation was found between dose (range 0.22-3.01 mg/kg) and mean plasma concentration (range 22-364 ng/ml). The profile of the 4 h concentration curve is described and found to be subject to considerable, individual variation. The present investigation supports the experience that good pain relief can be obtained, if individual requirements as to dose and dose interval are taken into consideration.

With the advances in the management of various neoplastic diseases and subsequent improvement in "disease-free" states, complications of therapy--particularly, infectious complications--have evolved as stumbling blocks to survival. Among neutropenic (absolute neutrophil count below 1,000/mm3) patients with cancer, infection is the major autopsy-determined cause of death. With expected "cure rates" of childhood leukemia approaching 60 to 70 percent, it seems unreasonable to lose such patients to an infectious cause of death, yet this, indeed, happens. The purpose of this review is to (1) define the magnitude of the problem; (2) describe the various agents responsible for infections in neutropenic patients; (3) attempt to more sharply define degrees of neutropenia and mechanical defenses; and (4) consider various approaches to studying and treating these infections.
