A case of a CA19-9-producing ureteral cancer is reported. A 58-year-old man presented with gross hematuria. Retrograde pyelography showed an irregular filling defect in the right ureter. The serum CA19-9 level was 932 U/ml (normal < 37). Right total nephroureterectomy was performed. The histological diagnosis was grade 3 transitional cell carcinoma. Immunohistochemical analysis showed CA19-9 to be expressed not only in the cancer cells but also in the normal transitional cell epithelium of the renal pelvis.

Lung cancer is known to carry multiple cytogenetic alterations. Among such chromosomal abnormalities, deletions at the short arm of chromosome 3 has been shown to be the most frequent and can be detected even in hyperplasia of the bronchial epithelium, suggesting its important roles in the pathogenesis of lung cancer. In this chapter, we describe current status of the positional-cloning attempts to identify the target tumor suppressor gene at 3p in our laboratories as well as in other laboratories.

Although a review of the literature on aging and cancer chemotherapy is not included in this particular presentation, information is very sparse. Thus, there is significant potential for valuable contributions from clinical research through partnerships between geriatric clinical pharmacologists and medical oncologists.

Authors reported on transplantation glomerulopathy (TGP), occurred in a patient's cadaver kidney allograft, as well as on orchitis, lymphogranulomatosis (Hodgkin's lymphoma) and lip cancer detected thereafter in the same patient. While TGP was definitely be verified morphologically, all other observed diseases are commonly and typically met with membranous glomerulopathy (MGP), which is a separate entity. Similar case has not yet been reported in the literature. Since in majority of the cases the MGP is a paraneoplastic phenomenon, and the TGP is a part of chronic rejection in kidney allografts the differential diagnosis may have therapeutic consequences.

Encouraged by reports of radioimmunoimaging of colorectal carcinomas and by examining an immunohistochemical report on resected pancreas cancer tissues, we studied the diagnostic potential of radioimmunoimaging with the radioiodinelabeled monoclonal antibody to the surface antigen of a pancreas cancer cell line. A monoclonal antibody (MoAb; HC-1) to a human pancreas cancer cell line (HGC25)5 was labeled with radioiodine and injected into athymic nude mice implanted with human pancreas cancer cells. Antibody HC-1 was cleared from the circulation and accumulated significantly in the implanted tumor sites.

The case history of a 27 year old male ex-intravenous drug user who presented with haemoptysis and indistinct lung shadowing is reviewed. During a 3 week period of investigation his lung shadowing increased dramatically and it became apparent that he had metastases from chorionic gonadotropin-producing testis cancer which had undergone spontaneous regression of the primary tumour. Unfortunately by the time he was referred for treatment he was in respiratory failure and died 48 hours after treatment started. The critical importance of the urinary pregnancy test (available in most casualty departments to diagnose ectopic pregnancy) in accelerating diagnosis and treatment of such a case is highlighted.

The authors report about a series of 22 papillary carcinomas of the thyroid gland. Among the operated patients, 6 underwent loboisthmectomy, 9 total thyroidectomy. For papillary carcinoma, a frequent cancer (1.3% of all cancers) dominating in women, the survival rate is virtually the same if the lesion is treated with maximal or conservative surgery. The results obtained with minimalist conservative procedures (decrease in morbidity, no recurrence) reinforces the authors' attitude, even more so as it corresponds to the data found in the literature.

A case was described in which the brachiocephalic artery ran transversely in front of the fourth tracheal ring. The abnormal course of this artery may present a possible hazard in the event of a tracheotomy. In this case the anomalous artery was accompanied by a shift of the trachea to the right and of the esophagus to the left. As a consequence, the left recurrent laryngeal nerve ascended in front of the esophagus. The anomalous nerve course may present the possibility of injury to the nerve in the event of a curative operation for esophageal cancer.

Although the 5-year survival rate (53%) for persons with oral cancer remains low, survival varies by stage at diagnosis (4). Detection of oral cancers by oral examination can reduce morbidity and death associated with this problem (5). To characterize examinations for oral cancer among U.S. adults, CDC analyzed data from the 1992 National Health Interview Survey-Cancer Control (NHIS-CC) supplement. This report summarizes findings from that analysis.

In 75-80% of all clinically significant prostate cancer, the peripheral zone appears to be hypoechoic on transrectal ultrasound (TRUS). However, examination by TRUS alone in a screening program is not recommended due to its low sensitivity. The combination of prostate-specific antigen, digital rectal examination, and TRUS increases the detection rate of prostate cancer. In prostate cancer arising from the transition zone, TRUS findings are often nonspecific. Even prostate biopsies might be negative due to the location of this tumor. Anterior biopsies of the transition zone will identify these prostate cancers. Color Doppler ultrasound of the prostate has little additional value over TRUS alone in the diagnosis of prostate cancer.

Chemotherapy has been shown to prolong survival and to be cost effective in the palliative treatment of non-small-cell lung cancer. A meta-analysis of 11 randomised trials has found a small benefit from combination chemotherapy in advanced NSCLC, increasing one-year survival rates from 5% to 15%. New combination regimens, especially carboplatin-paclitaxel, show higher response rates in selected patients, and promise improved survival rates, but this has yet to be confirmed in randomised trials.
