TITLE: [Anal condyloma: its management is still difficult]
PUBDATE: 20011001
Anal condylomas result from papillomavirus infection, the most common sexually transmitted disease. The principal risk is the development of cancer of the anal canal. The risk of contamination is high, even after a single sexual contact. No specific antiviral treatment is available and no consensus has been reached on the appropriate treatment of anal condylomas. Despite the development of various methods (interferon, imiquimod), electrocoagulation remains the treatment of choice. Finally, regular follow-up and treatment of the partner(s) are essential.

TITLE: Mismatch repair defects in cancer.
PUBDATE: 20000401
Post-replicative mismatch repair in humans utilises the hMSH2, hMSH6, hMSH3, hMLH1 and hPMS2 genes and possibly the newly identified hMLH3 gene. Recently, a link has been established between hMSH6 mutations and 'atypical' hereditary non-polyposis colon cancer (HNPCC) with an increased incidence of endometrial cancers. To satisfy the need for a diagnostic test capable of differentiating between pathogenic mutations and polymorphisms, several functional assays that fulfil these criteria have been described. These should allow for better diagnosis of HNPCC.

PUBDATE: 20001001
The perspective of urologic oncology in the twenty-first century is presented regarding detection, treatment, and prevention of cancer. There is increasing importance of a cancer information network. Biotechnology and medical engineering based on advances in computer science will be the two key technologies related to such advancement. Ethical, legal, social, and economic issues provoked by the technologic advances will increase in gravity at the same time. Balancing humans in terms of scientific technologic developments and state of mind will be a serious issue.

The authors have recited its causes through the case of a young papillary thyroid cancer patient during the diagnostics (aspiration cytology, diagnostic imaging technics, DNA-analysis) the treatment (surgery-external radiotherapy-radioiodine treatment) and the follow-up (HTG, diagnostic imaging).

This time, in 3008 lung cancer patients, the postoperative results were analyzed under the new stage grouping of TNM classification. All of those patients underwent the operation in 1989, and the 5 year-survival rates had beeb surveyed in 1996 by JACS (The Japanese Association for Chest Surgery). Under the new TNM classification established in 1996 worldwidey, T3N0M0 was transferred from IIIA to IIB. This report is the additional one in the focus of the results accompanied with the change of TNM classification.

This finding confirms that Cowden disease, a dominant cancer predisposing syndrome, and Bannayan-Riley-Ruvalcaba syndrome, which includes macrocephaly, multiple lipomas, intestinal hamartomatous polyps, vascular malformations, and pigmented macules of the penis, are allelic disorders at the PTEN locus on chromosome 10q.

Insulin-like growth factors (IGF) are pleiotrophic polypeptides affecting all aspects of growth and development. The IGF system, including ligands, receptors, binding proteins and proteases is also involved in pathophysiological conditions, such as cancer and degenerative conditions. In this review, the actions and interactions of the IGF system as it relates to Alzheimer's disease will be investigated.

A 57-year-old woman presented with subacute sensory, ataxic neuronopathy. Clinical investigation revealed a right-sided non-small-cell lung cancer. Serum investigation for specific antineuronal antibodies was negative. Histology showed T lymphocytic infiltrates in dorsal root ganglia. The observed histological pattern is similar to that described in antibody-positive cases. Thus, these findings suggest similar pathways in specific antineuronal antibody-negative and -positive cases of paraneoplastic subacute sensory neuronopathy.

PUBDATE: 20000701
The genes responsible for each of the inherited syndromes of intestinal polyposis and colorectal cancer are now known. This knowledge has led to genetic testing for syndrome diagnosis and for determining which persons in an affected family actually have the condition. Genetic testing has also allowed a more precise clinical characterization of each of the syndromes and their subtypes. Optimal application of genetic testing, proper cancer prevention, and management of nonmalignant features of each of the syndromes are the next goals. This report summarizes each of the syndromes, their genetics, and management and provides an approach for genetic testing.

TITLE: [A study on the spectra of L-fucose]
PUBDATE: 19981001
The concentration of L-fucose will extraordinarily increase in urine in patients with early stage of cancer. It seems that this is a significant clue for early phase diagnosis of cancer. Unfortunately the sensitive detection of L-fucose is difficult. Thus we carried out researches on IR-spectrum, Raman spectrum, ultraviolet absorption spectrum and ultraviolet fluorescence spectrum. We compared the results from patients and healthy subjects. It is confirmed that the spectroscopic method, especially ultraviolet fluorescence spectrophotometry, may be suitable for early stage screening of some kinds of cancer. This test is rapid, inexpensive and painless.

PUBDATE: 19980701
Developments in the adjuvant therapy of colorectal cancer have been reported recently. Important questions regarding the most appropriate regimen, the optimal duration of treatment, the preferred route of administration, and which subgroups are most likely to benefit are addressed. A number of studies in rectal cancer considered the timing of chemoradiation, mode of administration of chemotherapy, and the role of intraoperative radiotherapy. Quality of life issues also are being addressed. Several new agents and novel therapies have produced interesting and, it is hoped, clinically useful results. The prognostic significance of a number of molecular markers also is the subject of ongoing research.

PUBDATE: 20011101
Decreased aerobic (hypoxic) conditions in tumors induce the release of cytokines that promote vascularization and thereby enhance tumor growth and metastasis. Recent major advances have provided insight into the role hypoxia plays in cancer biology. The domain structure of the hypoxia-inducible factor 1alpha (HIF-1alpha) has been elucidated, as has the mechanism by which stabilization of HIF-1alpha leads to initiation of the transcription of target genes involved in growth of blood vessels.

PUBDATE: 20010701
A case of massive postoperative ascites in a woman treated for endometrial cancer is reported. A workup for typical causes of ascites yielded negative results, prompting a more detailed analysis of the patient's condition. Hypothyroidism was discovered. After correction of the hypothyroidism, the ascites slowly resolved. Since myxedema is an uncommon cause of ascites, this is usually a diagnosis of exclusion. However, hypothyroidism must be ruled out to prevent unnecessary and possibly inappropriate treatments for ascites.

STATEMENT OF PROBLEM: Investigators have attempted to resolve the question: do psychosocial, emotional, and attitudinal factors have any effect on cancer survival? PURPOSE: This article presents a literature review and an overview of the developments in the mind-body connection to examine the influence of attitude in cancer survival. CONCLUSION: Even though a causal effect of emotional issues on the initiation and promotion of cancer is speculative, the effect of emotions on numerous medical conditions cannot be ignored.

TITLE: Combination therapy for early Alzheimer's disease: what are we waiting for?
PUBDATE: 19981001
The practical pharmacological approaches currently available to palliate the cognitive and functional losses in early Alzheimer's disease (AD) include cholinesterase inhibitors (ChEI), antioxidants (e.g., vitamin E), anti-inflammatory agents, estrogen, seligiline, vasoactive agents, and ginkgo biloba. Reviewing available data on these therapies and using models from medical illnesses such as cancer and hypertension, we highlight the urgent need for evaluating combination therapies in early AD.
