The pathogenesis of the Guillain-Barre syndrome (GBS) is felt to involve alterations in cellular and humoral immune responses. We report the case of a 44-year-old female with stage IV breast cancer who developed GBS 2 days after receiving high dose chemotherapy with bone marrow and peripheral progenitor cell support, and granulocyte-macrophage colony stimulating factor (GM-CSF). Her clinical status stabilized with plasmapheresis. This report lends further support to hypotheses regarding cellular immune alterations in the pathogenesis of GBS.

We reviewed our experience performing mediastinoscopy as an outpatient procedure. From January 1, 1987, to December 31, 1992, 1,015 of 1,062 cervical mediastinoscopies were performed on an outpatient basis. There were no operative deaths. Complications developed in 14 patients, 10 of whom required hospitalization. Two additional patients were hospitalized for social reasons. Supraventricular arrhythmia was the most frequent complication requiring admission to the hospital. This article documents the ability to perform mediastinoscopy safely as an outpatient procedure and discusses why this is our preferred approach in managing patients with lung cancer.

We summarized here the urological emergency status accompanied by cancer, such as urinary obstruction, bleeding and renal failure. Hydronephrosis occurs due to obstructive change and post-renal failure finally results after complete obstruction of bilateral ureters. Patients with obstructive disease are treated with percutaneous nephrostomy or indwelling double-J catheter. Bladder tamponade develops after bleeding of the urinary tract. Patients with bleeding are treated by bladder irrigation or transcatheter arterial embolization. Acute renal failure (ARF) includes pre-renal ARF, renal ARF and post-renal ARF, necessitating emergency hemodialysis. The emergency status can be often observed simultaneously, so careful examination and rapid treatment are necessary.

Breast cancer is the most frequently encountered cancer in women and the first cause of death in the female population. Metastasis may occur ubiquitously depending on the histological type and the presence or absence of oestroprogesterone receptors. We report three cases with gastric metastasis. Signs of gastric involvement are often non-specific and should not be mistaken for simple side reactions to treatment. Hormone chemotherapy is indicated in most situations but surgery may be needed. It should be noted that gastrointestinal metastasis predominates in lobular breast cancers positive for progesterone receptors.

To improve the survival outlook for patients with small cell lung cancer a variety of chemotherapy strategies has been adopted. This review focuses on the evidence for schedule dependency, particularly with regard to etoposide, and reviews the use of alternating chemotherapy protocols and weekly regimens in small cell lung cancer treatment. Recent improvements in supportive care, with the use of haemopoietic growth factors and peripheral blood progenitor cells, have led to renewed interest in the concept of dose intensity. Preliminary results of dose intensification in small cell lung cancer are described and future prospects discussed.

In response to a variety of "emergencies", cells produce several stress proteins (heat shock proteins, HSP) that provide them with higher resistance and are able to repair protein damage. Their expression is regulated by heat shock transcription factors. Stress proteins are arousing growing interest in medicine, as major antigens in some infections and certain autoimmune diseases and also because of their possible involvement in the development and therapy of cancer. Elevated levels of stress proteins protect tissues and organs against ischemic injury and reduce infarct size. Stress proteins may also serve as a marker in diagnostic studies and in toxicology.

The effect of complex therapy (intra-arterial chemotherapy and radical surgery) on primary tumours and their metastases was studied in 38 oral squamous cell cancer cases. The therapy had different effect on the primary tumours and their lymph node metastases. There was a close correlation between the number of metastatic lymph nodes and survival. The extension and the chemotherapeutic regression rate of metastatic lesions showed a significant correlation with patients' survival.
