Twenty evaluable patients with advanced cervical cancer refractory to one prior chemotherapy regimen were treated with a 5-day schedule of fludarabine phosphate. No responses were noted. The major toxicities were anemia and leukopenia. Based on this study, fludarabine phosphate does not appear to be an active agent for patients with chemotherapy refractory cervical cancer.

Survival time of 119 consecutive patients with metastasizing carcinoma of the breast who had been treated with various chemotherapeutic schedules after radical operation was determined retrospectively. 57 women received chemotherapy and hormones, as well as radiotherapy, while 62 had radiotherapy plus hormone treatment. The mean prolongation of life in the former group over that of the latter was 12 months (p less than 0.01). Although chemotherapy delayed death, the dynamics of cancer were not fundamentally affected. The chemotherapy-induced prolongation (2.5 months to five years) was independent of any risk, primary location of the metastases or age.

The connective tissues underlying the precancerous changes of the epithelia were separated and combined with the epithelia which were separated from the normal mucosa. The combinations were transplanted in nude mice. Specimens were taken at 6 and 10 weeks after transplantation and routinely processed for microscopic examination. The results show that the connective tissue underlying epithelial dysplasia may induce the normal epithelium undergoing epithelial dysplasia changes. The experiment demonstrates the etiological importance of the connective tissue in the development of cancer, and may be applied clinically in the treatment of precancerous lesions.

TITLE: Characteristics of blood flow in cancer of the uterine cervix.

Blood flow characteristics were measured in patients with histologically proved cervical cancer and compared to normal values. Flow parameters were derived by color and pulsed Doppler ultrasound. The results showed a significant difference in the mean values of resistance index and pulsatility index among the diseased patients and healthy controls. However, the method does not have the required sensitivity for screening purposes, so that its value may mainly be in the follow-up of patients treated conservatively with radiation and chemotherapy.

Evidence indicates progress in the early detection of breast cancer. After 1982 there was a reported increase in the incidence of breast cancer, presumably as a result of increased screening for the disease. The increase was especially noted in early stage breast cancer. The incidence of in situ cases also increased. Since 1973 the San Francisco-Oakland Bay area has reported a statistically significant decrease in mortality for white women. The evidence supports continued efforts at early breast cancer detection.

Based on a series of 220 cases of reconstruction (63 cases) or correction (67 cases) of the nipple-areolar complex and various extramammary procedures (tattoo scar revision, lips, eyebrows, eyelashes) (90 cases), the authors present their 2-year experience of medical dermography in a cancer centre. Dermography allows breast reconstruction to be completed in a large number of patients who had initially refused to complete their reconstruction. This minimally aggressive outpatient technique which reconstitutes the areolo-nipple complex with sufficient quality, is now part of reconstruction protocols and is gradually replacing surgical techniques.

The majority of cancer patients (95%) whatever the malignancy location are shown to develop various pathological process in the lungs. 20% of lethal outcomes occur due to acute and protracted croup and focal pneumonias. Such pneumonias are often found after the patients death because of a symptomless and atypical course. These findings dictate a necessity of comprehensive pulmonological studies in cancer patients and administration of detoxication therapy to manage the bronchial obstruction syndrome registered in 39% of cancer patients probably as an early stage of further pulmonary impairment.

Sera of patients with chronic hepatitis and chronic haemodialysed uraemia were tested for both Hepatitis B virus and Hepatitis Delta virus markers from 1985. Hepatitis Delta virus seroconversion was found in 10 of 88 haemodialysed patients and anti-Delta-IgG sero-positivity in 11 of 108 chronic hepatitis patients. The clinical course of Delta superinfections in haemodialysed patients was as follows: 3 infections were symptomless, 7 patients had acute hepatitis, two of the latter group died because of fulminant hepatitis. Of the 11 HBsAg and anti-Delta-IgG positive chronic hepatitis patients 3 had primary hepatocellular cancer.

Earlier physical maturity in girls, involving an earlier and more marked growth hormone spurt and also an earlier menarche, is a marker of subsequent higher risk to breast cancer. Based on the results of recent research, it is postulated that interaction between growth hormone, insulin-like growth factor and sex steroids at an earlier age, has a major role in stimulating not only linear growth but also precocious proliferative activity in the breasts of adolescent girls. This may promote carcinogenesis in a susceptible mammary epithelium and could partly account for the rising breast cancer mortality rate among both Japanese and Western women.

From April 1989 to June 1991 63 patients with esophageal cancer were investigated by endosonography with the object of ascertaining the depth of intrathoracic tumor infiltration and lymphnode involvement. The sensitivity in diagnosing tumor infiltration amount to 0.74; the sensitivity for involvement of regional lymphnode was 0.84, specificity 0.44. In an in-vitro analysis endosonographic criteria for the assessment of regional lymphnodes are proved. Echogenic structure seems not to be a valuable criterium.

TITLE: The management of metastases to eye and orbit from carcinoma of the breast.

This report describes 11 patients who developed visual symptoms due to choroidal or orbital metastases from carcinoma of the breast. All patients were treated using palliative radiotherapy with regression of metastases in five of six patients (83%) having choroidal involvement and three of five patients (60%) having orbital metastases. There were no complications due to radiotherapy. All these patients had advanced metastatic breast cancer with poor survival following development of orbital or choroidal metastases.

Physiotherapists, whether trained in intensive care or internal medicine, must be attentive to patients at the end of their lives. Palliative-care physiotherapy requires basic training in rehabilitation techniques; the integration of a rehabilitation professional in a caregivers team; a profound communication with the patient, leading to an effective accompaniment and direct response to the patient's needs; and recognition of the patient's functional needs. The involvement of the physiotherapist can extend to the very end of life. In a series of 56 geriatric patients (35 suffering from cancer, 21 from other pathologic conditions) who died at the Center of Continuous Care, 44 (79%) had a treatment of respiratory physiotherapy until their last 24 hr.
