TITLE: [Esophagojejunostomy--how safe is manual suture today?]

Between 1982 and 1986 a total of 137 patients were gastrectomized for cancer of the stomach. The esophago-jejunal anastomosis was always carried out as an end-to-side anastomosis applying the conventional two layer technique. Only in 1 patient (0.7%) an insufficiency of this anastomosis was observed; 9 reoperations (6.6%) were necessary and 3 patients (2.2%) died in the postoperative course. These results confirm that with a precise, standardised conventional suture technique the esophageal anastomosis can be safely performed today.

Twenty-six evaluable patients with advanced nonsquamous cell cervical cancer were treated with aminothiadiazole at a dosage of 125 mg/m2 weekly. Sixteen had received prior chemotherapy. Two patients had complete responses neither had received prior chemotherapy; nine had stable disease; 15 had increasing disease. There was a single life-threatening toxic episode. Aminothiadiazole used in this dosage and schedule has minimal activity in previously treated patients with nonsquamous cell carcinoma.

In 53 patients suffering from bladder cancer the acelylization phenotype was examined. The frequency of slow acetylizers (70%) was significantly higher than in the control group. The more frequently occurrence of slow acetylizers in the group with professional pollutant exposition is an important reference to a higher bladder cancer morbidity in the case of carcinogen contact. A screening of the acetylization phenotype in professions with carcinogen contact may be important in the prophylaxis of bladder cancer.

A systematic approach to identifying the cause of pain and rational use of drug therapy are keys to providing pain relief to cancer patients. Aspirin, acetaminophen and nonsteroidal anti-inflammatory drugs are effective for mild to moderate pain, and they enhance the effectiveness of weak oral narcotics, such as codeine. For severe pain, morphine is the drug of choice. A variety of adjuvant drugs can be used to enhance the effect of narcotics and to treat specific side effects of the disease or of therapy. For the terminally ill patient, a peaceful death with dignity should always be possible.

A case-control study of maxillary sinus cancer was performed in Hokkaido with 116 cases and 232 controls matched for sex, age (within 5 years), and residence (same health center region). Preliminary univariate analyses showed that a history of chronic sinusitis [relative risk (RR) = 3.1]; nasal polyps (RR = 5.7); an occupational history of being a carpenter, joiner, furniture maker, or other woodworker (RR = 2.9); and a history of current or past cigarette smoking (RR = 3.3) were statistically significant risk factors for men. No single item was a significant risk factor for women.

TITLE: [Local use of metronidazole in dimethyl sulfoxide in the radiation therapy of cervical cancer]

A method of the local use of metronidazole dissolved in dimethylsulfoxide (DMSO) for cervix uteri cancer patients was worked out. Applications of 1-2 g of metronidazole were well tolerated by the patients. Metronidazole concentrations in cervical tumors were high (about 1000 micrograms/g), in the blood they did not exceed 16 micrograms/ml. Experiments showed that metronidazole in DMSO diffused in the tissue, its concentrations at a distance of 2-3 cm from the surface were 180-260 micrograms/g. The local use of metronidazole in DMSO caused an increase in the rate of tumor radiation regression.

Calcitonin and parathyroid hormone concentrations were determined using a radioimmunoassay in the blood serum of lung, breast and kidney cancer patients who had undergone combined treatment for major disease, healthy males, patients with spinal tuberculosis and patients with eosinophilic granuloma of the cranial bones. A significant rise of the calcitonin level and change in the ratio of calcitonin and the parathyroid hormone were established in the blood serum of patients with tumor metastases to the skeleton, spinal tuberculosis and eosinophilic cranial granuloma. During cancer patients' monitoring the determination of calcitonin is recommended as a screening test for skeletal metastases to select patients for gamma-topographic investigation.

TITLE: Immunohistochemical measurement of estrogen receptors in breast cancer tissue samples.

A new estrogen receptor immunocytochemical assay (ER-ICA) which uses monoclonal antibodies to the estrogen receptor protein was applied to 97 breast cancer tissues. The results were correlated to those obtained by conventional dextran coated charcoal assays. The presence or absence of nuclear staining was significantly associated with positive or negative estrogen receptor status by dextran-coated charcoal (P less than 0.001). Furthermore ER-ICA results showed a high degree of correlation with a light microscopic grading. The relationship between ER-ICA results and response to endocrine therapies in patients with advanced disease was assessed in 20 patients. Six of 11 (55%) ER-ICA positive patients responded, whereas 8 of 9 (89%) ER-ICA negative patients failed.

Teletherapy or radiotherapy combined with 131I for thyroid cancer metastases to the bones caused the development of bone reparation in 70%, in 17% the picture remained unchanged; signs of further tumor growth were observed in 13%. An early x-ray sign of therapeutic efficacy was the decreased soft tissue component of a metastatic tumor 3-5 mos after the initiation of therapy. Disease stabilization was observed in 2.5-3 yrs. X-ray manifestations of bone reparation looked as sclerotic changes along the periphery of a focus of lesion and depended on the sizes of a metastasis and its localization in the skeleton.

During the past twenty years, evidence has been accumulating of a significant association between concentration levels of urban air pollutants and acute or chronic respiratory diseases, including lung cancer. Identification of the responsible atmospheric pollutants is necessary for the development of rational air standards and effective control strategies. The main sources of information are experimental animal studies, controlled exposure of human volunteers to specific pollutants at realistic concentrations, and "in vitro" bioassays, concerning genotoxicity and biological effects of toxic gases and particles on the main target cells of the respiratory tract, particularly alveolar macrophages.

Two patients who had unresectable pancreatic cancer were treated with a combined chemotherapy using Cisplatin, PSK and UFT. A remarkable decrease of tumor size was observed on CT. These two cases corresponded to partial response (PR) according to the response criteria proposed by Koyama-Saitoh. No significant side effects were observed during this therapy. From the results in these two cases, this combination chemotherapy was considered to be one of the most effective antineoplastic therapies available for pancreatic cancer.

Between 1973 and 1984, 27 patients with a cancer of the esophagus or the cardia and suffering dysphagia underwent palliation with an endoluminal prosthesis. Intubation under esophagoscopy or through a gastrotomy, or both, allowed the placement of 23 Celestin tubes and 4 Mousseau-Barbin prostheses. The early postoperative death rate was 11%. The overall morbidity was 37%. It included such complications as tube displacement (18.5% of patients), tracheal compression by the tube (7.4%), obstruction of the prosthesis (11%), esophageal fistula (7.4%), aspiration (7.4%) and wound infection (7.4%). The ability to swallow was acceptable in 24 patients.

Recent changes in hormonal therapy for breast cancer were reviewed and discussed. In hormonal therapy for metastatic or recurrent cases, major forms of ablative therapy other than oophorectomy have been less popular because of the introduction of the hormone receptor assay, tamoxifen therapy, high-dose medroxyprogesterone acetate therapy, etc. Together with this, combined modality therapy including chemo-endocrine therapy or multi-hormone therapy has been investigated. Hormonal therapy as a post-surgical adjuvant therapy consists of oophorectomy (surgical or irradiation) and long-term administration of tamoxifen. Although these treatments are still associated with controversial results in several reports, further study and evaluation should be continued.
