TITLE: Presence of the rare D-variant heat-stable, placental-type alkaline phosphatase in normal human testis.

In 11 adult testes studied, about 0.3 to 4.6% of the total alkaline phosphatase activity was heat stable and L-phenylalanine sensitive but L-homoarginine insensitive. The testicular heat-stable enzyme was more susceptible to inhibition by L-leucine and ethylenediaminetetraacetate than were the normal placental and intestinal enzymes. By antibody-directed enzyme inhibition test, the testicular heat-stable enzyme cross-reacted completely with normal placental enzyme but clearly distinguished itself from a heat-stable component of normal intestinal enzyme. Thus, placental alkaline phosphatase D-variant is synthesized in testis, indicating that the gene for elaborating this placental protein is probably already active in the testicular cells. The high incidence of this protein in cancers of testis and ovary is probably due to its increased production by gonadal genes present in the genome of these particular tumors.

The available knowledge on this problem is comprehensively reviewed to present, in a single source, the requisite knowledge for any physician treating head and neck cancer. Metastatic spread is reviewed according to incidence, detection methods, anatomical sites, and correlation with characteristics of the primary tumor. Finally, methods of detecting metastasis are reviewed and a practical approach to patient evaluation recommended.

A case of adrenal insufficiency secondary to metastatic colonic carcinoma is reported. Because the patient was taking prednisone for an unrelated condition, his initial presentation mimicked isolated aldosterone deficiency with an elevated serum potassium of 8.7 mmol/l. Cortisol deficiency was documented by ACTH testing. The diagnosis of metastatic disease causing adrenal insufficiency was suspected after a computed body scan of the abdomen revealed marked bilateral adrenal enlargement. Adrenal insufficiency secondary to metastatic cancer is reviewed.

TITLE: Clinical conditions mistaken for metastatic cancer to the choroid.

Thirteen patients who were referred to the Oncology Unit with the diagnosis of metastatic cancer to the choroid were studied and found to have other conditions. All of the patients were female. Nine of the 13 had a history of systemic cancer of which 6 were breast cancer. Five patients were currently being treated with chemotherapeutic agents for their systemic cancer. Three of the 13 patients had bilateral fundus lesions. The most common diagnostic entities confused with metastatic disease were either inflammatory or hemorrhagic lesions of the retina. The preceding history of cancer in the patient with a puzzling fundus lesion may predispose the referring ophthalmologist to suggest a diagnosis of metastatic cancer to the choroid.

Using a computerized mathematical model the authors found that either a small [5%] or large [50%] hormone-dependent reduction in tumour cell population at the beginning of adjuvant chemotherapy for breast cancer greatly increases the probability fo achieving a cure. The fact that even a minor response to hormonal therapy early in the treatment of the tumor is effective underlines the importance of using estrogen receptors to to select potential responders in whom a cure is at least a strong theoretical possibility.

Some recent studies on the psychological trauma endured by patients with breast disease are summarized and a number of conclusions are drawn. There appear to be a number of stages through which a patient with a breast lump passes psychologically. Breast cancer attitudes in a community are dependent upon accumulated experience and cannot be changed overnight. Delay in presentation results from unconscious factors which may not be amenable to rational public health appeals. It is possible to identity a group of patients high at risk in breast surgery, although human resilience is such that the outcome if often satisfactory. There is a great need for controlled prospective studies in many areas of breast surgery rehabilitation. Some personal observations are included to stimulate discussion and interest in the area.

TITLE: Value of postirradiation screening for thyroid nodules. A controlled study of recalled patients.

We determined the occurrence of palpable thyroid abnormalities in 214 persons with verified history of head and neck irradiation and in 243 concurrently examined, nonirradiated persons (control subjects). Examinations were performed by two physicians, each unaware of the medical history of the examinees and of the findings of the other examiner. Thyroid nodules, either single or multiple, occurred in 6.1% of irradiated persons and in 4.5% of control subjects (95% confidence interval of the difference, -2.6% to +5.7%). Thyroid enlargement without nodules occurred in 5.6% of irradiated persons and in 3.7% of control subjects (95% confidence interval of the difference, -2.0% to +5.8%). Thyroid cancer was detected in one irradiated and in two nonirradiated persons. Our controlled study did not find an excess number of thyroid abnormalities in previously irradiated subjects.

An in-vitro double labelling technique with 3H- AND 14C-Thymidine was applied to biopsy specimens of the human urinary bladder mucosa in order to determine proliferation kinetics of normal bladder epithelium as well as its benign and malignant alterations. Application of above mentioned technique led to evaluable autoradiographs. The mean 3H-Thymidine labelling-index in bladder cancer was remarkably higher than in normal bladder epithelium and in benign papillomas. There was a good correlation between histologic grading of malignancy and increase of the mean 3H-Thymidine labelling index. Addition of 5-Fluorodesoxyuridine to the incubation medium had no influence on the labelling of the incubated tissue.

The extent to which new information on colorectal cancer (C.C.) is being transmitted to medical undergraduates was assessed in a repeated survey of new interns' knowledge of and attitudes on C.C. A probability sample of 214 graduates of the two Sydney medical schools was drawn as they commenced their internships in 1978. A comparable sample of 186 was drawn in 1980. Interns in each sub-sample completed a questionnaire of 54 pre-coded items of knowledge of C.C. epidemiology, early diagnosis, and followup, and on their attitudes towards C.C. On most topics knowledge was not well developed, and 1980 graduates were not significantly better informed than those of 1978. Almost three-quarters of 1980 interns intend using faecal occult blood testing.

Whether the common human cancers possess tumour-specific antigens which produce an immune response in the host still remains the crucial question in tumour immunology. Recent reports and discussions at the Third International Congress of Immunology suggested that much of the previously published work was not valid. The reasons for this have been analysed by comparing the well-established experimental data with human cancer. It is suggested that much of the current confusion in tumour immunology stems from the poor models used to study human cancer and the inadequate assessment of in vitro assays which have measured the host response. The present methods require refinement, or new techniques need to be developed, before this question can be answered. The field has been well researched, and there do not appear to be any recent major advances in tumour immunology which can be applied in clinical practice.

A prospective study of 40 patients with cancer of the oesophage or cardia considered for surgery showed that 27 had no evidence of protein or calorie deficiency, while 13 had undernutrition. The 18 patients who benefited from ablative surgery belonged to the group with normal nutritional status; none of the undernourished patients could have benefited from resection. Only controlled studies will show whether or not correcting undernutrition is advantageous, but the number of people with both resectable tumour and corrigible undernutrition is probably very small.

A survey of over 250 consecutive parotidectomies performed at The Mason Clinic indicates a surprising incidence of metastatic malignant disease to the parotid gland. Four percent of all parotidectomies performed indicated a metastatic focus of malignancy from an ;unsuspected primary outside areas of the head and neck usually implicated in parotid disease. Twenty-five percent of all cancer discovered at parotidectomy was metastatic malignant disease to the parotid gland. The most common site was the lung. A high index of suspicion, evaluation of pathologic specimens by electron microscopy, and detailed metastatic work-up for patients with malignant disease in parotid lymph nodes are recommendations which should be considered by the surgeon performing parotid procedures.

Local free cell (eosinophils and macrophages) and extracellular (acid mucopolysaccharides) reactions were studied by histochemical techniques in 72 primary lung cancers (Group A), 17 pulmonary metastases and 5 lung tumours of unknown origin (Group B). Strong cellular reactions were more frequent in Group A than in Group B, whereas extracellular reactions were more frequent in Group B than in Group A. In both groups the degrees of eosinophilic infiltration and accumulation of acid mucopolysaccharides tended to be negatively correlated. In contrast, macrophages and eosinophils showed a positive correlation in the primary cancers, but none in the other tumours. The extracellular material had physical and chemical properties (solubility, affinity to stains and degradability by enzymes) of the sulphated acid mucopolysaccharides. From its distribution it appeared to be derived from fibroblasts and/or tumour cells.

In presenting five new cases of renal adenomas the authors describe the general nature of these rare tumours. If the small adenoma corresponding to microscopic forms is frequent, the large adenoma is rare and always solitary. Haematuria and pain are the most common signs. The classic criterion of non-malignancy in a solid tumour measuring less than 3 cm has no diagnostic value. From a review of the lieterature on the subject, the authors demonstrate the angio-architecture corresponding to the different types of adenoma: the tubular adenoma which is hypervascularised and well circumscribed; the papillary adenoma, often cortical, hypovascular or avascular with bordering arcade; the alveolar adenoma; the mixed forms. They stress the difficulty of radiological diagnosis and the frequency of transitional forms with cancer. The place of angiography is in the pre-operative diagnostic assessment and in defining localised anomalies in angio-architecture which can guide histological examination in the search for neoplastic foci.
