TITLE: Behavior of cancer at the anterior commissure of the larynx.

The present research is concerned with the behavior of the vestibular cancer with respect to the portion of the anterior commissure of the larynx which the authors have named the "x-space." The research is based on the analysis of semiseriated vertical sections showing cancers of the vestibule in stages T1 and T2 with histological gradings 1 and 2. The specimens show that, at least in the early stages of the neoplastic spread, the fibrous structures of the x-space act as a fairly effective barrier to deep diffusion of the tumor. This fact enables the surgeon to estimate macroscopically the lower borders of the tumor growth on the basis of the superficial spread of the cancer in the zone of the x-space.

TITLE: Breast cancer with osseous metastasis and herniated lumbar disc. A cautionary tale.

A 39-year-old white woman with breast cancer, metastatic to her skeleton, developed low back and left lower extremity pain and lower extremity weakness. A bone scan evidenced increased radioisotope activity in her lumbar spine and a computed tomography (CT) scan showed a lesion of the L4 vertebra. Because of myelographic findings of a extradural defect at the L4-5 disc space and the possibility of a herniated disc causing the patient's pain and neurologic deficit in her lower extremities, the patient underwent surgery and a large herniated L4-5 disc was removed. As a consequence, the patient experienced relief of the lower extremity pain and return of strength in her lower extremities. She died a considerable time later from refractory hypercalcemia.

The incidence of deep venous thrombosis of the legs (DVT) was studied in 119 critically ill patients by 125I-labeled fibrinogen scanning; the efficacy of low-dose heparin prophylaxis was assessed in a randomized, double-blind study. DVT occurred in 29% of control patients and in 13% of patients receiving heparin 5000 U subcutaneously twice daily. DVT was found mainly in men and was associated with circulatory impairment, respiratory failure and recent vascular or cancer surgery. In a comparison study of medical patients, DVT occurred in 10% untreated and 2% treated. In conclusion, the critically ill are at high risk of venous thromboembolism and low-dose prophylaxis is warranted in those who have no hemostatic impairment.

Between 1982 and 1986 a total of 137 patients were gastrectomized for cancer of the stomach. The esophago-jejunal anastomosis was always carried out as an end-to-side anastomosis applying the conventional two layer technique. Only in 1 patient (0.7%) an insufficiency of this anastomosis was observed; 9 reoperations (6.6%) were necessary and 3 patients (2.2%) died in the postoperative course. These results confirm that with a precise, standardised conventional suture technique the esophageal anastomosis can be safely performed today.

The psychiatric morbidity associated with mastectomy and brest cancer was substantially reduced by a specialist nurse who counselled women before and after surgery and monitored their progress. A controlled study was, therefore, carried out to determine what this cost. National Health Service costs were almost wholly covered by savings made because counselled subjects who developed psychiatric problems were recognised and treated much earlier than control patients. Counselled and control subjects suffered considerable financial losses, but these were offset in the counselled group by their relatives' earlier return to work. Such counselling schemes are necessary and effective and may be implemented at little extra cost.

TITLE: Randomised clinical trial of levonantradol and chlorpromazine in the prevention of radiotherapy-induced vomiting.

Levonantradol is a cannabis derivative. Cannabinoid anti-emetics are being assessed in cancer chemotherapy but have been little used in radiotherapy to date. A pilot study and randomised trial compared the anti-emetic effect of a standard drug (Chlorpromazine 26 mg) with Levonantradol at two doses (0.5 and 0.75 mg) in patients receiving palliative single fraction radiotherapy to sites likely to cause nausea and vomiting. Most patients were out-patients. Both drugs were well tolerated. The frequency of vomiting was similar in all three groups in both the pilot study and randomised trial.

As reports about HCG positivity in serum or tissue of patients with breast cancer show divergent results, the present investigation was undertaken to study the frequency of positive staining for beta-HCG in breast cancer tissue and to correlate the staining pattern with the histology of the tumor and stage of disease. With the immunoperoxidase technique we studied 129 cases of breast carcinomas. We obtained a positive staining for beta-HCG in 12%. However, in most of the tumors only very few cells contained beta-HCG. Morphologically the positive cells could not be distinguished from beta-HCG-negative cells. We conclude from our data that until now the beta-HCG staining provides no immediate useful information for the management of patients.

Twenty-six evaluable patients with advanced nonsquamous cell cervical cancer were treated with aminothiadiazole at a dosage of 125 mg/m2 weekly. Sixteen had received prior chemotherapy. Two patients had complete responses neither had received prior chemotherapy; nine had stable disease; 15 had increasing disease. There was a single life-threatening toxic episode. Aminothiadiazole used in this dosage and schedule has minimal activity in previously treated patients with nonsquamous cell carcinoma.

Fifteen patients with locally advanced refractory breast cancer have been treated with recombinant leucocyte interferon ( rIFN -alpha A) for up to 12 weeks. Toxicity was considerable with the initial dosage schedule employed but became acceptable after reducing the starting dose by 50%. Minor side effects occurred in all patients and major CNS toxicity in six. Nine patients showed some evidence of tumour regression at 4 weeks. Only two of these were still responding at 12 weeks. Response was unrelated to the length of previous history, oestrogen receptor status or previous responsiveness to cytotoxic or hormone therapy.

In 53 patients suffering from bladder cancer the acelylization phenotype was examined. The frequency of slow acetylizers (70%) was significantly higher than in the control group. The more frequently occurrence of slow acetylizers in the group with professional pollutant exposition is an important reference to a higher bladder cancer morbidity in the case of carcinogen contact. A screening of the acetylization phenotype in professions with carcinogen contact may be important in the prophylaxis of bladder cancer.

A 56-year-old man was hospitalized with the complaint of upper abdominal discomfort. A gastrointestinal series revealed a filling defect in the body of the stomach. Under the diagnosis of gastric cancer, total gastrectomy was performed. Borrmann III type cancer measuring 5 X 4 cm, extending to the antrum and the upper portion of the body of the stomach, was identified in the body. Histologically, most of the tumor consisted of squamous cell carcinoma. Adenocarcinoma and a small nest of squamous metaplasia were observed at the peripheral wall of this tumor. The pathological diagnosis was adenosquamous carcinoma. In some regional lymph nodes, metastasis of adenocarcinoma was observed.

A systematic approach to identifying the cause of pain and rational use of drug therapy are keys to providing pain relief to cancer patients. Aspirin, acetaminophen and nonsteroidal anti-inflammatory drugs are effective for mild to moderate pain, and they enhance the effectiveness of weak oral narcotics, such as codeine. For severe pain, morphine is the drug of choice. A variety of adjuvant drugs can be used to enhance the effect of narcotics and to treat specific side effects of the disease or of therapy. For the terminally ill patient, a peaceful death with dignity should always be possible.

A case-control study of maxillary sinus cancer was performed in Hokkaido with 116 cases and 232 controls matched for sex, age (within 5 years), and residence (same health center region). Preliminary univariate analyses showed that a history of chronic sinusitis [relative risk (RR) = 3.1]; nasal polyps (RR = 5.7); an occupational history of being a carpenter, joiner, furniture maker, or other woodworker (RR = 2.9); and a history of current or past cigarette smoking (RR = 3.3) were statistically significant risk factors for men. No single item was a significant risk factor for women.

TITLE: [Local use of metronidazole in dimethyl sulfoxide in the radiation therapy of cervical cancer]

A method of the local use of metronidazole dissolved in dimethylsulfoxide (DMSO) for cervix uteri cancer patients was worked out. Applications of 1-2 g of metronidazole were well tolerated by the patients. Metronidazole concentrations in cervical tumors were high (about 1000 micrograms/g), in the blood they did not exceed 16 micrograms/ml. Experiments showed that metronidazole in DMSO diffused in the tissue, its concentrations at a distance of 2-3 cm from the surface were 180-260 micrograms/g. The local use of metronidazole in DMSO caused an increase in the rate of tumor radiation regression.
