TITLE: Presence of the rare D-variant heat-stable, placental-type alkaline phosphatase in normal human testis.

In 11 adult testes studied, about 0.3 to 4.6% of the total alkaline phosphatase activity was heat stable and L-phenylalanine sensitive but L-homoarginine insensitive. The testicular heat-stable enzyme was more susceptible to inhibition by L-leucine and ethylenediaminetetraacetate than were the normal placental and intestinal enzymes. By antibody-directed enzyme inhibition test, the testicular heat-stable enzyme cross-reacted completely with normal placental enzyme but clearly distinguished itself from a heat-stable component of normal intestinal enzyme. Thus, placental alkaline phosphatase D-variant is synthesized in testis, indicating that the gene for elaborating this placental protein is probably already active in the testicular cells. The high incidence of this protein in cancers of testis and ovary is probably due to its increased production by gonadal genes present in the genome of these particular tumors.

TITLE: Mortality of male members of the Danish Semi-skilled Unskilled Workers' Union in 1973.

The purpose of this cross-sectional study is to evaluate contemporary age and cause specific mortalities of a large group of employed men. The population is the 228,612 male members, aged 20-64, of Denmark's largest trade union, the Semi-skilled and Unskilled Workers' Union (SID). A Standard Mortality Ratio (SMR) of 93.9 compared to all men in Denmark is found. However, the distribution of mortality by age does not resemble that found in many other studies, and is usually said to result from the 'healthy worker effect'. Excess mortality is found in the younger age groups, while the opposite is the case in the older groups, mainly because of a deficit of deaths from chronic illness. Notably higher age and cause specific SMRs are found for violent and unnatural deaths, and higher SMRs for cancer of the lungs and of lymphatic and haemopoietic tissues. Regional variations are found, but no correlation found between mortality and size of union 'local'. However, cancer mortality is higher among members of specialized locals, which are situated in larger urban area. The excess mortality of members of the largest locals in the Copenhagen area is due to deaths by cancer and by violent and unnatural causes. Findings and problems will be examined further in an ongoing cohort study and in other specialized studies under preparation.

The presence or absence of detectable metastasis is valuable knowledge both in planning treatment and in predicting prognosis. Knowledge of the metastatic behavior patterns of such tumors, and of the diagnostic means of detecting metastasis, is essential. The available knowledge on this problem is comprehensively reviewed to present, in a single source, the requisite knowledge for any physician treating head and neck cancer. Metastatic spread is reviewed according to incidence, detection methods, anatomical sites, and correlation with characteristics of the primary tumor. Finally, methods of detecting metastasis are reviewed and a practical approach to patient evaluation recommended.

TITLE: Adrenal insufficiency from metastatic colonic carcinoma masquerading as isolated aldosterone deficiency. Report of a case and review of the literature.

A case of adrenal insufficiency secondary to metastatic colonic carcinoma is reported. Because the patient was taking prednisone for an unrelated condition, his initial presentation mimicked isolated aldosterone deficiency with an elevated serum potassium of 8.7 mmol/l. Cortisol deficiency was documented by ACTH testing. The diagnosis of metastatic disease causing adrenal insufficiency was suspected after a computed body scan of the abdomen revealed marked bilateral adrenal enlargement. Adrenal insufficiency secondary to metastatic cancer is reviewed.

TITLE: Clinical conditions mistaken for metastatic cancer to the choroid.

Thirteen patients who were referred to the Oncology Unit with the diagnosis of metastatic cancer to the choroid were studied and found to have other conditions. All of the patients were female. Nine of the 13 had a history of systemic cancer of which 6 were breast cancer. Five patients were currently being treated with chemotherapeutic agents for their systemic cancer. Three of the 13 patients had bilateral fundus lesions. The most common diagnostic entities confused with metastatic disease were either inflammatory or hemorrhagic lesions of the retina. The preceding history of cancer in the patient with a puzzling fundus lesion may predispose the referring ophthalmologist to suggest a diagnosis of metastatic cancer to the choroid.

Using a computerized mathematical model the authors found that either a small [5%] or large [50%] hormone-dependent reduction in tumour cell population at the beginning of adjuvant chemotherapy for breast cancer greatly increases the probability fo achieving a cure. The fact that even a minor response to hormonal therapy early in the treatment of the tumor is effective underlines the importance of using estrogen receptors to to select potential responders in whom a cure is at least a strong theoretical possibility.

Results of epidemiological investigations point out that environmental factors have an influence on the development of at least 80% of cancer diseases. The cancer mortality rates in Austria are analyzed with regard to this point. Austrian mortality trends of the most common malignancies are similar to those of other western industrialized countries. The relationship between cancer risk and the size of the Austrian communities is shown. As opposed to other common cancer sites, cancer of the stomach is most common in small communities with mainly an agricultural population. Etiological hypotheses about the origin of stomach cancer are discussed with regard to its epidemiological findings as well as the dietary habits of the population.

Some recent studies on the psychological trauma endured by patients with breast disease are summarized and a number of conclusions are drawn. There appear to be a number of stages through which a patient with a breast lump passes psychologically. Breast cancer attitudes in a community are dependent upon accumulated experience and cannot be changed overnight. Delay in presentation results from unconscious factors which may not be amenable to rational public health appeals. It is possible to identity a group of patients high at risk in breast surgery, although human resilience is such that the outcome if often satisfactory. There is a great need for controlled prospective studies in many areas of breast surgery rehabilitation. Some personal observations are included to stimulate discussion and interest in the area.

We determined the occurrence of palpable thyroid abnormalities in 214 persons with verified history of head and neck irradiation and in 243 concurrently examined, nonirradiated persons (control subjects). Examinations were performed by two physicians, each unaware of the medical history of the examinees and of the findings of the other examiner. Thyroid nodules, either single or multiple, occurred in 6.1% of irradiated persons and in 4.5% of control subjects (95% confidence interval of the difference, -2.6% to +5.7%). Thyroid enlargement without nodules occurred in 5.6% of irradiated persons and in 3.7% of control subjects (95% confidence interval of the difference, -2.0% to +5.8%). Thyroid cancer was detected in one irradiated and in two nonirradiated persons. Our controlled study did not find an excess number of thyroid abnormalities in previously irradiated subjects.

The growth of transitional epithelial cells with different growth media and growth supports was examined. Sephadex G-10, Bio-Gel P-20, Bio-Glas-1000, DEAE-Sephadex A-50, DEAE-cellulose, CM-Sephadex C-50, acid-soluble collagen, and immobilized collagen fibers were used to enhance plating efficiency. Acid-soluble collagen layers optimally increased the plating efficiency of primary cultures of bladder carcinoma. Media alterations with serial combinations of fetal calf, newborn calf, calf, bovine, and bull serum with minimum essential medium, Roswell Park Memorial Institute Tissue Culture Medium 1640, Connaught Medical Research Laboratories Medium 1066, Medium 199, Grand Island Biological, National Cancer Tissue Culture 135, 1415, McCoy's 5A, and National Cancer Institute medium were established. No promotion of cell division was noted with any one of these basic medium formulations.

An in-vitro double labelling technique with 3H- AND 14C-Thymidine was applied to biopsy specimens of the human urinary bladder mucosa in order to determine proliferation kinetics of normal bladder epithelium as well as its benign and malignant alterations. Application of above mentioned technique led to evaluable autoradiographs. The mean 3H-Thymidine labelling-index in bladder cancer was remarkably higher than in normal bladder epithelium and in benign papillomas. There was a good correlation between histologic grading of malignancy and increase of the mean 3H-Thymidine labelling index. Addition of 5-Fluorodesoxyuridine to the incubation medium had no influence on the labelling of the incubated tissue.

Among 16,394 necropsies during the 12-year period, 1958-1969 (representing about 60% of all persons who died in Malmö during this period, 747 cases of bronchial cancer were found in 744 individuals. There were considerable differences in frequency of different histologic types with regard to sex. Two main classes have therefore been distinguished: Group I, comprising squamous epithelial cancer and anaplastic cancer (including oat-cell cancer), which was predominant among men; and Group II, comprising adenocarcinoma, bronchiolar carcinoma, carcinoid and bronchial gland tumour, which was of more or less equal frequency in both sexes. Compared with other malignant tumours, bronchial carcinoma had a strong tendency to metastasise to the adrenals, brain, kidneys and skeleton. Bronchial cancer did not differ from other tumours with regard to lymphogenous spread but had a stronger tendency to spread via the blood stream. Squamous epithelial cancer showed the strongest tendency to remain confined to the thoracic cavity.

A preliminary report describes the use of both a single and a combined chemotherapeutic drug regime before radiotherapy for locally advanced and radioresistant cancers. Some tumours have been rendered operable with safety by this sequence. This method is suggested to replace the current practice of following surgery with radiotherapy and finally chemotherapy. In our experience, chemotherapy before radiotherapy is assisting in the effective control of locally advanced and hitherto radioresistant cancers of many types, and in converting inoperable cancers into cancers that can be resected sometimes with safety. Also, when chemotherapeutic drugs are given before radiotherapy, damage to normal adjacent tissue is less than that seen after radiotherapy alone.

The anti-cancer action of retinyl acetate (Vitamin-A acetate, VAA), chosen as a representative retinoid substance, is attributed to its power to exercise immunopotentiation, though other possibilities are considered. The reasons for forming this opinion were: (1) chronic administration of VAA brought about enlargement of the thymus and peripheral lymph nodes; (2) the administration of VAA curtailed the life of skin allografts though (3) its action could be reversed by the concomitant administration of immunosuppressive agents.

TITLE: Microinvasive carcinoma of the uterine cervix: A long-term followup study of eighty cases.

One hundred and eleven patients with microinvasive carcinoma of the uterine cervix were studied over a 21-year period. Thirty-five cases of carcinoma in situ with questionable stromal invasion were excluded. Cases with pathologic examination of less than a cervical cone or the entire cervix were not accepted. The sole pathologic criterion for inclusion was unequivocal invasion to a depth of no more than 5.0 mm. Ninety-one patients were followed for 5 years or until death, and 80 patients for 10 years or until death. One patient was lost to follow-up at 5.5 years. The two deaths officially attributed to cervix cancer prior to 10 years were signed out by nonphysician assistant coroners. Available clinical evidence indicates that at least one of these patients, and probably both, did not die of cervix cancer. From these data, simple hysterectomy would seem to be the maximal treatment indicated. Since the prognosis of microinvasive carcinoma is similar to that of carcinoma in situ, it is suggested that such cases not be included when considering the end results of Stage I cervix cancer.
