Twenty evaluable patients with advanced cervical cancer refractory to one prior chemotherapy regimen were treated with a 5-day schedule of fludarabine phosphate. No responses were noted. The major toxicities were anemia and leukopenia. Based on this study, fludarabine phosphate does not appear to be an active agent for patients with chemotherapy refractory cervical cancer.

TITLE: Guillain-Barre syndrome following autologous bone marrow transplantation.

The pathogenesis of the Guillain-Barre syndrome (GBS) is felt to involve alterations in cellular and humoral immune responses. We report the case of a 44-year-old female with stage IV breast cancer who developed GBS 2 days after receiving high dose chemotherapy with bone marrow and peripheral progenitor cell support, and granulocyte-macrophage colony stimulating factor (GM-CSF). Her clinical status stabilized with plasmapheresis. This report lends further support to hypotheses regarding cellular immune alterations in the pathogenesis of GBS.

The present research is concerned with the behavior of the vestibular cancer with respect to the portion of the anterior commissure of the larynx which the authors have named the "x-space." The research is based on the analysis of semiseriated vertical sections showing cancers of the vestibule in stages T1 and T2 with histological gradings 1 and 2. The specimens show that, at least in the early stages of the neoplastic spread, the fibrous structures of the x-space act as a fairly effective barrier to deep diffusion of the tumor. This fact enables the surgeon to estimate macroscopically the lower borders of the tumor growth on the basis of the superficial spread of the cancer in the zone of the x-space.

TITLE: Breast cancer with osseous metastasis and herniated lumbar disc. A cautionary tale.

A 39-year-old white woman with breast cancer, metastatic to her skeleton, developed low back and left lower extremity pain and lower extremity weakness. A bone scan evidenced increased radioisotope activity in her lumbar spine and a computed tomography (CT) scan showed a lesion of the L4 vertebra. Because of myelographic findings of a extradural defect at the L4-5 disc space and the possibility of a herniated disc causing the patient's pain and neurologic deficit in her lower extremities, the patient underwent surgery and a large herniated L4-5 disc was removed. As a consequence, the patient experienced relief of the lower extremity pain and return of strength in her lower extremities. She died a considerable time later from refractory hypercalcemia.

TITLE: Presence of the rare D-variant heat-stable, placental-type alkaline phosphatase in normal human testis.

In 11 adult testes studied, about 0.3 to 4.6% of the total alkaline phosphatase activity was heat stable and L-phenylalanine sensitive but L-homoarginine insensitive. The testicular heat-stable enzyme was more susceptible to inhibition by L-leucine and ethylenediaminetetraacetate than were the normal placental and intestinal enzymes. By antibody-directed enzyme inhibition test, the testicular heat-stable enzyme cross-reacted completely with normal placental enzyme but clearly distinguished itself from a heat-stable component of normal intestinal enzyme. Thus, placental alkaline phosphatase D-variant is synthesized in testis, indicating that the gene for elaborating this placental protein is probably already active in the testicular cells. The high incidence of this protein in cancers of testis and ovary is probably due to its increased production by gonadal genes present in the genome of these particular tumors.

TITLE: [Effect of chemotherapy on survival in advanced breast cancer]

Survival time of 119 consecutive patients with metastasizing carcinoma of the breast who had been treated with various chemotherapeutic schedules after radical operation was determined retrospectively. 57 women received chemotherapy and hormones, as well as radiotherapy, while 62 had radiotherapy plus hormone treatment. The mean prolongation of life in the former group over that of the latter was 12 months (p less than 0.01). Although chemotherapy delayed death, the dynamics of cancer were not fundamentally affected. The chemotherapy-induced prolongation (2.5 months to five years) was independent of any risk, primary location of the metastases or age.

The purpose of this cross-sectional study is to evaluate contemporary age and cause specific mortalities of a large group of employed men. The population is the 228,612 male members, aged 20-64, of Denmark's largest trade union, the Semi-skilled and Unskilled Workers' Union (SID). A Standard Mortality Ratio (SMR) of 93.9 compared to all men in Denmark is found. However, the distribution of mortality by age does not resemble that found in many other studies, and is usually said to result from the 'healthy worker effect'. Excess mortality is found in the younger age groups, while the opposite is the case in the older groups, mainly because of a deficit of deaths from chronic illness. Notably higher age and cause specific SMRs are found for violent and unnatural deaths, and higher SMRs for cancer of the lungs and of lymphatic and haemopoietic tissues. Regional variations are found, but no correlation found between mortality and size of union 'local'. However, cancer mortality is higher among members of specialized locals, which are situated in larger urban area. The excess mortality of members of the largest locals in the Copenhagen area is due to deaths by cancer and by violent and unnatural causes. Findings and problems will be examined further in an ongoing cohort study and in other specialized studies under preparation.

The incidence of deep venous thrombosis of the legs (DVT) was studied in 119 critically ill patients by 125I-labeled fibrinogen scanning; the efficacy of low-dose heparin prophylaxis was assessed in a randomized, double-blind study. DVT occurred in 29% of control patients and in 13% of patients receiving heparin 5000 U subcutaneously twice daily. DVT was found mainly in men and was associated with circulatory impairment, respiratory failure and recent vascular or cancer surgery. In a comparison study of medical patients, DVT occurred in 10% untreated and 2% treated. In conclusion, the critically ill are at high risk of venous thromboembolism and low-dose prophylaxis is warranted in those who have no hemostatic impairment.

Anal condylomas result from papillomavirus infection, the most common sexually transmitted disease. The principal risk is the development of cancer of the anal canal. The risk of contamination is high, even after a single sexual contact. No specific antiviral treatment is available and no consensus has been reached on the appropriate treatment of anal condylomas. Despite the development of various methods (interferon, imiquimod), electrocoagulation remains the treatment of choice. Finally, regular follow-up and treatment of the partner(s) are essential.

We reviewed our experience performing mediastinoscopy as an outpatient procedure. From January 1, 1987, to December 31, 1992, 1,015 of 1,062 cervical mediastinoscopies were performed on an outpatient basis. There were no operative deaths. Complications developed in 14 patients, 10 of whom required hospitalization. Two additional patients were hospitalized for social reasons. Supraventricular arrhythmia was the most frequent complication requiring admission to the hospital. This article documents the ability to perform mediastinoscopy safely as an outpatient procedure and discusses why this is our preferred approach in managing patients with lung cancer.

Post-replicative mismatch repair in humans utilises the hMSH2, hMSH6, hMSH3, hMLH1 and hPMS2 genes and possibly the newly identified hMLH3 gene. Recently, a link has been established between hMSH6 mutations and 'atypical' hereditary non-polyposis colon cancer (HNPCC) with an increased incidence of endometrial cancers. To satisfy the need for a diagnostic test capable of differentiating between pathogenic mutations and polymorphisms, several functional assays that fulfil these criteria have been described. These should allow for better diagnosis of HNPCC.

Between 1982 and 1986 a total of 137 patients were gastrectomized for cancer of the stomach. The esophago-jejunal anastomosis was always carried out as an end-to-side anastomosis applying the conventional two layer technique. Only in 1 patient (0.7%) an insufficiency of this anastomosis was observed; 9 reoperations (6.6%) were necessary and 3 patients (2.2%) died in the postoperative course. These results confirm that with a precise, standardised conventional suture technique the esophageal anastomosis can be safely performed today.

Metastatic spread of head and neck cancer signifies a grave change in prognosis for the patient. The presence or absence of detectable metastasis is valuable knowledge both in planning treatment and in predicting prognosis. Knowledge of the metastatic behavior patterns of such tumors, and of the diagnostic means of detecting metastasis, is essential. The available knowledge on this problem is comprehensively reviewed to present, in a single source, the requisite knowledge for any physician treating head and neck cancer. Metastatic spread is reviewed according to incidence, detection methods, anatomical sites, and correlation with characteristics of the primary tumor. Finally, methods of detecting metastasis are reviewed and a practical approach to patient evaluation recommended.
