Malignancy is the second most common cause of ascites. Tissue diagnosis is often difficult because the cytology of ascitic fluid is positive in only 57% of cases. Peritoneovenous shunting is often used as palliation in such patients and has proven superior to nonoperative management for some patients. We present three cases of malignant ascites with negative cytologies managed by using laparoscopic biopsies to confirm intraperitoneal cancer and assist in the placement of a peritoneovenous shunt. Results suggest that exploratory laparoscopy and shunt placement is a valuable procedure in these patients with a limited life expectancy and is preferable to open laparotomy.

OBJECTIVE: To evaluate the effect of preoperative chemotherapy on the prognosis of stage II or III patients with esophageal carcinoma. METHODS: From February 1991 to June 1994, 50 patients with stage II, III esophageal carcinoma were treated with preoperative combination (CDDP) chemotherapy and compared with 50 patients received operation alone. RESULTS: The 5-year survival rate of the operation group and preoperative chemotherapy group was 32% and 46%, respectively. The difference was statistically significant. CONCLUSION: Preoperative chemotherapy improves survival of surgically treated esophageal cancer patients.

TITLE: 15 years after Chernobyl: new evidence of thyroid cancer.

The Chernobyl nuclear power plant accident happened on April 26, 1986. We investigated the cause of the striking increase in frequency of thyroid cancer in children who lived within a 150 km radius of Chernobyl and who were born before and after the accident. No thyroid cancer was seen in 9472 children born in 1987-89, whereas one and 31 thyroid cancers were recorded in 2409 children born April 27, 1986, to Dec 31, 1986, and 9720 born Jan 1, 1983, to April 26, 1986, respectively. Short-lived radioactive fallout caused by the Chernobyl accident probably induced thyroid cancer in children living near Chernobyl.

TITLE: [A case of lung cancer with hypercalcemia and renal failure responsive to pamidronate]

A 46-year-old man was diagnosed as Stage IV lung cancer with neck lymph node and bilateral adrenal metastases. He was treated with seven courses of combined chemotherapy (CDDP + etoposide), until he showed extreme hypercalcemia and acute renal failure. The bisphosphonate, pamidronate with elcatonin and prednisolone dramatically lowered his serum calcium level and normalized his renal function. This is a representative case of humoral hypercalcemia of malignancy whose serum calcium could be safely and effectively controlled with pamidronate.

The c-Jun N-terminal kinase (JNK) group of mitogen-activated protein kinases (MAPKs) is activated in mammalian cells by environmental stress, pro-inflammatory cytokines, and mitogenic stimuli. Biochemical and genetic studies demonstrate that JNK regulates the activities of many transcription factors, and that the JNK pathway is required for the regulation of inflammatory responses, cell proliferation, and apoptosis. The involvement of JNK in apoptotic cell death is particularly intriguing, and has been actively studied in recent years. An improved understanding of JNK-mediated apoptotic signaling may provide novel strategies in prevention and treatment of cancers.

While collecting information for a consumer pamphlet to assist cancer patients in navigating through the medical system, the National Cancer Policy Board discovered the data that were available were critically insufficient. In response, the turned their attention to analyzing our ability to assess quality of cancer care. Their findings along with ten recommendations were published in April 1999. In particular, their conclusions concerning data collection and volume outcomes surprised many in the oncology community. These comments along with five of their recommendations--high-volume settings, standard guidelines, the elements of quality care, measuring and monitoring quality care, and end-of-life care--are presented in this panel discussion.

Drug resistance is a major problem in cancer chemotherapy. P-glycoprotein plays a major role in multidrug resistance in cancer cells. P-glycoprotein is expressed in some normal tissues and has physiological functions. These include protecting the brain against toxic substances at the blood-brain barrier site, excreting toxic substances from the liver, kidney, and gastrointestinal tracts, and transporting steroidal hormones in the adrenal grand. Once expressed in cancer cells. P-glycoprotein effluxes a variety of anticancer drugs, such as doxorubicin, vinca alkaloids, etoposide and taxol, and thereby allows cancer cells to show resistance to these drugs.

In the 50 years since Leksell developed the concepts and initial hardware for modern brain radiosurgery, the treatment has progressed to the point where it is used commonly for arteriovenous malformations, benign masses, and metastases. Radiosurgery offers patients an effective treatment of life-threatening lesions with a reasonably low risk for discomfort and injury. In the 1990s, the procedure was used widely as primary and adjuvant treatment. The difficulty of defining the boundaries of primary brain cancers makes determining treatment targets problematic. Better imaging and computing offer a bright future for the technology.

The erb type 1 tyrosine kinase receptors are important for mediating proliferation and differentiation, and aberrant activation may contribute to tumour development and progression. They comprise epidermal growth factor receptor (EGFR), c-erbB-2, c-erbB-3, and c-erbB-4. There are overlaps and differences in the cellular distribution of the four receptors, which is of significance since they can form heterodimers that give differing responses to different ligands. The different distributions could be of particular importance in cancer if therapeutic modalities are identified to inhibit or stimulate the different ligands.
