Acrometastases are rare and often misdiagnosed or overlooked. When it involves the feet, it generally attacks the larger bones containing the higher amounts of red marrow. The patient may or may not have a known history of cancer, which makes diagnosis much more difficult. The symptomatology is generally vague and can mimic other conditions, such as osteomyelitis, gouty rheumatoid arthritis, Reiter's syndrome, Paget's disease, osteochondral lesions, and ligamentous sprains. Therefore, the physician must consider metastatic disease in the differential diagnosis. Once the diagnosis is made, the prognosis is poor and treatment is limited to pain relief and maintaining function.

While TRUS is not of value as a screening tool, it is of considerable value in the early detection of cancer of the prostate in men with abnormal DRE and/or PSA. TRUS enables calculation of PSA density to identify men at low risk, thus eliminating many unnecessary biopsies. This results in favorable impact on both cost and morbidity. When biopsy is necessary, TRUS greatly enhances accuracy. Its role in staging is of less importance but can be useful when positive, and it adds little to the cost or complexity of the examination.

About 10% of patients with newly diagnosed colorectal cancer will have locally advanced disease without gross evidence of distant or discontiguous intraabdominal metastases. Long-term cure or disease-free survival frequently follows extended resection for a locally advanced primary colorectal carcinoma. The use of adjuvant combined chemo-radiation therapy is still being evaluated. While complete resection of recurrent disease sometimes provides the best long-term survival when compared with other treatment modalities, few patients are suitable candidates for curative resection. In our experience, major extended resection for recurrent pelvic disease is largely palliative and is rarely associated with long-term survival.

Laparoscopic pelvic lymphadenectomy for gynecological malignancy involves mobilizing and exteriorizing the lymph nodes. We removed the nodes by passing a 4- to 5-cm-diameter tube through the vagina to the abdomen following the hysterectomy. The nodes were passed through the tube rather than pulled through the abdominal ports. The tube is easy to insert and is a valuable conduit for exteriorizing tissue during laparoscopic pelvic surgery for gynecological cancer.

The consequences of anorexia include muscle wasting, weakness, depression, increased susceptibility to disease complications, and decreased immunocompetence. In the older population, the major causes of anorexia are pulmonary and cardiac diseases, cancer, dementia, alcoholism, depression, and medications. The diagnosis is based on the physical exam, a history of weight loss, and a review of dietary intake. Treatment strategies include managing GI disturbances, encouraging physical activity and socialization, reducing fatigue and food aversions, and adding high-calorie supplements to the diet.

In the present work the role of histamine H1-receptors in the modulation of neurotransmitter receptors activity in human lung parenchyma in cancer is reported. It appeared that the activation of histamine H1-receptors may lead to the reduction of beta-adrenoceptors and the enhancement of muscarinic cholinergic receptors in cancer lung parenchyma. The above findings suggest the possibility of the interaction between neural and inflammatory systems in human lung cancer.

From December 1987 to 1994, a total of 193 patients with carcinoma of the gastric cardia were surgically treated by partial esophagectomy, partial gastrectony and end-to-side esophagogastric anastomosis with fundoplication. When compared to those patients during the same time period similarly treated but with end-to-end esophagogastrostomy, the incidences of anastomotic leakage, anastomotic stricture and esophageal reflux were all significantly decreased (P < 0.01). It is a novel surgical treatment of choice for cancer of the gastric cardia.
