TITLE: Adriamycin, cyclophosphamide and etoposide (ACE) in the treatment of small cell lung cancer.

Thirty-five small cell lung cancer (SCLC) patients were treated with combination chemotherapy including adriamycin, cyclophosphamide, etoposide (ACE). Out of 32 evaluable patients there were 21.9% complete responses and 53.1% partial responses with an overall median survival of 37 weeks (50 weeks for patients with limited disease and 34 weeks for patients with extended disease). Toxicity was generally well tolerated. In conclusion the ACE regimen results in being active and safe in the treatment of SCLC.

TITLE: Treatment of carcinoma of the penis: the case for primary lymphadenectomy.

There were 65 patients with squamous cell carcinoma of the penis followed for a minimum of 5 years. Patients with stage I disease uniformly survived with local treatment only. Of the patients with stages II and III carcinoma of the penis 88 and 66 per cent, respectively, were free of disease if lymphadenectomy was performed shortly following treatment of the primary lesion, compared to only 38 and 0 per cent, respectively, if the primary lesion was treated locally and no lymphadenectomy was performed. These data support the concept that lymphadenectomy should be an integral part of the primary treatment in patients with stages II and III penile cancer.

All patients had an elevated bronchitis score over that of normal controls. The cause of this bronchitis is likely multifactorial and was not uniformly predicted by bronchial neutrophil counts, current smoking history, or PFT abnormality. All patients had, in addition to high bronchitis score, evidence of increased bronchial neutrophils and/or PFT abnormality or cancer. These findings lead to a change in management prior to heart transplantation.

Patients with thyroid cancer benefit from treatment with exogenous thyroid hormone for two reasons: it provides adequate levels of thyroid hormone to peripheral tissues, and it reduces the level of thyrotropin, which may be an important growth factor in patients with differentiated malignant neoplasms. The use of radioactive iodine for thyroid cancer is highly controversial. Its most appropriate applications are in follicular cancers, in older patients, and in distant functioning metastases. Its value in papillary cancer is questionable, particularly in young patients. There is a great need for effective basic and clinical research on the natural course of differentiated thyroid cancer and the effects of specific therapies.

Serum testosterone level (STL) was examined in 25 male patients with cardiac or gastric carcinoma before and after operation and 28 male patients with benign diseases as control. The preoperative STL of patients with carcinoma was lower than that of the control (P less than 0.001). Postoperatively STL of these patients rose to the level of control group. When cancer recurred, STL again dropped to low level. STL was lower when cancer had invaded beyond the serosa than that limited to the musculature. Therefore, STL may be a good parameter for diagnosis and prognosis in patients with cancer of the stomach and gastric cardia.

Twenty-nine patients with inoperable non-small cell lung cancer were treated as out-patients with vindesine and mitomycin C. Eight patients had a complete response, and nine a partial response. Response was generally associated with either stable or improving ECOG and symptom scores. The incidence of serious side-effects was low--only two patients' white cell count fell below 3000 cells/mm2, only one patient's platelet count fell below 100,000 cells/mm2, and two patients developed vomiting associated with treatment. The combination of vindesine and mitomycin C appears to be effective in the treatment of inoperable non-small cell lung cancer. Side effects were generally well tolerated and allowed out-patient treatment.

Estrogen receptor determination was performed on 120 breast cancer cytosols, using the dextran-coated charcoal method (DCC) and an enzyme immunoassay (EIA) to compare the efficiency of the two techniques. A strong correlation was noted between ER concentrations determined by DCC and EIA (P less than 0.001). The mean ER-EIA value was significantly higher than the mean ER-DCC value in premenopausal (P less than 0.001) as well in postmenopausal (P less than 0.001) patients.

Epidural spinal cord compression is common in patients with metastatic cancer. Back pain is usually the first symptom and may be present for months before neurologic abnormalities occur. A favorable outcome depends on early diagnosis and treatment. For the management of this problem, we propose an algorithm that begins with the treatment of patients who need emergency care and proceeds with an orderly approach to the evaluation of less urgent cases. The central elements include the criteria for myelography and the rational use of corticosteroids, radiation therapy, and surgery.

Comparison of the intrinsic thermosensitivity of malignant and normal homologous cells are important in evaluating the therapeutic role of hyperthermia in cancer. However, such comparisons had problems with establishing the origin of cells grown in culture. Using monoclonal antibodies we have shown that human kidney cancer lines, Caki-1 and Caki-2, and a normal human renal epithelium line, NHK-4, originated from proximal convoluted tubular epithelium. The observed order of thermosensitivity was Caki-2 greater than Caki-1 greater than NHK-4.

TITLE: [Therapeutic cranial irradiation in patients with metastatic brain tumors from pulmonary carcinoma]

Fifty-three patients with intracranial metastases from pulmonary carcinoma were treated with cranial irradiation between 1976 and 1983. There was improvement in neurologic symptoms in 80%, and the one and two-year survival rates for the entire group of patients were 21% and 4%. Those or patients without metastasis tumor in other organs were 38% and 9%, respectively. It was suggested that the total dose should be in excess of 40 Gy. Histologically, adenocarcinoma and epidermoid carcinoma showed better prognosis than other types of cancer.

Twenty-five women with primary carcinoma of the cervix were examined with magnetic resonance imaging (MRI). A number of different pulse sequences with T1 and T2 weighting were employed to determine optimum tissue contrast, and the potential for staging cervical cancer was assessed and compared with the clinical findings. MRI is seen to give a unique view of cervical cancer in particular in stage I and II disease where other imaging techniques have known limitations. In addition, stages III and IV were well seen with demonstration of tumour comparable with that of computerized axial tomography (CT).

An animal model of ureterosigmoidostomy was employed to elucidate the etiology of urinary intestinal anastomotic cancer. One hundred ninety-four rats were divided into five groups: vesicosigmoidostomy rats fed water without drug (controls); those fed ammonium chloride, or sodium bicarbonate, or ascorbic acid; and, finally, vesicosigmoid interposition rats. A significantly decreased incidence of cancer was evident only in the bowel interposition group (p less than 0.001). Ascorbic acid and altered urinary pH were not prophylactic. Possible mechanisms are discussed.

We report 28 cases of malignant disease in pregnant women. They were divided into 14 cases of intrapelvic tumors and 14 of extrapelvic tumors. The intrapelvic tumors were cervix cancer in nine and ovarian cancer in five, while the extrapelvic tumors were brain tumors in three, maxillary cancer in one, tongue cancer in one, pharyngeal cancer in one, breast cancer in one, gastric cancer in two, osteosarcoma in one Hodgkin's lymphoma in one, and leukemias in three. The prognoses of the patients with intrapelvic tumors were relatively good. But those of extrapelvic diseases were poor.
