TITLE: [Clinical significance of colorectal cancers arising after partial or total gastrectomy]

Among colorectal cancer patients operated at the 2nd Department of Surgery, Kyushu University Hospital between 1974 to 1980. 6 developed this cancer after partial or total gastrectomy for gastric cancer or ulcer. The interval between the gastric operation and colorectal surgery ranged from 15 months to 17 years. The colorectal cancers arose in the transverse colon (1) the sigmoid colon (2), there were 3 rectal cancers. Therefore, not only the occurrence of recurrent or primary cancer in the remnant stomach, but also the occurrence of colorectal cancer should be monitored carefully after partial or total gastrectomy for gastric cancer or ulcer.

A recent review of the curriculum at University of Kansas Medical Center-Kansas City revealed several deficiencies in oncology teaching in the medical school curriculum. Analysis of the surgery core course final examination showed a weaker response by students to surgical oncology items than nononcologic questions. A multidisciplinary cancer course and tumor conference were then added to the surgery core curriculum. Analysis of student performance since this addition reveals that the multimodal course improved the successful response rate to surgical oncology questions on the final examination. This effect has been sustained through nine consecutive student group rotations. These results suggest that surgical oncology teaching can be effectively improved by a multidisciplinary effort.

In a case-control study of 401 breast cancer cases and 625 age-matched controls, the possible etiologic role of hair dye usage was investigated. No association was found between hair dye use and breast cancer (odds ratio = 1.02; 95% confidence interval = 0.77-1.32) or any factors affecting dosage, frequency, duration, type, or color of hair dye. Important confounders of hair dye use included religion and smoking status. When stratified by 10-year age groups, there was no appreciable trend in risk status between case and referent populations. No dose-response relationship (frequency X duration X shade X type of dye used) was found, even after controlling for factors affecting hair dye use. A multiple logistic regression analysis supported the absence of this relationship.

We discussed the diagnostic value of urinary enzymes as non invasive test of renal integrity in medicine. Then we assayed the N-Acetyl-beta-D-glucosaminidase (NAG) excretion in urines of inpatients receiving cis-platinum, a useful anti-cancer but nephrotoxic agent fort the proximal tubule; we used the colorimetric instead of fluorimetric procedure, recently described in detail by Price who tested it on patients with a variety of renal diseases. The results demonstrated the sensibility and the specificity of colorimetric procedure, and therefore we concluded that the assay of urinary NAG activity may be used in the clinical chemistry and toxicology laboratory, because the test is simple to perform.

In this report, operated advanced gastric cancer was analysed based on 96 cases of carcinoma of the proper muscle layer (pm-cancer) experienced between 1961 and 1982 at the Third Department of Internal Medicine, Tohoku University School of Medicine, and its associated hospital. Patterns of macroscopic findings in pm-cancer were classified into three types: (1) early type, (2) early-like cancer type, and (3) Borrmann's variety type. The degree of lymph node metastasis ascended in types (1), (2) and (3) in that order, and the five-year survival rate descended in the same order.

Random bone marrow aspiration biopsies of iliac crests and bone scans/skeletal X-rays were evaluated in 163 new cases of carcinoma of the prostate in order to assess the incidence of bone metastases and to compare the sensitivity of these two methods. Bone marrow biopsy revealed cancer cells in 18.4% of all cases. The detection rate of skeletal metastases of bone scan/skeletal X-ray was 32.5%. In this series all patients with positive biopsies had also positive scans. The superior sensitivity of bone scans compared with bone marrow biopsies in detecting metastatic spread of prostatic cancer is shown.

TITLE: Space-time clustering of nasopharyngeal carcinoma in Greenland Eskimos.

Evidence of epidemicity of nasopharyngeal carcinoma (NPC) was sought in Greenland Inuits, who have a high incidence of this cancer, by examining the births of NPC cases for evidence of clustering in time and space. Births of cases were concentrated in autumn and winter. Fifty-four cases were analysed, and a two-fold excess of clustering within one year was observed, both within single districts and between adjacent districts. This excess was not significant at the 5% level; about 90 cases would have been required to confirm the observed effect at this level of significance. It is suggested that a search for space-time clustering of NPC cases in larger high-risk populations might prove more fruitful.

TITLE: Detection of antibodies against the cellular protein p53 in sera from patients with breast cancer.

Antibodies reacting with the host protein p53 were found in the sera of patients with primary or secondary carcinoma of the breast. Fourteen out of the 155 sera from breast cancer patients tested were positive for anti-p53 antibodies (9%) and no positives were detected among 164 control sera from normal women tested. The locations of the first metastasis in patients with positive sera were unusual, with more lung metastases and fewer bone metastases than expected. The detection of anti-p53 antibodies indicates that p53 is altered in amount, type or presentation in breast tumors so that it becomes immunogenic.

Four cases of so-called carcinosarcoma of the breast were studied histopathologically. The patients were females aged 44, 37, 44 and 77 who were admitted to the Cancer Institute Hospital for breast tumors. Histologically, the tumors showed proliferation of sarcomatous cells with relatively small amounts of papillotubular carcinoma or scirrhous carcinoma. There were transitions between the sarcomatous portion and carcinoma. Two cases showed squamous metaplasia merging into the sarcomatous component, whereas the other 2 showed transition from adenocarcinoma to sarcomatous element. Lymph nodal metastases in 2 cases were papillotubular carcinoma. One patient died of lung and brain metastases, and autopsy revealed only sarcomatous element.

We have investigated the modulation of prostatic acid phosphatase expression in the human prostatic cancer cell line LNCaP in response to the natural androgens testosterone and dihydrotestosterone, the female sex steroid estradiol and the synthetic androgen R1881 (methyltrienolone). Testosterone and dihydrotestosterone at 1 microgram/ml enhance the acid phosphatase synthesis by a factor of 3.5, while a hundred-fold lower concentration of the synthetic androgen R1881 induces an almost five-fold increase in the expression of this enzyme. The stimulation by all androgens tested and estradiol was dose-dependent. The synthetic glucocorticoid triamcinolone acetonide does not modulate the prostatic acid phosphatase expression in LNCaP cells, neither alone nor in combination with R1881.

Endometrial aspiration with the Karman cannula can be used to detect endometrial pathology. To document the feasibility and accuracy of endometrial aspiration with this cannula, 49 women were evaluated by this technique prior to diagnostic dilation and curettage (D&C). Completion rates for both endometrial aspiration and D&C were 96%. Endometrial aspiration yielded tissue adequate for histologic evaluation in 82% of cases as compared with 76% of D&C cases. Three cases of cancer (two endometrial and one endocervical) were identified by both techniques. A majority of patients expressed a preference for endometrial aspiration over D&C. Endometrial aspiration with the Karman cannula appears to be a convenient, accurate and acceptable method of detecting endometrial pathology.

Regional lymph nodes were examined in 4 groups of oncologic patients (with cancer of oral cavity, breast, stomach and lung). Histological types of the lymph node reaction were correlated to patients' survival. Favourable prognosis in all four groups was related to lymphocytic predominance, especially when combined with sinus histiocytosis. The predominance of germinal centres (combined with sinus histiocytosis) was a favourable sign but in stomach cancer. A marker of unfavourable prognosis was lymphocytic depletion, especially when sinus histiocytosis was absent.

The side effects of primary percutaneous radiotherapy in 100 patients with prostatic cancer were evaluated and classified into different degrees of gravity. It was shown that especially chronic side effects are benign in most of all cases. Severe - but not vitally dangerous - complications in bladder and intestine are rare (3%); their percentage corresponds roughly to the average mortality rate of surgical intervention (radical prostatectomy). Contrary to operation, troubles of the sexual function appear only in one third of the patients. Taking into consideration the similar therapeutic results of surgical intervention and radiotherapy, radiotherapy is more favorable with respect to side effects. This essential aspect should be considered when making a decision whether to apply a surgical or a radiotherapeutic treatment treatment.

TITLE: [Regression of adenomas in Gardner's syndrome induced by systemic chemotherapy]

A 31-year-old male was admitted to our hospital with Gardner's syndrome and sigmoid colon cancer. Palliative resection (sigmoid colectomy) was performed due to hepatic and lymph node metastasis. Systemic chemotherapy with MMC, 5-FU and PSK was started postoperatively. Barium enema study on the 23rd successive post-operative day and fiberscopic study on the 134th post-operative day showed regression of the size and number of the polyps in the remaining colon and rectum. We suggest that the administration of anticancer drugs may be useful in the treatment of familial polyposis or Gardner's syndrome which have been treated with only surgical therapy.

TITLE: Knowledge of testicular cancer risk and need for self-examination in college students: a call for equal time for men in teaching of early cancer detection techniques.

College athletes were surveyed about their knowledge and practice of early cancer detection techniques. Males were almost completely unaware of their risk for testicular cancer (87%). Only 9.6% had been taught testicular self-examination and only half of these by their physician. Six percent actually examined themselves regularly. In comparison, more than 60% of women had been taught breast self-examination (75% by a physician) and about one third were doing regular examinations. More than 90% of the young men and women had been seen by physicians for a physical examination within the past 3 years. Physicians need to begin educating males about testicular cancer and its early detection.
