The data of 63 female patients suffering from pulmonary and/or pleural metastases of breast cancer are recorded. The patients received chemotherapy, according to the CMF (Cyclosphosphamide-Methotrexate-Fluorouracil) scheme, and in case of appropriate indication, a hormone-therapy as well. The mean survival time of the deceased was 22 months whereas the mean follow-up time of those still living is 61 months. These results are better, than those of the literature, mainly if they are compared to the data of patients with inoperable primary lung cancer.

High level of antibodies to microbial glycoprotein in blood sera was revealed in cancer and precancer patients. Analogical results were obtained in mice A/Sn and Balb/c with inductive or transplanted tumors. It has been suggested the use of Bacillus megaterium H glycoprotein (M. m. 65-70 kD) for immunological monitoring.

A case of disseminated testicular cancer relapsing after 6 and 8 yr is reported, and the literature on late relapses is reviewed. About 4% of potentially cured testicular cancer patients do relapse after two or more years. Early detection is important and we recommend an indefinite follow-up, because salvage therapy can be effective when the extent of disease is limited.

Bleomycin-reactive iron was detected in the sera of six out of nine adults undergoing intensive chemotherapy for acute non-lymphocytic leukemia. In these individuals the corresponding transferrin saturation ranged from 96% to 113% and the serum ferritin from 775 to 9975 micrograms/l. Nontransferrin-bound iron has been postulated to be a factor in organ toxicity in iron overload conditions such as beta thalassemia and hereditary hemochromatosis by facilitating the production of tissue-damaging free radicals. We propose that bleomycin-reactive iron should be considered as a possible factor in organ dysfunction seen with intensive cancer chemotherapy.

TITLE: Vaginosonographic detection of endometrial cancer in postmenopausal women.

In a prospective study 155 postmenopausal women not on hormone treatment underwent a vaginal ultrasound investigation. All women were routine patients of the regular ambulance. In addition to uterus biometry, the endometrial thickness was also measured. An endometrium of greater than or equal to 4 mm thickness was clarified histologically by means of a curettage. In total we found 7 endometrial cancers and 1 cervical cancer. Vaginal sonography proved to be a very sensitive (81%) and specific (89%) method for the detection of endometrial neoplasia.

Nitrosamines are ubiquitous in our environment and diet. Many nitroso compounds are carcinogenic in animals and most probably in man. Nitrosamines are formed from the reaction of nitrite with primary, secondary, or tertiary amines in an acid medium. Nitrate should be considered as a nitrosating agent because it can be converted to nitrite by microbial action. Many aliphatic and nitrogen-containing heterocyclic compounds can be nitrosated to form carcinogenic substances. The occurrence in food and in some drugs of several nitrosamines or their nitrosatable precursors is described. Several tobacco-specific nitrosamines have been considered as possible causative agents for human cancer. Nitrosamines may be implicated in the induction of certain human gastric cancers.

Fecal specimens from 10 colorectal cancer, 1 duodenum diverticulosis and 2 healthy adults were examined. Nine fecal enrichments contained Methanobrevibacter. The percentage of methanogen positive individuals was 69% Methanobrevibacter were isolated from fecal enrichments of 4 colorectal cancer and 1 duodenum diverticulosis. The percentage of breath methane positive individuals and methane production had significantly increased in colorectal cancer patients. It will be hopeful that methane production in breath samples at the end of exhalation will became a rapid, simple and non-penetrative method for monitoring precancerous colonic or rectal carcinogenesis and monitoring the relapse of individuals with carcinosectomy.

A model for carcinogenesis that postulates two rate-limiting events for malignant transformation is a generalization of the recessive oncogenesis hypothesis, according to which inactivation of homologous tumor suppressor genes leads to cancer. This model has been shown to be consistent with a large body of epidemiologic and experimental data and has recently been used for the analysis of altered hepatic foci in rodents. These foci are considered to be premalignant lesions. In this paper the necessary mathematics for the joint analysis of premalignant and malignant lesions are developed within the framework of this model.

Peak concentrations were observed within 1 h of dosage and concentrations then declined biexponentially. Plasma morphine-3-glucuronide (M3G) and morphine-6-glucuronide (M6G) concentrations were measured in two patients and plasma M3G concentrations were observed to be much higher than plasma M6G and morphine concentrations. Peak plasma M6G concentrations occurred within 1.0 h of dosing and plasma M6G concentrations then remained higher than plasma morphine concentrations.

Three siblings whose mother had an ovarian mass excised as a teenager all developed malignancies in late childhood. Two were diagnosed as having malignant germ cell tumors of the ovary, whereas the third was found to have a soft tissue sarcoma. No underlying familial disease or constitutional chromosomal aberration has been detected and no known carcinogenic chemical exposure has been identified. All three children have been successfully treated with chemotherapy and are doing well off treatment. This is the first family cancer syndrome reported to have an association of ovarian germ cell tumors with embryonal sarcoma.

We studied samples from gallbladders of 10 subjects with invasive and 10 with in situ carcinoma of this organ. The number of nucleolar organizer regions, as revealed by colloidal silver staining, was at least 4 times larger in cancer samples than in control ones. Mean number of nucleolar organizer regions for invasive lesions was higher than for in situ ones (11.6 +/- 2.3 vs 9.4 +/- 1.3, p less than 0.04). The study of cellular markers may be helpful in establishing the preneoplastic nature of gallbladder lesions.

The incidence of melanoma is increasing more rapidly than any other form of cancer (with the exception of lung cancer in women). Public education programs have the potential to prevent future skin cancers and promote early detection of skin cancer and melanoma. This article describes a statewide awareness campaign and outlines important nursing activities and educational resources. Among results of a pre- and postevaluation poll are an increase in the number of respondents correctly identifying melanoma as the most serious type of skin cancer and an increase in the number of respondents who stated that sunburn in children was very serious.

Combination chemotherapy with Cyclophosphamide, Adriamycin and Cisplatin (CAP) is one of the most effective chemotherapies for ovarian cancer. Severe vomiting induced by this treatment may cause a disorder of the serum electrolytes. Severe hyponatremia was observed in one patient who complained of convulsions and unconsciousness. Accordingly we studied the disorder of serum electrolytes in 158 courses of CAP treatment (44 patients) during the five years from 1984 to 1988. The serum electrolytes were influenced by the amount of vomiting in this study. Consequently frequent examination of serum electrolytes is necessary during this treatment.

Incidentally discovered cancer of the prostate may be divided into focal and diffuse disease. The focal tumour tends to be of low grade and low-volume and in the majority of patients runs a clinically benign course. In 10-15% of untreated patients, however, progression occurs by 10 years after diagnosis. At the same stage of follow-up 30-63% of the patients have died of other causes, with no evidence of recurrence. In patients with low-grade focal cancer of the prostate, radical prostatectomy may be curative. An alternative management option is to closely observe these patients. Digital rectal examination, prostatic specific antigen, transrectal prostatic ultrasound and repeated prostatic biopsies can all make contributions to the follow-up of these patients.
