TITLE: A statistical model for predicting response of breast cancer patients to cytotoxic chemotherapy.

A binary logistic model is used for predicting response to cytotoxic chemotherapy for a breast cancer patient on the basis of her tumor enzyme activity profile. The enzymes used in the model are lactate dehydrogenase, nicotinamide adenine dinucleotide phosphate-isocitrate dehydrogenase, and phosphoglucomutase, all of which were measured on primary tumor specimens from each patient. The statistical model provides an estimate of the probability that an individual will respond to treatment. Chemotherapeutic treatment consisting of combination cytotoxic drugs and subsequent evaluation of patient response followed cooperative group protocol guidelines, including outside review to confirm the patient evaluation. The model based on this study, which represents 5 years of patient follow-up, correctly predicts clinical outcome in 32 of the 37 cases available.

It is hypothesized that chronic gastritis and ulcerative colitis both are induced by viral infection, and that such chronic infection of the mucosa may lead to ulceration and occasionally cancer. Duodenal ulcer disease and Crohn's disease may on the other hand, be due to activation of latent viral infection of the corresponding neural ganglions, with subsequent migration of virus along the nerves to the gut wall. The gastric acid hypersecretion often occurring in patients with duodenal ulcer disease might be a consequence of viral interference with the efferent nerve function of vagal ganglions. Correspondingly, non-ulcer dyspepsia as well as irritable colon may reflect viral infection of afferent nerve function leading to pain and discomfort.

We see no other avenue that offers as bright a prospect for a meaningful reduction in breast cancer mortality and the monstrous toll--medical, emotional, sociologic and monetary--it exacts.

TITLE: [Changes in the proliferative activity of human breast cancer cells cultivated in vivo in diffusion chambers in response to antineoplastic agents]

A study was made of the effect of nitrosomethylurea (NMU), thiophosphamide and diazane on proliferative activity of human breast cancer cells cultured in diffusion chambers in vivo. NMU highly suppressed the proliferation of human breast cancer cells throughout the experiment. The maximum effect of diazane and thiophosphamide became apparent after 24 hours, while almost complete renewal of the proliferation was observed by the 72nd hour of culture. Diazane and NMU hold promise for clinical trials in human breast cancer.

TITLE: Hodgkin's disease in patients with previous infectious mononucleosis.

Patients with a positive reaction to the Paul-Bunnell test in the period 1961-72 were identified at nine different laboratories in Norway and matched against cases of malignant lymphoma registered at the Cancer Registry of Norway in the period 1961-75. Among 5,840 patients having a positive Paul-Bunnell test a total of six developed malignant lymphoma, three of these more than 1 year after the positive Paul-Bunnell test. The expected number of malignant lymphoma was 2.04. Of the six lymphoma cases, five were classified as Hodgkin's disease (HD). The present investigation agrees well with the findings of other epidemiological studies. There seems to be a small excess in incidence of HD among patients with previous infectious mononucleosis (IM), but both diagnostic problems and possible confounding factors must be taken into account before a possible causal association is considered.

TITLE: Breast self-examinations: who does them and why.

In a population of 158 women with a breast concern, two-thirds examined their own breasts. Most examiners (80%) practiced breast self-examination at least once a month, and over 60% had been regular examiners for more than 2 years. No significant difference in frequency of breast self-examination was found between women who had been examiners for longer and shorter time periods. Examiners had two main reasons for beginning and continuing breast self-examination: (1) an awareness that it is desirable to detect breast cancer early and (2) an awareness that they themselves could get breast cancer, while nonexaminers tended to deny this possibility. Therefore, to become examiners, nonexaminers may need to be convinced not only that early detection is effective but also that they too are at risk of breast cancer. The importance of informing, but not alarming, women about their breast cancer risk is discussed.

As some authors suspect that ozone influences the metabolic process of fat, we tried to analyse the influence of an ozone-oxygen gas which was applied parenterally. 40 patients with gynecological cancer received 10 ml ozone-oxygen gas with a content of 450 gamma ozone and venous blood was removed before and 10 minutes after application. The serum was lyophilized and the level of fatty acids and triglycerids was determined by the method of Randerath (1965). a statistically significant decrease of the concentrations was observed after application of ozone. Different theories as to the cause of this action are discussed.

A relatively short, simple procedure is presented to separate serum high-density lipoproteins discretely into the two main classes, those with densities between 1.063 and 1.125 g/ml (HDL2) and those with densities between 1.125 and 1.210 g/ml (HDL3). A 3.5% polyacrylamide gel in 10 cm glass tubes and the use of Tris/glycine buffer, pH 8.4, will accomplish this separation. The components can be identified in several different ways, examples of which are given. This procedure will give rapid and reliable estimations of both HDL2 and HDL3, and can be used to relate their levels and proportional amounts to incidence or risk of atherosclerosis, coronary-artery disease and possibly cancer.

The pharmacokinetics of fluorouracil were examined after single 250-mg iv doses and 500-mg oral doses to female patients with breast cancer. In five patients who received intravenous fluorouracil, the mean peak plasma level of unchanged drug was 13.4 microgram/ml, the elimination half-life was 6.3 min, and the plasma clearance was 1410 ml/min. The last value is similar to the hepatic blood flow. In six patients who received oral fluorouracil, the mean peak value of unchanged drug in plasma, which occurred within 20 min of dosing, was 8.3 microgram/ml, and the fluorouracil elimination half-life was 7.2 min. The overall bioavailability of oral fluorouracil as unchanged drug was 28%, and the variation in plasma drug levels between individuals was similar following oral and intravenous doses. The data provide additional evidence of saturable hepatic metabolism of fluorouracil during the first pass.

From the clinical and pathologic findings, the characteristics of recurrence of gastric cancer more than five years postoperatively were evaluated as compared with recurrence of less than five years postoperatively. The late (more than 5 years postoperative) recurrence rate was 2.0 per cent, or 17 of 864 patients who had undergone the so-called radical operation. The duration of disease prior to the first operation was comparatively long in the patients with late recurrence. Late recurrence is characteristic of localized cancer, such as Borrmann's type II, and differentiated adenocarcinoma with nonserosal invasion (pathologic findings of the resected stomach at the first operation) and by a recurrence pattern of hematogenous metastasis, especially liver metastasis.

This monograph has attempted to summarize in a general fashion the ever-enlarging spectrum of the paraneoplastic syndromes. Recognition of their presence at times can lead to the earlier diagnosis of an underlying malignancy. In many instances, palliative therapy or correction of the syndrome may produce relief from the multiplicity of symptoms that develop. The syndromes may be readily apparent or subtle in their onset. Some are life threatening. Prompt recognition and institution of therapy are essential in those instances. The paraneoplastic syndromes usually are categorized by organ system, target organ involved or, where possible, by mediating factors. A careful study of these symptom complexes may lead to better methods of detecting cancer, as well as to a better understanding of its etiology and prevention in the human.

This study describes the mortality experience of a cohort of 61 males involved in a 1964 chloracne incident. Presumably as a result of skin absorption of the process contaminant, 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD or dioxin), 49 of these trichlorophenol production workers developed the skin condition. Within the limitations posed by cohort size and length of follow-up, TCDD does not appear to have adversely affected mortality experience. Overall, four deaths occurred and 7.8 were expected. Of these, one death was due to cardiovascular disease (3.8 expected) and three deaths were attributed to cancer (1.6 expected). None of the findings were statistically significant at alpha = 0.05. Further follow-up is contemplated.

Appropriate death was defined as the kind of death a person might choose for himself/herself, if given a choice. Ss were 382 college students who stated their preferences for (a) age at time of death; (b) cause of death; (c) site of death; and (d) means of disposal of remains. Preferences were compared with national actuarial projections. The preferred average life expectancy for women was lower than the national average life expectancy (p less than .001). Natural causes and accidents were oversubscribed, while cardiovascular ailments and cancer were undersubscribed as causes of death. A non-institutional site for death was preferred by 91.3% of the Ss; 39% of all deaths now occur outside an institution. Traditional burial underground was the choice of just more than half the sample, but a slight trend toward less traditional means of disposal of remains was noted over the five semesters of the study.

TITLE: Effect of streptococcal preparation (picibanil) on the postoperative rise in serum alanine aminotransferase activity in patients with urogenital cancer.

The effect of Picibanil, a streptococcal agent, on the development of liver injury after operations for urogenital cancer was studied retrospectively in the light of serum alanine aminotransferase (ALT) activity. The series comprised 32 cases receiving Picibanil and 33 controls with otherwise comparable clinical backgrounds. Picibanil reduced the incidence of postoperative ALT rise over 50 U/l within 6 weeks but increased it thereafter. The increase in ALT activity after 6 weeks was relatively small and was seen more often in patients given blood transfusions. It was interpreted as retardation and suppression of ALT rise and as being related to the induction of interferon or to immunopotentiation. Other antihepatotoxic effects of Picibanil, due to its antioxidant activity, for example, may also account for the prevention of the early postoperative rise in ALT activity.

TITLE: Angiosarcoma metastatic to the maxillary tuberosity gingiva.

A case of angiosarcoma metastatic to the maxillary gingiva is presented. A review of the literature and an epidemiologic survey of angiosarcoma cases since 1944 at the M. D. Anderson Hospital and Tumor Institute prove this to be an extremely rare occurrence. Generally, metastasis to the oral/perioral tissues occurs as a late sequela secondary to diffuse metastatic disease. In this case, however, two extensive metastatic tumor surveys determined that the only metastasis was to the oral site. The lesion clinically mimicked an inflammatory process. It is hoped that this presentation will reinforce aggressive pursuit of innocuous-appearing oral lesions when the patient has a history of malignant disease and will further encourage dental involvement in the oral/dental assessment and are of the cancer patient.
