Experience with endocurietherapy of skull base tumors is reviewed. We present our cases of recurrent pituitary hemangiopericytoma, radiation-induced recurrent meningioma, recurrent clival chordoma, recurrent nasopharyngeal cancer involving the cavernous sinus, and recurrent parotid carcinoma of the skull base which were all successfully retreated with high-activity 125iodine (I-125) permanent implantation.

Gastrostomy by endoscopy is a simple and fast procedure which does not require general anaesthesia. Its morbidity and mortality rates are low. With the initial technique, the gastrostomy tube was pulled in by a thread introduced percutaneously into the stomach. In cases with oesophageal stenosis, using a Foley balloon catheter is useful. Gastrostomy can be transformed into jejunostomy by a similar method. The main indications of percutaneous endoscopic gastrostomy are cancer of the upper respiratory or digestive tract, notably those complicated by fistulae, and disorders of deglutition due to abnormal buccal and pharyngeal stages.

Several specific areas discussed include: problems of superinfection with newer antimicrobial agents; the current inadequacy of diagnostic techniques; the potential for early treatment with monoclonal antibodies; the potential for augmenting one or more host defenses prior to sundering other defenses during antitumor chemotherapy; the potential value of a variety of nutrients that either may be depleted or might be of benefit in enhancing host defense mechanisms.

The degree of T cell infiltration was divided into 4 groups according to the number of infiltrating T cells. There was a good correlation between the degree of T cell infiltration and the survival period or disease-free period. It may be clinically useful to check the degree of T cell infiltration in cancer tissues for predicting the prognosis of patients with breast cancer.

TITLE: Consumption of dairy produce and alcohol in a case-control study of breast cancer.

In a French case-control study of 1,010 breast cancer cases and 1,950 controls with nonmalignant diseases, the risk of breast cancer was found to be positively associated with frequency of cheese consumption and the level of fat in the milk consumed. A negative association was found between frequency of yogurt consumption and the risk of breast cancer. No association was found between the consumption of butter and the risk of breast cancer. The positive association between a daily consumption of alcohol and the risk of breast cancer previously reported was not altered when dairy produce consumption was taken into account.

We report a protocol for concomitant purification to homogeneity of both prostatic acid phosphatase [orthophosphoric-monoester phosphohydrolase (acid optimum), EC 3.1.3.2] and prostate-specific antigen, from human seminal fluid. The method requires only two chromatographic steps: passage through an Affigel-Blue column and gel filtration HPLC. This is a fast, efficient procedure for purification of these two important tumor markers, which are specific for prostatic cancer.

A decade ago the five-year survival rate in patients with stage I ovarian carcinoma was only 60% to 70%. Significant advances in staging and treatment, however, promise an improvement. Initially, improvement will be seen because some patients with clinically localized ovarian cancer will be discovered to have subclinical or occult metastasis at initial surgical staging and thus will be upstaged and treated appropriately for more advanced disease. More encouraging are results of recent studies on adjuvant therapy, which indicate that in some patients the need for postoperative adjuvant therapy may be obviated. A major advance in diagnosis would be development of a serum test specific for ovarian cancer--a discovery that may be forthcoming.

Anthracyclines are amongst the most effective antitumor agents currently utilized. The major impediment to their use has been the development of cardiotoxicity with chronic administration. In an effort to overcome this limitation we have utilizated an approach to abrogate the cardiotoxicity with a drug, ICRF-187, which protects against the anthracycline cardiomyopathy in all animal species so far tested. This strategy assumes that separate mechanisms of cardiac toxicity and antitumor activity exist. We are currently testing this hypothesis in a randomized clinical trial of women with advanced breast cancer who are randomized to receive 5FU, doxorubicin, and cyclophosphamide (FAC) or FAC plus ICRF-187.

TITLE: Comparison of flutamide and Emcyt in hormone-refractory metastatic prostatic cancer.

A prospective randomized trial for metastatic prostatic cancer in 220 patients who were refractory to hormone therapy was conducted by the National Prostatic Cancer Project from 1984 to 1985. As of July, 1986, the evaluation of these patients reflected no difference in response to either estramustine phosphate (Emcyt) or flutamide. Toxicities were minimal, and the observed survival and progression-free survival intervals were noteworthy in view of the overall prospects for such patients. Future studies dealing with specific quality of life issues seem to be indicated by our results.

Cancer of the esophagus has shown peculiar worldwide geographic clustering; it has also occurred in excess among individuals in lower socioeconomic census tracts of certain cities. These facts have prompted inquiry into its etiology. Emphasis has been given to environmental effects, particularly mechanical and/or irritating processes, dietary deficiency, and a variety of carcinogens. Immunological susceptibility has been considered, although only infrequently. Overall, primary genetic factors have received short shrift in epidemiological investigations of this disease. An hypothesis of stress related cancer in concert with genotypic susceptibility is presented.

TITLE: An alternative approach to the vertical midline abdominal incision for staging in ovarian carcinoma.

The case report describes a situation that required staging laparotomy for ovarian cancer but the patient refused vertical incision for cosmetic reasons. The technique employed a low transverse skin incision with development of a skin flap and horizontal transection of the midabdominal wall near the level of the umbilicus. Operative exposure was adequate for standard staging procedures and the cosmetic requirements of the patient were satisfied.

TITLE: [Study of immunosuppressive acidic protein (IAP) in patients with renal cell cancer]

We evaluated immunosuppressive acidic protein (IAP) as a tumor marker in renal cell cancer. The study was comprised of patients with renal cell cancer (6 stage A, 3 stage C, 11 stage D, 7 recurrences). The IAP positive rate of renal cell cancer was 84%, the mean was 937.5 +/- 442.1 ug/ml. The positive rate is statistically higher than in other malignant and benign urological diseases. The IAP positive rate tended to increase with cancer stage. IAP may represent a tumor marker in renal cell cancer.

TITLE: Synchronous double primary malignant lymphoma of low grade malignancy and early cancer (collision tumor) of the stomach.

Collision tumors of the stomach are rare tumors, with about 32 cases reported in the English and German literature. In this report the case of a 65-year-old male with a synchronous primary malignant lymphoma (immunocytoma) and an early cancer of the stomach is presented. A total gastrectomy with esophago-jejunostomy was performed. Postoperatively the affected region was irradiated. 30 months later a re-evaluation did not reveal any recurrence of the disease. The diagnostic and therapeutic problems of collision tumors of the stomach and a possible relationship between the two tumors are discussed against the background of the literature.

TITLE: Our experience with resection of primary and secondary liver tumours.

The authors retrospectively analyse the results achieved in recent years by surgical treatment of 103 malignant primary or secondary liver cancers: (Three patients have been operated on twice at different times for recurrence.) 23 hepatic resections were performed for benign lesions. The operative mortality is very low, the incidence of postoperative complications is minimal, and the survival up to 8 years is rather satisfactory. In all cases we used Ton That Tung's surgical technique, that is, "finger fracture with intraparenchymal ligature of the vasculobiliary pedicles."

The incidence of pancreatic cancer has increased recently. The average age of pancreatic cancer patients in our series was 60.6 years; the male to female ratio was 2.3:1. As for tumor location, the head (45.5%) was the most frequent followed by the body (32.7%) and the tail (20%). Pancreatic cancers were histologically classified as duct carcinoma (88.2%), acinar cell carcinoma (0.9%), islet cell carcinoma (1.8%) and undifferentiated carcinoma (9.1%). In 17 cases (15.5%), diabetes mellitus was an accompanying complication.
