TITLE: Rheumatic manifestations of malignancy.
PUBDATE: 19960101
Rheumatic manifestations of malignancy include a wide spectrum of osteoarticular, muscular, glandular, endocrinologic, and systemic features, posing a therapeutic challenge. The clinician should be aware that Sjögren's syndrome, polymyositisdermatomyositis, rheumatoid and rheumatoid-like arthritis, polymyalgia rheumatica and temporal arteritis, and diverse osteomuscular conditions may be the immunopathogenic features of a neoplasm, the direct consequence of osteomuscular tumors, the effect of tumor-associated hormones, or the consequence of cancer therapy. The principal articles that have appeared in the past year on these associations are discussed. We also review the association of x-ray irradiation and cancer in patients with ankylosing spondylitis.

TITLE: An open systems approach to risk assessment.
PUBDATE: 19940301
The development of risk assessment in occupational and environmental cancer control is outlined, with emphasis on the differing assumptions of Doll and Selikoff. These differences are explained by philosophic perspectives from which closed and open system approaches to assessment were respectively chosen. Open system, with negative entropy and progressive organization, are characteristic of living systems and, heuristically, should be characteristic of risk assessment models. The abuse of either approach occurs when the "habit of truth" is overcome by consciously contrived confusion, a betrayal of society by les clercs.

Breast examination for early cancer detection in high risk patients are being performed even more frequently in our country and in other ones. Wide availability of mammosonography facilitates such management. X-ray mammography in view of irradiation is less frequently used. An attempt has been undertaken at carrying out breast examinations of a selected Pałuski area. The analysis included 180 women who after carrying out comprehensive breast examination and obtaining of history data were qualified for the group of high risk for breast cancer.

TITLE: Relationship between DNA methylation and gene expression of the HOXB gene cluster in small cell lung cancers.
PUBDATE: 19960201
The expression pattern of the HOXB gene cluster in four xenografted small-cell lung cancers was compared to the methylation of the DNA in the corresponding genomic regions. In 90% (17/19) of the studied cases, the expressed genes were in methylated regions whereas 70% (12/17) of the unexpressed genes were in unmethylated regions. This specific behavior could correspond to a particular gene expression regulation mechanism of the HOX gene network. Since some genes (HOXB2, HOXB4, HOXB7) were always inactive when unmethylated, this unexpected relationship might indicate their key function(s) in the HOX gene network.

PUBDATE: 19940701
In this report, 88 cases of gallbladder adenoma collected from Chinese medical literature including the author's own 12 cases were analysed. Canceration was found in 17 (19.3%) cases. Risk factors included multiple and sessile adenoma, tubular adenoma, adenoma of more than 1 cm in diameter, and adenoma coexistent with gallstones. It is the authors' believe that any polypoid neoplasms of the gallbladder of more than 0.5 cm in diameter found by B-mode ultrasonography justify surgical exploration in order not to overlook a possible canceration.

Ten patients with carcinoma of the lower rectum and anus underwent laparoscopic abdominoperineal resection. They were 7 male and 3 female patients. The ages ranged from 32 to 81 years. The technique is fully described. The immediate results were satisfactory. There was 1 case of postoperative bleeding that required open surgery and 1 case of threatened colostomy gangrene that was easily corrected. There was no mortality. The advantages were less pain, less scar, easier care of the colostomy site, quicker return of bowel function, and less manipulation of the cancer. The laparoscopic approach was as efficient as open surgery.

Our study demonstrates that tamoxifen, when administered to postmenopausal women at a conventional dosage, reduces LDL levels and protects LDL from oxidation. The protective effect of tamoxifen against the development of breast cancer in women considered at risk is being investigated in a placebo-controlled trial sponsored by the National Institutes of Health. Whether tamoxifen also protects against the development of cardiovascular disease in this trial is also of considerable interest.

The development of interventional radiology and the like has made it possible to reduce the invasion and expand the applicability of intra-arterial infusion chemotherapy for unresectable cancer. By employing such methods and combining chemoembolization with continuous intra-arterial infusion chemotherapy, an response rate of 64% has been achieved in cases of unresectable hepatic metastases of colon cancer. These successful cases were reported along with personal comments by the authors.

PUBDATE: 19940301
The role of tumor markers in the management of patients with epithelial ovarian cancer is expanding rapidly. Initially using heteroantisera and, most recently, monoclonal antibody-dependent assays, clinicians are equipped with new modalities that may be useful in evaluating disease status, monitoring the efficacy of therapy, diagnosing early recurrence, and screening patients for ovarian cancer. A knowledge of the efficacy of the most widely tested markers is important if they are to be used judiciously and in appropriate settings.

TITLE: Lack of recognition of a mouse centromere by anti-centromere proteins.
PUBDATE: 19961001
In studies dealing with cancer as well as mutagenesis, the centromere region is being looked at critically. Generally, this region of chromosomes reacts to certain antibodies present in the sera of scleroderma patients. Some chromosomes in the Cl1D cell culture of mouse origin fail to respond to these antibodies even though, in a sub-metacentric chromosome, they do react at a site distal to the centromere. This site of reaction, though, coincides with the presence of minor satellite DNA. Is it that neither the minor satellite nor the so-called CENPs constitute an essential component of a functional centromere?

A 44-year-old premenopausal woman with bone metastases of breast cancer was initially treated with systemic chemotherapy (CEF) and radiation therapy after standard mastectomy. However, progressive change of bone metastases with elevation of tumor markers (CEA, NCC-ST 439) was detected, so continuous administration of LH-RH agonist and combination chemotherapy (CEF) were conducted. Subsequently, complete objective regression was attained after 40 weeks.

The activity of 17 was similar to that of bruceantin (1) in this in vitro cell line panel.

Genomic imprinting is defined as a differential expression of genes depending on their parental origin. IGF2 gene on chromosome 11p, which is maternally imprinted in normal human development, has been demonstrated to be activated by loss of imprinting (LOI) during tumorigenesis of Wilms tumor or other kidney neoplasms in childhood. Although the molecular mechanism of genomic imprinting is not clarified yet, it is certain that disruption of the imprinting mechanism has a causative role in some human cancers.

More than 100 years ago, William Coley published the first report of tumor regression induced by immune system activation. Since that time, numerous clinical trials of cancer vaccines have been carried out. However, active immunotherapy has not yet become an established modality of cancer therapy. Recently, striking progress has been accomplished in both the understanding of antitumor immune response and the characterization of tumor rejection antigens in human species. This progress serves as a basis for the development of new cancer vaccines.

PUBDATE: 19970401
Hyperfractionation is generally expected to allow an escalation of total dose, thereby increasing tumour control rate, without increasing the risk of late complications. The purpose of this review is to assess the empirical evidence for this therapeutic gain from hyperfractionated radiotherapy. Although extensive clinical data have been accumulated until now, especially on treatment of head and neck cancer, the line of evidence is not consistent. The present analysis indicates that the dose per fraction generally used in standard radiotherapy is already a good choice.
