In Vestfold, with a population of 192,200, all patients who died from cancer in 1987 were followed retrospectively through their last six months. 61% (n = 342) had cancer-related pain and received analgesic treatment. In 9% pain remained a problem. In the same group, 74% suffered from other symptoms, such as nausea, dyspnoea, anxiety and depression. These symptoms were reduced efficiently in only 58%. Nausea was the main problem. We found a significant connection between nausea, chemotherapy and opiate treatment. Anxiety and depression were recognised in about 20% of the patients, and were seldom adequately relieved. In order to provide optimal quality of life, more attention should be drawn not only to pain but also to the other symptoms.

In the USA malignant neoplasms of the skin are the most common cancers of man. Annually, over 500,000 new cutaneous cancers are diagnosed. Of particular concern is the inexorable increase in the incidence and mortality rate of malignant melanoma (in 1978: 25,000 new cases; 5800 deaths). There are substantial data that (1) implicate sunlight as the probable cause of many cancers of the skin, and that, (2) indicate early diagnosis and prompt treatment results in cure in most instances. The time has come to create, implement, and evaluate a National Program on Education of the Medical Profession and the Public on Cancers of the Skin with emphasis on cutaneous malignant melanoma.

During the cancer check-up of a 46-year-old woman a PAP V was diagnosed in the cervical scrapings. The atypical cells of the abrasion specimens from the corpus uteri arouse strong suspicion of malignancy. Connected with the clinical data, this finding primarily indicated a metastatic involvement of the inner genitals. This suspicion was confirmed by exploratory surgery laparotomy. A large tumor was found intraoperatively which was occupying the entire stomach. Concurrently, a diffuse peritoneal carcinomatosis was found with involvement of the left adnexae in the sense of a Krukenberg's tumor.

Chemotherapy prolongs survival of patients with small-cell lung cancer, but very few are cured, and the treatment is unpleasant. Thirty patients, 28 with advanced disease, were treated with etoposide 250 mg/m2 orally, daily for 5 days, plus vincristine 2 mg intravenously on the first day, the cycle being repeated 3-weekly, to a maximum of 6. There was a response rate of 70%, a median survival of 249 days, and an 11% 2-year survival. Symptomatic side-effects were less pronounced than with most other regimens of comparable efficacy.

Early esophageal cancer was discovered in a 55-year-old male with liver cirrhosis six months after the start of treatment for esophageal varices by a series of endoscopic sclerotherapeutic injections using 5% ethanolamine oleate. A histological examination of the entire length of the resected esophageal wall revealed a carcinomatous lesion oralwards and far removed from the lesion where subsequent changes due to sclerotherapy were prominent. Further, the causal relationship between sclerotherapy and carcinoma is discussed.

A patient developed a large perineal hernia and prolapse of a myocutaneous neovagina 13 months after total pelvic exenteration for recurrent squamous cell cancer of the cervix and gracilis myocutaneous vaginal reconstruction. The neovagina was removed and the pelvic floor defect was repaired with preserved human dura. Twelve months postoperatively, the patient remained asymptomatic. Current and potential uses for human dura allografts are discussed.

TITLE: Stomach cancer: a prospective study of anastomotic failure following total gastrectomy.

The incidence and cause of esophagojejunal anastomotic leakage were prospectively studied in 350 patients following total gastrectomy for stomach cancer. Anastomotic leak was demonstrated in 30 patients (8.6%), ten of whom underwent reoperation. Nine of the 30 patients died. Statistical analysis revealed that the outcome was better for patients with stapled esophagojejunostomy than when the anastomosis was hand-sutured. The odds for leakage were 2.37 times higher in patients with hand-sutured, than in those with stapled anastomosis. Antibiotic prophylaxis may be significant in preventing leakage. No association was found between anastomotic leakage and type of hospital, patient age or sex, preoperative weight loss, concomitant splenectomy or residual tumor in the resection margin.

During the last 9 years, 1,892 patients underwent surgery of the prostate. Benign prostatic hypertrophy (BPH) accounted for 1,812 cases, and prostatic cancer 80 cases. In the case of BPH, transurethral resection was done in 1,645 cases, cryosurgery in 136 cases, and suprapubic prostatectomy in 31 cases. Latent cancer was detected in 54 (3.3%) of the BPH cases. The average patient age of the patients with latent cancer (73.6 +/- 6.9 years) was higher than that of those without latent cancer (69.5 +/- 7.7 years). This suggested a significant relationship between the age of the patient and the occurrence of latent cancer.

One hundred four psoriatic patients who had 8-methoxypsoralen-ultraviolet light A (PUVA) treatment between 1975-1987 were investigated to determine if skin cancer had ever occurred during or after the therapeutic period. Incidence data of skin cancer was applied to this group of patients to measure the expected number of patients with skin cancer. The significance of observed number versus expected of PUVA-treated patients with skin cancer was analyzed. The authors did not observe a significant increase of patients with skin cancer.

Mastectomy and CS and RT are currently the two major options for patients with early breast cancer. Prospective randomized trials have shown comparable survival rates for these two treatments. The major selection criterion for CS and RT is the relative size of the tumor and the breast. The surgeon must be able to resect the tumor and still preserve a good cosmetic appearance. In most cases, an axillary dissection is also performed and RT is delivered to the breast alone. With good surgical and radiotherapeutic technique, good to excellent cosmetic results are generally obtained, and complications are uncommon.

TITLE: [Spontaneous urinary extravasation: report of three cases]

We report three cases of urinary extravasation successfully recovered by conservative measures. Two of them were regarded as spontaneous peripelvic extravasation caused by small ureteral stones. After several days' indwelling ureteral catheter extravasation was ceased and the stones were discharged spontaneously in both cases. The other was regarded as spontaneous rupture of the renal pelvis with urinoma formation due to ureteral obstruction by bladder cancer invasion. Intra-arterial chemotherapy, radiation and hyperthermotherapy to the bladder under percutaneous nephrostomy yielded recanalization of obstructed ureter and resolving of urinoma. We shortly review previous reports and discuss the role of diagnosis and treatment.

TITLE: The role of partial cystectomy in the management of transitional bladder cancer.

A study on 81 patients who underwent partial cystectomy for bladder cancer is presented. The low postoperative mortality (2.46%) and the high 5-year survival (72%) show that this type of surgery is effective and without major complications. Thirty-eight patients were treated with endovesical thiotepa (OTT); the remaining 41 patients were untreated after surgery. Results showed that survival and number of relapses were similar in the two groups. This fact seems to prove that chemotherapy with OTT is not essential after segmental cystectomy. To explain this assertion, several hypotheses are presented.

TITLE: Isolation and purification of placental type alkaline phosphatase from a seminoma.

An alkaline phosphatase (EC 3.1.3.1) of the placental type was isolated from a seminoma type of human testicular cancer tissue and was purified to homogeneity by sulfate-mediated chromatography on a column of Cibacron Blue Sepharose 4B. The purified enzyme had a specific activity of 40.6 kU per gram of protein and was obtained in a yield of 37%. The purification procedure used was simple and economical, and may be used to purify alkaline phosphatase isoenzymes from other cancer tissues. This is the first report of the purification of the enzyme in seminoma. Inhibition studies suggest that this enzyme is a Nagao variant rather than the Regan type reported in several cancer tissues.
