TITLE: The fat/fiber antagonism in experimental colon carcinogenesis.

Current epidemiologic and experimental data emphasize the importance of nutrition in colon carcinogenesis. The interactions between dietary fat and dietary fiber were studied in the dimethylhydrazine model for colon cancer. A strict pair-feeding system was developed, and rats were fed one of five semisynthetic diets containing various amounts and types of fat and fiber. The incidence and number of tumors per animal were increased in all fat-fed groups. Adding fiber to the diets provided either partial (polyunsaturated fat) or complete (saturated fat) protection against the deleterious effect of fat. The antagonism between fat and fiber and the clinical relevance of these data are discussed.

TITLE: [Thoracic esophageal cancer in a young adult]

A 34-year-old Japanese male was admitted to the Second Department of Surgery, Chiba University Hospital complaining of swallowing difficulties of three months duration. Radiological examination with barium swallowing revealed thoracic esophageal cancer, classified as EiEa, was 6cm long and of the serrated type. Preoperative combined therapy included Fast Neutron, Pepleomycin, Bestatin and hyperthermia. The operative procedure was total thoracic esophagectomy and ante-thoracic esophago-gastrostomy using the right thoracotomy approach plus laparotomy. Postoperative combined therapy included Linac irradiation and Cisplatin. His postoperative course was uneventful and he was discharged without complications. The tumor was of ill-defined ulcerative type macroscopically; the pathological diagnosis was moderately differentiated squamous cell carcinoma.

TITLE: Metronidazole as a radiosensitizer and high-dose radiation in advanced vulvovaginal malignancies, a pilot study.

Advanced vulvovaginal cancers have a survival of less than 25% in 5 years when treated with radiotherapy alone. The radiosensitizer metronidazole was used with unconventional radiation fractions in 10 patients. Local control was achieved in 9 out of 10 patients treated. Five of 10 patients have remained free of disease 9-30 months postdiagnosis. Two patients died with metastatic disease, but with local control. In only 1 patient did this approach fail to control local disease. The outcome of these 10 patients represents a significant improvement over conventional radiation. A randomized prospective study has been started on the basis of this pilot study.

Radiation therapy under the conditions of hyperbaric oxygenation (HBO) using the method of mean fractionation (3.3 Gy 3 times a week up to the summary dose of 33 Gy) was employed to overcome tumor hypoxia and to raise selectively radiosensitivity of laryngeal cancer. A randomized study of 120 patients has shown that the use of radiation therapy under HBO conditions makes it possible to reduce the frequency of radiation reactions, to increase tumor resorption and the degree of radiation damage, to reduce the frequency of postoperative purulent complications, and to prevent the occurrence of early and late recurrences and metastases.

TITLE: Mitoxantrone and cyclophosphamide in patients with advanced breast cancer.

Twenty-eight patients with advanced breast cancer who had no prior chemotherapy received 14 mg/m2 of mitoxantrone iv on Day 1 and 600 mg/m2 of cyclophosphamide on Day 2, repeated every 4 weeks. Three patients (10%) achieved complete response and ten (36%) achieved partial response. The dose-limiting toxicity was leukopenia, and no septicemia occurred. Slightly greater than 50% of the patients had a wbc count nadir less than 2000/mm3. Based on the remarkably good tolerability, ie, mild nausea, rarely severe alopecia, and observed remission rate, this combination represents a positive addition to chemotherapy for advanced breast cancer.

Characteristics of both descriptive and analytic epidemiology of stomach cancer were summarised. Dietary changes, especially reduced intake of salted food mainly due to the wider use of electric refrigerator, were considered as the major reasons of recent decline in mortality. Other features of descriptive epidemiology such as socio-economic variation, lower risk in Okinawa residents and Japanese migrants to the U.S.A. were also described. In analytic epidemiology, daily cigarette smoking was found as a risk-enhancing factor in addition to frequent intake of salted food. Daily intake of green-yellow vegetables, soybean paste soup and milk were considered as risk reducing factors. Possibilities of primary prevention of stomach cancer was discussed based on these findings.

560 cases of lung cancer were detected from 1100 cases examined by fibrobronchoscopy. 91.2% of these cases were diagnosed by histology or/and cytology. 71.9% were squamous cell carcinoma. The fibrobronchoscopy gave a positive detection rate of 89.8%, and in 51.1% of cases, it was able to see the tumor or the tumor with necrosis. The positive rate of the cellular brushings was 48%, and that of biopsy, 73.7%. The fibrobronchoscopy is most efficient in localizing the occult lung cancer (positive cytology but negative chest films). The false negative causes of the biopsy or cellular brushings are discussed.

A house-to-house survey of 11 villages selected by random sampling was carried out in Bilugyun. Chaung-zone township. Mon State of South Eastern Burma to study the prevalence of oral precancerous lesions and smoking and chewing. A total of 6000 villagers who were above 15 years at the time of this study were examined. The prevalence of preleukoplakia was 0.3%, leukoplakia 1.7%, lichen planus 0.4%, leukokeratosis nicotina palati 2.3, erythroplakia 0.1%, submucous fibrosis 0.1%, and cancer 0.03%. A correlation was done between habits and lesions.

5-Fluorouracil (5-FU) requires activation to the metabolites FdUMP and FUTP for its cytotoxic effect. This activation involves the intracellular enzymes uridine and thymidine phosphorylase. We have assayed the levels of these enzymes in colorectal and gastric cancers and have shown that, in the majority of cases, the enzyme levels were higher than in the adjacent normal mucosa. It is suggested that, while the ratio tumour/normal mucosa enzyme activity may give some indication of the relative toxicity of 5-FU in these tissues, the absolute activities of the phosphorylases in tumour tissue could give a better indication of tumour responsiveness.

Cross-lip and distal flaps are still advocated in current literature for the reconstruction of large, traumatic, or surgically created lip defects. Myocutaneous advancement flaps, however, have yielded a functionally and cosmetically superior result. Moreover, they represent single-stage repairs that are technically easy to create. The most common complication noted with the use of a Karapandzic labioplasty has been a relative microstomia. An easily introduced, tripartite maxillary denture has been designed for thus affected edentulous patients. The use of these flaps in the management of advanced lip cancers dictates careful perioperative planning, principles of which are described.

In a single, rapid and precise analysis, monoacetylputrescine, N8-acetylspermidine, N1-acetylspermidine, putrescine, spermidine, and spermine can be separated using a five-buffer system on an automatic amino acid analyzer. This method allows, for the first time, the separation of all the known acetyl derivatives of putrescine and spermidine as well as the parent compounds in urine and tissues with a single automated procedure. The method has been applied to the analysis of biological samples from normal volunteers, cancer patients and a rat liver supernatant. Mass spectral confirmation was obtained for each compound.

Even though intracranial tumors are the second most common neoplasm in childhood, controlled trials for children with brain tumors are unusually difficult to conduct. The major hindrance to these studies appears to be a lack of numbers in each of the histologic subtypes and a lack of an effective collaborative effort among neurosurgeons, neurologists, radiotherapists, and pediatric oncologists, all of whom have some degree of responsibility for the primary care of these children. Improved communication between the Children's Cancer Study Group and the Pediatric Oncology Group, as well as the disciplines mentioned above, could result in significant advances in the understanding and treatment of children with brain tumors.

During the period 1968-79, a total of 90 thoracotomies for lung metastases was performed in 76 adult patients (42 men and 34 women, aged from 16 to 72 years) treated at the Gustave Roussy Institute, Paris. Forty-three had carcinoma, 29 had sarcoma and 4 had melanoma. Thoracotomy was performed once in 64 patients, twice in 10 patients and 3 times in 2 patients. Complete resection could be performed in 88%. The operative mortality rate was 1.1%. Four benign lesions were encountered. The survival rate was 56% at 2 years and 43% at 3 years. Several prognostic factors were identified. The indications for pulmonary metastasectomy within the overall strategy of metastatic cancer treatment are discussed.

Carcinoembryonic antigen (CEA) levels in sputum from patients with lung cancers (15), other malignancies (7) and non-malignant pulmonary diseases (30) were measured by enzyme immunoassay. The mean value +/- S.E. (ng/ml) of CEA levels in sputum was 3,682 +/- 1,648, 630 +/- 473 and 444 +/- 102, respectively. There was a significant difference in CEA levels between lung cancers and benign pulmonary diseases (p less than 0.01), while no correlation between the serum levels and sputum levels was found. These observations led us to conclude that assay of CEA in sputum appears to be a useful simple test for the diagnosis of bronchopulmonary neoplasms.

A total of 138 patients with various tumor sites were examined using lymphoductoscintigraphy with 113mIn. A distinct image of the thoracic lymphatic duct was obtained in 72 cases; 3 variants of normal lymphoscintigrams were singled out. In 66 out of 138 patients no distinct image of the thoracic lymphatic duct was obtained; the retardation of the radiopharmaceutical at the level of the chief lymphatic collector, its winding shift, reflux to the intercostal lymphatic vessels, sometimes to the liver, spleen and kidneys were observed. The importance of lymphoductoscintigraphy in esophageal and gastric cancer is emphasized.
