A case of metastatic tumor of the penis from lung cancer is reported. The patient, who had received a right pneumonectomy 17 months previously for a squamous cell carcinoma of the lung, complained of urinary retention and painful erection of the penis. He underwent an emergency suprapubic cystostomy. Twenty days after the procedure, he died of disseminated lung carcinoma. The autopsy demonstrated massive metastasis to the penis from squamous cell carcinoma of the lung. Penile metastasis from lung cancer is a very rare condition and only 14 cases of this secondary carcinoma have been reported.

The recorded rise in cancer mortality, particularly in industrialized nations such as the United States, is often attributed to increasing environmental carcinogens and has resulted in the public expectation for increased regulation. However, longitudinal Gompertzian analysis indicates that the major factor for rising cancer mortality is the increasing deterministic competitiveness of cancer in a climate that is becoming more conducive to human survival. Hence, rising cancer mortality is a predictable manifestation of a natural relationship between human aging and mortality, and should not be interpreted as evidence of increased environmental carcinogens.

Using a personal computer and custom-designed software as part of a quality assurance (QA) program dramatically improved the performance of health maintenance in a community hospital outpatient clinic. Previously health maintenance interventions were prompted by a simple review of the records. The computer program tracks each patient encounter with the clinic, recording the dates of immunizations, cancer screenings, and other health maintenance interventions performed by the physician responsible for the patient's care. When indicated, the computer prompts the physician to perform a health maintenance intervention. This system is an inexpensive and acceptable means of promoting and teaching health maintenance in an outpatient hospital clinic.

The records of 1383, terminal cancer patients have been reviewed to determine whether opioid requirements vary in a consistent pattern with age and with site of primary cancer. Opioid requirements are shown to decrease with age in a regular pattern and from early adult life. Opioid requirements also differ significantly with site of primary cancer and there appears to be a discernible pattern of opioid need for different cancers. Patients with sexually related cancer have the highest opioid requirements, irrespective of sex. When the sexually determined cancers are excluded, the analgesic needs of men and women are not significantly different (P = 0.191).

Aggressive therapy clearly plays a role in the management of pancreatic cancer, be it localized to that organ, limited to the upper abdomen, or metastatic. A significant percentage of patients with resectable disease can be salvaged by radical resection of the cancer and aggressive adjuvant therapies. Five-year survival rates exceeding 30% have been reported from Japan. A supportive-care-only treatment philosophy ensures that almost all patients will die of cancer and that new treatment regimens will not be evaluated, creating a self-fulfilling prophecy. Such a policy is no longer tenable.

Eccentric esophagogastrostomy was performed in 122 patients with cancers of gastric cardia and fundus from 1989 to 1991. Three patients (2.4%) had anastomotic stricture. There were no postoperative anastomotic leakage, gastroesophageal reflux and postoperative death. Since the procedure is performed on an opened stomach under direct vision, the operator can adjust the length of anastomosis. Postoperative X-ray imaging showed that the anastomotic form resembles the anatomical features of gastric cardia and fundus with a larger diameter than that made by routine operation. Therefore, the eccentric esophagogastrostomy may help to reduce the possibility of postoperative anastomotic stricture, prevent gastroesophageal reflux and anastomotic leakage.

Multifocal squamous cell carcinoma in the bronchus of the anterior segment of the right lung and in the subsegmental bronchus of the posterior subsegment of the left upper lobe was diagnosed in a 61-year-old man. The right-sided upper lobectomy followed by Nd:YAG laser photocoagulation of the superficial cancer in the left side resulted in total eradication of the tumor. Subsequent follow-up examinations for three years have revealed no relapse of tumor growth.

We describe the creation, and the first 2 years experience, of the Gambian National Cancer Registry. The major problems involved in the creation of such a registry in a developing country are discussed. The data accumulated show a low overall rate of cancer incidence compared to more developed parts of the world and indicate that the prevalent cancers, hepatoma, carcinoma of the cervix and lymphoma, are likely to be due to infectious agents. It is hoped that immunisation of children under one year against hepatitis B will drastically reduce the incidence of hepatocellular carcinoma.

All complications were registered in 139 consecutive second-look operations for ovarian cancer. Forty-one complications were registered in 33 patients, none lethal. The most frequent complications were anaemia requiring blood-transfusion (10.7%) and acute cystitis (4.3%). Other complications were seen with frequencies between 0.7 to 2.8 percent. No differences with respect to complications were found between patients with residual tumour versus those without, nor between patients where only biopsies were taken versus those who had organs removed. Although the patients were in clinical complete remission, 53% were found to have residual disease at second-look operation.

Nonmelanoma skin cancer is the most common form of cancer in the United States. The incidence of melanoma is increasing more rapidly than any other form of cancer (with the exception of lung cancer in women). Public education programs have the potential to prevent future skin cancers and promote early detection of skin cancer and melanoma. This article describes a statewide awareness campaign and outlines important nursing activities and educational resources. Among results of a pre- and postevaluation poll are an increase in the number of respondents correctly identifying melanoma as the most serious type of skin cancer and an increase in the number of respondents who stated that sunburn in children was very serious.

A complex segregation analysis was conducted on two British series (one consecutive series of probands with breast cancer and one series ascertained through a normal consultand). Altogether there were 1248 nuclear families with breast cancer. A dominant gene with a frequency of 0.003 giving a lifetime penetrance of 0.83 is favoured. Ovarian, endometrial and cancers associated with the SBLA syndrome, as well as benign breast disease, were significantly more common in familial breast cancer than in families of single cases. Probands in families with more than one individual with breast cancer were non-significantly younger than isolated probands.
