Anne Mullens used a recent fellowship provided by the Atkinson Foundation to take an in-depth look at euthanasia in the Netherlands. During her time in Holland, she discussed the issue with doctors who support and oppose euthanasia. She accompanied a doctor as he visited a patient who was dying of cancer and was beginning to consider the possibility of euthanasia. She talked to a nonphysician who is adamantly opposed to euthanasia and carries a card stating that. She visited a hospital in Amsterdam that has received requests from foreigners seeking euthansia. Mullens offers a comprehensive look at an issue that continues to provoke strong feelings among Canadian physicians and patients.

We report a case of renal cell carcinoma associated with synchronous contralateral renal pelvic cancer and bladder tumor. The patient underwent total nephroureterectomy for the left renal pelvic cancer with transurethral resection of the bladder tumor. After 2 courses of combination chemotherapy, we performed surgical enucleation of the right renal cell carcinoma. Although transurethral resection of the recurrent bladder tumors was undergone 17 months after the last operation, the patient retains normal renal function.

Expression of the neu oncogene has been extensively examined in frozen and paraffin section breast cancers; however, very few studies examine neu oncoprotein oncoprotein expression in fine-needle aspirates. To this effect, we compared the expression of neu oncoprotein in formalin-fixed paraffin-embedded breast cancers and the corresponding fine-needle aspirates of these cancers. There was 100% correlation between the expression of neu oncoprotein in the paraffin-embedded breast cancers and the fine-needle aspirates, indicating the suitability of fine-needle aspirates for the expression of neu oncoprotein in breast cancers.

We describe a patient with advanced cancer whose severe symptoms of congestive heart failure were successfully treated with dobutamine. The intermittent intravenous administration of dobutamine 5 micrograms/kg/min for 3 hr per day at home enabled control of dyspnea, leg edema, and pain, and increased urine output after 1 day. An improvement in renal function was observed in the following days. The mechanism and the utility of a palliative approach with dobutamine are discussed.

A case of metastatic tumor of the penis from lung cancer is reported. The patient, who had received a right pneumonectomy 17 months previously for a squamous cell carcinoma of the lung, complained of urinary retention and painful erection of the penis. He underwent an emergency suprapubic cystostomy. Twenty days after the procedure, he died of disseminated lung carcinoma. The autopsy demonstrated massive metastasis to the penis from squamous cell carcinoma of the lung. Penile metastasis from lung cancer is a very rare condition and only 14 cases of this secondary carcinoma have been reported.

The recorded rise in cancer mortality, particularly in industrialized nations such as the United States, is often attributed to increasing environmental carcinogens and has resulted in the public expectation for increased regulation. However, longitudinal Gompertzian analysis indicates that the major factor for rising cancer mortality is the increasing deterministic competitiveness of cancer in a climate that is becoming more conducive to human survival. Hence, rising cancer mortality is a predictable manifestation of a natural relationship between human aging and mortality, and should not be interpreted as evidence of increased environmental carcinogens.

Using a personal computer and custom-designed software as part of a quality assurance (QA) program dramatically improved the performance of health maintenance in a community hospital outpatient clinic. Previously health maintenance interventions were prompted by a simple review of the records. The computer program tracks each patient encounter with the clinic, recording the dates of immunizations, cancer screenings, and other health maintenance interventions performed by the physician responsible for the patient's care. When indicated, the computer prompts the physician to perform a health maintenance intervention. This system is an inexpensive and acceptable means of promoting and teaching health maintenance in an outpatient hospital clinic.

From the time the first case of bronchial cleft carcinoma was reported over a century ago, very few cases have been reported which prove to be a cancer of primary branchial origin, while many metastatic cystic squamous cell carcinomas in the neck have been wrongly categorized as branchial carcinoma. Here, we describe a case of primary branchiogenic carcinoma in a 53 year old Libyan female showing a transformation of a benign epithelium into a malignant type. We further report that this transformation is perhaps brought about by an irregular radiotherapy to the head and neck region.

Recent European Organization for Research and Treatment of Cancer studies show that there is less concern about the importance of platinum as an ingredient of combination regimens or as an adjunct to radiotherapy. The presence of cisplatin is related to better response and survival in patients with locoregional and distant metastatic disease.
