TITLE: The Dutch experience with euthanasia: lessons for Canada?
PUBDATE: 19950601
Anne Mullens used a recent fellowship provided by the Atkinson Foundation to take an in-depth look at euthanasia in the Netherlands. During her time in Holland, she discussed the issue with doctors who support and oppose euthanasia. She accompanied a doctor as he visited a patient who was dying of cancer and was beginning to consider the possibility of euthanasia. She talked to a nonphysician who is adamantly opposed to euthanasia and carries a card stating that. She visited a hospital in Amsterdam that has received requests from foreigners seeking euthansia. Mullens offers a comprehensive look at an issue that continues to provoke strong feelings among Canadian physicians and patients.

We report a case of renal cell carcinoma associated with synchronous contralateral renal pelvic cancer and bladder tumor. The patient underwent total nephroureterectomy for the left renal pelvic cancer with transurethral resection of the bladder tumor. After 2 courses of combination chemotherapy, we performed surgical enucleation of the right renal cell carcinoma. Although transurethral resection of the recurrent bladder tumors was undergone 17 months after the last operation, the patient retains normal renal function.

PUBDATE: 19950301
Expression of the neu oncogene has been extensively examined in frozen and paraffin section breast cancers; however, very few studies examine neu oncoprotein oncoprotein expression in fine-needle aspirates. To this effect, we compared the expression of neu oncoprotein in formalin-fixed paraffin-embedded breast cancers and the corresponding fine-needle aspirates of these cancers. There was 100% correlation between the expression of neu oncoprotein in the paraffin-embedded breast cancers and the fine-needle aspirates, indicating the suitability of fine-needle aspirates for the expression of neu oncoprotein in breast cancers.

The evidence that genetic factors are of some importance in the etiology of cancer originates has been determined by genetic epidemiology, molecular biology, cytogenetics. Genetic counselling is one of the possible strategies for controlling cancer through genetics. Genetic counselling in this field more often concerns the family of the affected individual than the individual himself. Its aim is to develop a family oriented preventive medicine.

Much controversy surrounds the appropriateness of screening for prostate cancer. The individual benefit from screening is unproven. Screening may result in many men being unnecessarily treated for prostate cancer with the associated risks of developing treatment related side effects, including impotence and incontinence. Men requesting screening need to be informed of these issues before they decide whether to proceed. This article reviews the current position in relation to screening, critically appraising current thinking in order to clarify the issues.

We describe a patient with advanced cancer whose severe symptoms of congestive heart failure were successfully treated with dobutamine. The intermittent intravenous administration of dobutamine 5 micrograms/kg/min for 3 hr per day at home enabled control of dyspnea, leg edema, and pain, and increased urine output after 1 day. An improvement in renal function was observed in the following days. The mechanism and the utility of a palliative approach with dobutamine are discussed.

A case of metastatic tumor of the penis from lung cancer is reported. The patient, who had received a right pneumonectomy 17 months previously for a squamous cell carcinoma of the lung, complained of urinary retention and painful erection of the penis. He underwent an emergency suprapubic cystostomy. Twenty days after the procedure, he died of disseminated lung carcinoma. The autopsy demonstrated massive metastasis to the penis from squamous cell carcinoma of the lung. Penile metastasis from lung cancer is a very rare condition and only 14 cases of this secondary carcinoma have been reported.

The use of specific chemicals to prevent the development or retard the progression of carcinogenesis--known as chemoprevention--is a rapidly growing area in cancer research. Developed through the integration of strong laboratory and epidemiologic evidence, chemoprevention research has moved successfully into the clinical research setting, with diverse agents being tested for their efficacy in individuals at high risk of developing certain cancers. The public health potential for reducing cancer incidence and mortality through chemical intervention provides a unique and promising opportunity, and continued support of this multidisciplinary approach to cancer prevention remains a high priority for the U.S. National Cancer Institute.

TITLE: Rising cancer mortality in the United States, 1962-1987: evidence against environmental causation.
PUBDATE: 19920801
The recorded rise in cancer mortality, particularly in industrialized nations such as the United States, is often attributed to increasing environmental carcinogens and has resulted in the public expectation for increased regulation. However, longitudinal Gompertzian analysis indicates that the major factor for rising cancer mortality is the increasing deterministic competitiveness of cancer in a climate that is becoming more conducive to human survival. Hence, rising cancer mortality is a predictable manifestation of a natural relationship between human aging and mortality, and should not be interpreted as evidence of increased environmental carcinogens.

We report the first case of stiff-limb syndrome in a patient with multiple myeloma undergoing treatment with thalidomide, and explore the potential link to the cancer and its treatment.

TITLE: Cancer screening and immunization quality assurance using a personal computer.
PUBDATE: 19900801
Using a personal computer and custom-designed software as part of a quality assurance (QA) program dramatically improved the performance of health maintenance in a community hospital outpatient clinic. Previously health maintenance interventions were prompted by a simple review of the records. The computer program tracks each patient encounter with the clinic, recording the dates of immunizations, cancer screenings, and other health maintenance interventions performed by the physician responsible for the patient's care. When indicated, the computer prompts the physician to perform a health maintenance intervention. This system is an inexpensive and acceptable means of promoting and teaching health maintenance in an outpatient hospital clinic.
