TITLE: [Effect of blood transfusions on the survival of patients with colorectal cancers]

Blood transfusion is reported to cause immunosuppression. An adverse relationship between perioperative blood transfusions and the risk of subsequent recurrence of cancer was reported recently. We reviewed the records of 282 patients and analyzed the interaction between blood tranfusion and the outcome of Dukes stages A, B and C colorectal cancers treated by radical resection during the years 1978-1985. 53 of these patients did not receive any blood transfusions. The actuarial survival analysis (Cutler and Ederer) showed no significant difference for the overall and recurrence-free survival. This study did not support the hypothesis that blood transfusions had an adverse effect on survival of patients with colorectal cancer.

From basic research to the different situations preceding mucosal malignancies, there is considerable evidence to propose that chronic and deep-seated abnormalities in local pH may have both a direct and an indirect role in the etiology of epithelial human cancer. A high microenvironmental pH also appears to act as a universal mediator and activating factor of many different chemical and biological carcinogens.

Disorder of this system may result in the multi-step event of malignant transformation. This review discusses some aspects of the interaction of growth controlling agents, cell receptors, signal transmission, and cellular response. It focuses on the role of cell receptors that recognize and transduce information of cancer initiating and promoting agents to the cell: e.g. viruses, cancerogens, peptide and steroid hormones, growth factors, local growth promoting agents, and other environmental factors. All these growth modulators may be involved in triggering cell division. The mutual interactions of promotor induced signals, genes, and oncogenes are also discussed.

TITLE: Recurrent medullary carcinoma of the thyroid demonstrated by thallium-201 scintigraphy.

Thallium-201 scintigraphy has been used in the evaluation of thyroid neoplasms, but uptake in recurrent or persistent medullary thyroid cancer has not been reported. We present two cases where focal 201TI activity localized superior mediastinal recurrences which were subsequently resected. Postoperative 201TI scans in both cases showed elimination of the focal uptake seen before surgery, as well as return of previously elevated serum calcitonin levels to normal. Scanning with 201TI may be useful in the preoperative localization of recurrent medullary thyroid cancer.

TITLE: The influence of colonoscopy on the operative management of colorectal cancer.

Total colonoscopy was used in the evaluation of 85 patients with colorectal cancer, either preoperatively or within six months of surgery. Synchronous lesions (six carcinomas, 22 adenomatous polyps, one villous adenoma and nine cases of severe diverticular disease) were detected in 38 (44.7%) patients. The planned surgical procedure was altered in 10 patients (11.8%)--a more extensive resection being performed in each case. In particular, five of the six cases of synchronous cancers were located in different surgical segments. Colonoscopy is a necessary prerequisite in the evaluation of patients with colorectal cancer.

The discovery that etoposide is one of the most active drugs in small-cell lung cancer (SCLC) led to its incorporation into a number of first-line combination chemotherapy regimens. The cyclophosphamide/doxorubicin/etoposide (CAE) regimen was shown to be as active or more active than other standard regimens in nonrandomized studies. In a randomized study presented in this report, the CAE regimen was significantly superior to the cyclophosphamide/doxorubicin/vincristine (CAV) regimen for response duration and survival in extensive-stage patients. In limited-stage patients the results were slightly better with CAE. In addition, CAE lacked the neurotoxicity of CAV.

Patients with specific types of cancer have achieved substantial prolongation of survival after successful cancer chemotherapy. Furthermore, cancer chemotherapeutic agents are being widely used as immunosuppressive drugs in patients with benign conditions. Both of these patient populations are at risk for development of late complications of treatment. Though the late effects of chemotherapy are less well defined, they are insidious in onset and not manifested clinically until damage have become overt and irreversible. Many reports on the late effects of chemotherapy demonstrate the importance of assessing not only the therapeutic results of various treatment program but also of evaluating the long-term complications of therapy in order to maximize the benefit of the treatment. Thus, skillful and judicious application of chemotherapy produces a minimum of delayed consequences.

In 1987, 11 cases of primary lung cancer were diagnosed on fibreoptic bronchoscopy and six patients were found to have clinical and radiological signs of lung malignancy. 12 of these patients were from Zomba district. This gives an estimated annual incidence of diagnosed lung cancer for Zomba of nearly 3 in 100,000. This figure is most probably an underestimate. Lung cancer was known to be rare in Malawi until 1980. If the actual incidence of this malignancy in Zomba can be applied to the rest of the country, it may be concluded that primary lung cancer is more common in Malawi than previously thought.

The paper is concerned with an analysis of the results of positive scintigraphy and thermography obtained from an investigation of 84 laryngeal and laryngopharyngeal cancer patients. The diagnostic accuracy of positive scintigraphy (38 patients) was 89.5%, that of thermography--78.9%, of both methods--93.3%. The authors described methodological aspects of the use of both methods and worked out indications for their use in combined diagnosis of laryngeal cancer: topical and differential diagnosis of malignant and benign laryngeal lesions, definition of a degree of tumor spreading (visualization of metastases to the lymph nodes), determination of the presence of laryngeal cancer recurrences and their differential diagnosis with postradiation edemas.

Twelve variants of uterine cavity structure were defined on analysis of 52 hysterocervicograms performed in patients with cancer of the uterine body. Using the method of linear programming, the most effective schemes for the location of radiation sources were proposed to ensure optimum dose distribution in intracavitary irradiation. Sources I and II were the main contributors to dose distribution. The use of sequential radiation sources was appropriate in considerable sizes of the uterine cavity. In a spherically shaped uterine cavity dose distribution could be ensured by using only one source, located in the center.

TITLE: Tamoxifen withdrawal response. Report of a case.

A postmenopausal patient with breast cancer with a left infrahilar lung mass was treated with tamoxifen. She became asymptomatic, but after having stable disease for eight months, she developed progressive increase in the size of the mass necessitating tamoxifen withdrawal. Complete resolution of the mass occurred following withdrawal of tamoxifen. Withdrawal responses, though known to occur with additive hormonal treatment, especially complete regression of metastatic disease as seen in our patient, is not widely recognized. Thus, when clinically possible, patients should be observed off treatment for up to six weeks, before initiating a new therapeutic modality.

TITLE: Indium 111-A-bleomycin--a new tracer for imaging orofacial neoplasms.

Forty-six patients with maxillofacial squamous cell cancer of variable keratinization underwent radioisotope diagnosis using Indium 111-A-bleomycin. This new technique was applied to expand the battery of diagnostic tools available for detecting maxillofacial neoplasms. Its high specificity (100%) and sensitivity (96%) make it well suited for detection of both local and distant recurrences as well as tumor regrowth. Preliminary whole body retention measurements also indicate that this technique looks promising for potential therapeutic use.
