TITLE: Cytogenetic evidence for premeiotic transformation of human testicular cancers.

To determine the point at which transformation of the germ cell occurs during meiosis in nonseminomatous testicular cancer, the sex chromosome compositions of 15 cell lines derived from primary tumors or metastases of 12 patients with testicular cancer were analyzed by trypsin G-banding analysis and Y-body staining. The simultaneous existence of both X- and Y-chromosomes in a single cell has been confirmed in 14 cell lines. This suggests that transformation of the cell occurs before the first meiotic division because it is known that segregation of X- and Y-chromosomes occurs during the first meiotic division. An incidental finding was the presence of Barr bodies in some cell lines containing more than one X-chromosome, which is consistent with the known primitive nature of testicular cancer and its ability to differentiate independently from the male host.

TITLE: The relationship between water quality and health in the London area.

In a retrospective study of the relationship between reuse of water and hazards to health, the mortality experience from different causes, principally cancer, was examined for 29 boroughs and districts in the London area for the period 1968 to 1974. Information concerning the source of water supply to each borough was obtained both for the current situation, and historically back to 1926. Socioeconomic characteristics of the boroughs were extracted from 1971 census data. Differential mortality experiences for each sex were analysed using the standardised mortality ratio for ages 25 to 74. Most of the causes of death examined were associated with one or more of the socioeconomic characteristics of the boroughs. These socioeconomic characteristics accounted for the statistical associations between water reuse and mortality for each cause studied, except male stomach cancer, where a weak residual association remained. This association disappeared when variation in the size of boroughs was taken into account.

TITLE: Recurrence of carcinoma of the colon and rectum at the anastomotic suture line.

A total of 1,315 patients were treated by potentially curative resection for a single primary malignant tumor of the large intestine. Thirty-five or 2.7 per cent of the patients subsequently presented with a recurrent tumor at the site of the anastomosis. Fourteen of these patients were treated by further operation with curative intention, resulting in a median cancer-specific survival time of 41 months. The prognosis after resection of a recurrence of the tumor at the suture line was significantly better, p=0.001, than for patients in whom no further resection or palliative operation only was performed. None of the latter has survived for more than 30 months after confirmation of the recurrence, with a median survival time of 8.5 months.

The patterns of the occurrence of breast cancer in 11 high-risk families were evaluated by segregation and linkage analysis. These patterns were consistent with the hypothesis that increased susceptibility to breast cancer was inherited as an autosomal dominant allele with high penetrance in women. The postulated susceptibility allele in these families may be chromosomally linked to the glutamate-pyruvate transaminase (E.C. 2.6.1.2, alanine aminotransferase) locus. Confirmation of this linkage in other families would establish the existence of a gene increasing susceptibility to breast cancer. Since there is no association in the general population between a woman's glutamate-pyruvate transaminase genotype and her cancer risk, the glutamate-pyruvate transaminase linkage cannot be used as a screening test for breast cancer.

A specific and sensitive double antibody radioimmunoassay for measuring human serum thyroglobulin (Tg) has been developed. Serum Tg levels are reported for patients with differentiated and undifferentiated thyroid carcinomas, post-treated and untreated hyperthyroidism, thyroiditis, and cold nodular goiters. We have studied results of serum Tg levels in the follow-up and care of patients who have undergone total thyroidectomy for differentiated thyroid carcinomas. Tg has no diagnostic value in the detection of thyroid cancer, but is a reliable index of metastatic growth in the follow-up of patients with differentiated carcinomas. In addition, serum Tg determination might be helpful in the aetiological diagnosis of bone or lung metastases of unknown origin.

A rapid, reproducible immune complex screening assay was used to quantitate levels of circulating immune complexes in the sera of normal subjects, patients with documented increases in immune complexes from rheumatoid arthritis, patients with clinically or microbiologically documented infections, and patients with cancer. Although wide variations in individual values within the groups were noted and the concurrent elevation of polyethylene glycol-circulating immune complex levels by infection was documented as expected, significant differences were found in the values in patients with cancer compared with those in normal subjects. The overall clinical application of polyethylene glycol-circulating immune complex screening is discussed and current application of screening of serial sera samples from individual patients for correlation with measurable tumor volume is proposed.

TITLE: [Findings on the knowledge of self-examination of the breast]

A research on the degree of knowledge about the problem of breast self-examination was carried out in a group of 500 women in the Out-patients Department of the Cancer Institute in Milan. The women were given a questionnaire to fill up at home and to return at the moment of the medical examination. The analysis of data showed that, though the level of education of the subjects was fairly good, the knowledge and the practise of the periodical self-examination as a method for an early diagnosis was very low. 70% of the women declared to have some knowledge about the method, but only 9% declared to practise it regularly.

Sebaceous neoplasia have been observed in members of four families exhibiting the cancer family syndrome (CFS). This disorder is characterized by adenocarcinomas, particularly involving the (proximal) colon, endometrium, and ovary; an excess of multiple primary cancer; early age of cancer onset; and autosomal dominant pattern of inheritance. Multiple adenomatous polyps are lacking in this disorder. In four patients from three of these cancer-prone kindreds, cutaneous lesions were accompanied by multiple visceral adenocarcinomas, fulfilling the criteria for Torre's syndrome, a disease that heretofore has not shown notable familial clustering characteristic of the CFS. Therefore, the coexistence of rare sebaceous neoplasia and visceral cancer in CFS supports the notion that some cases of Torre's syndrome may in fact represent the more full phenotypic expression of the gene responsible for the CFS.

Various cancer chemotherapeutic agents have been examined for their ability to produce increases in sister chromatid exchanges. Those agents which had been shown previously to produce oncogenic transformation as well as chromosomal breaks also showed significant increases in sister chromatid exchanges. Those drugs which had not been shown to be oncogenic or clastogenic in cell culture produced no increases in sister chromatid exchanges. In general, concentrations which yielded increases in sister chromatid exchanges were considerably lower than those which had been shown previously to produce oncogenic transformation and chromosomal breakage. This was particularly true for the alkylating agents. Thus, we concur that examining increases in the production of sister chromatid exchanges may be an additional sensitive method for detecting potential mutagenic and/or oncogenic agents in our environment.

AmB/NCS combination therapy against cancer was evaluated. Six gastric cancer patients were treated in this manner, and 7 gastric cancer patients treated with NCS alone to serve as the control. AmB sirup was administered orally for 4 days before surgical operation. NCS was given intraveously at the onset of gastric surgery. Lesion tissues and healthy tissues were collected from each patient and the NCS titers were measured by bioassay. It was shown in the majority of the gastric cancer cases that the NCS levels in the lesion tissues were substantially higher than in the surrounding normal tissues, whereas in the gastric cancer patients who received NCS alone, no significant differences were found between the tissues.

Ferritin from malignant tissue differs electrophoretically from normal ferritin. The molecular basis of this difference has not yet been defined. Malignant tissue contains a mixture of ferritins from normal cells, inflammatory cells as well as cancer cells. GW-39 is a pure colon carcinoma cell system that synthesizes human carcinoembryonic antigen. Therefore, ferritin was isolated from normal colon mucosa and colon cancer tissues, as well as from the colon carcinoma cell line, to clarify the molecular relationship between normal and malignant ferritins. Colon carcinoma ferritin differs in primary structure from normal colon mucosal ferritin and contains at least six additional different tryptic peptides. These six peptides were also found in the ferritin from the colon carcinoma cell line. These data suggest that the alteration in ferritin structure occurs at the cellular level and is associated with the malignant state.

TITLE: Abnormalities in water homeostasis in small cell anaplastic lung cancer.

Forty-one patients with newly diagnosed small cell anaplastic lung cancer were evaluated for abnormalities in water homeostasis. Each patient underwent a standard water load (SWL) test. Overall, 68% had abnormalities in the SWL test. Abnormalities were found in 47% of the patients with carcinoma clinically limited to one hemithorax and in 86% of the patients with more extensive carcinoma. The determination of urinary antidiuretic hormone levels was available for 27 patients. Abnormally elevated levels were found in 44% of those patients. Forty-six patients had clinically detectable syndrome of the inappropriate secretion of antidiuretic hormone (SIADH); in 12% of patients water restriction was necessary. The incidence of detectable abnormalities in water homeostasis in this study was higher than has been previously recognized. The SWL test is a sensitive and useful means of determining the presence of impaired water handling in patients with small cell carcinoma of the lung.

The investigation of colorectal disorders follows from a thorough clinical examination, examination of stools, and proctosigmoidoscopy. In many patients, barium enema examination will also be required, and the value of good air contrast examinations is stressed. Colonoscopy has revolutionised the diagnostic and therapeutic capabilities in colonic disease, but this examination is not readily available to all, its indications limited, and the risks and discomfort associated with it by no means small. In New Zealand, colorectal cancer is as common as anywhere else in the world, and the major hope of any improvement in this area is with preclinical diagnosis. This could be achieved by more widespread faecal occult blood testing especially in high risk groups (Table 2) and thorough investigation of those with positive results. Serological tests for colorectal cancer (carcino-embryonic antigen) have proved disappointing, and much less specific than originally hoped.

A propos 24 cases of gastro-intestinal tract's embolization of which eleven were performed in emergency for bleeding, the principle indications are described and illustrated. In some cases embolization gracefully replaces the ligation by a rapid and simple procedure at the time of diagnostic arteriography, preparing with safety the operative cure. In other cases embolization alone is curative: arteriovenous fistulas, telangiectases in 'Osler-'s disease, radiotherapic bleeding lesions, inoperable cancers. This implies the presence of trained radiologic teams and a tight collaboration with intensive care, endoscopy and surgery in large medical center. In this view the existence of radio-surgical rooms is useful.
