In a routine sigmoidoscopy clinic a specially trained nurse practitioner examined 825 patients between the ages of 23 and 88 with a mean age of 59. Seventy-two patients were found to have polyps; only eight of these patients had a history of rectal bleeding. Seventy-nine percent of these polyps were above 25 cm or beyond the reach of the rigid scope. Three patients with polyps were found to have cancer in the polyp. None of these patients had noted rectal bleeding. Significantly, there was a 10.6% yield of positive pathology among patients 50 and over. The training of the nurse practitioner is described in the article.

TITLE: [Effect of blood transfusions on the survival of patients with colorectal cancers]

Blood transfusion is reported to cause immunosuppression. An adverse relationship between perioperative blood transfusions and the risk of subsequent recurrence of cancer was reported recently. We reviewed the records of 282 patients and analyzed the interaction between blood tranfusion and the outcome of Dukes stages A, B and C colorectal cancers treated by radical resection during the years 1978-1985. 53 of these patients did not receive any blood transfusions. The actuarial survival analysis (Cutler and Ederer) showed no significant difference for the overall and recurrence-free survival. This study did not support the hypothesis that blood transfusions had an adverse effect on survival of patients with colorectal cancer.

Five-year results in 60 oral and pharyngeal cancer patients treated with combined approach chemoradiotherapy (39 patients) and chemoradiotherapy plus lymph node surgery (21 patients) are reported. Complete remission (CR) was achieved in 16/39 (41%) patients treated with chemoradiotherapy alone, and in 16/21 (76%) patients who had chemoradiotherapy plus surgery. The number of CR was statistically (chi-square test) higher (p less than 0.025) in the second group. The 5-year actuarial survival was 39.7% in the group of patients treated with chemoradiotherapy plus surgery. After 5 years 53% of the patients who reached CR are living free of disease in the first group and 76% in the second group.

A retrospective computed tomography evaluation of proved low-attenuation splenic lesions in nontraumatic cases was done. Computed tomography was able to distinguish cystic from solid lesions. Although computed tomography examination is sensitive in the detection of low-attenuation lesions, the computed tomography findings alone are not helpful in differentiation of different low-attenuation lesions. Associated computed tomography findings in other organs and clinical findings are more helpful than the size, shape, and computed tomography attenuation of the lesions. Splenic lesions may be the only metastatic manifestation in some cancer patients. A thin needle aspiration may be done to document the nature of the pathologic lesion in problematic cases.

When considering therapeutic options for localized prostate cancer, stage and grade of disease have been the most important determinants. In the elderly, the nominal age has assumed increasing importance in the final decision. A balanced judgment must be reached between the patient's normal life expectancy and the rapidity with which the cancer may be expected to express its malignant potential. By careful attention to patient selection and the details of treatment, definitive irradiation can improve quality of life and survival. Of 63 patients aged seventy-three to ninety years referred for irradiation, 56 were found medically suitable for definitive treatment. A review of our experience is presented.

Case-control studies of cryptorchidism and inguinal hernia were undertaken on white males born within the cohort of the Collaborative Perinatal Project of the National Institute of Neurological and Communicative Disorders and Stroke. Significantly elevated risks of cryptorchidism were found for low birthweight, for a high maternal Quetelet's index and for oestrogens administered during gestation (R = 2.8). Similarly, in the case of inguinal hernia, significantly increased risk ratios were observed for low birthweight, for gestational use of progestins (RR = 2.1), and for breech labour (RR = 2.3). Because these malformations and testis cancer share many of the same risk factors, a common mechanism for production of these diseases is proposed which is mediated by testicular hypoplasia induced by excess exogenous or endogenous oestrogen in the mother.

Seven commonly available chemotherapeutic drugs were used to determine the concentration and time of exposure necessary to kill the newly available human pancreatic cancer cell line, Colo-357. The exposure periods were 2, 6, 12, and 24 hours. The most active drugs were methotrexate, mitomycin, and doxorubicin, each with an ID50 less than 0.1 microM for both a 12- and a 24-hour exposure. Cisplatin was intermediate, with an ID50 of approximately 0.7 microM at 12 and 24 hours. 5-FU, carmustine, and streptozocin were the least effective agents, with ID50 values greater than 50 microM after a 12-hour exposure and greater than 25 microM after a 24-hour exposure.

A method for the prevention of radiation injuries of the urinary bladder and rectum for cervical cancer patients was worked out. It was based on the local application of the radioprotective agent dimethylsulfoxide (DMSO) before a session of interstitial irradiation with the AGAT-B apparatus. Concomitant radiation therapy with DMSO was provided to 22 cervical cancer patients. The control group included 59 patients who received similar treatment without DMSO. The expression of early reactions and late injuries of the rectum and urinary bladder were significantly lower in the DMSO group. A radioprotective DMSO effect with relation to tumor was not found.

Two patients with multiple sebaceous adenomas associated with visceral carcinoma (Torre's syndrome) are described, and reported cases in the literature are summarized. Certain aspects of Torre's syndrome are emphasized: that the presence of multiple sebaceous adenomas is a cutaneous marker for visceral carcinomas, that the associated carcinomas are of comparatively low-grade malignancy, and that it is a genetic condition probably related to the Cancer Family syndrome. In addition, three morphologic patterns of the sebaceous adenoma are described: solid, cystic, and "keratoacanthoma-like"; then, the potential pitfalls in the histopathologic interpretation of this tumor are discussed.

48 patients with metastatic breast cancer and extensive bone involvement were treated with salm-calcitonin and polychemo-therapy or hormonal treatment. They were compared with a control group of 73 patients without calcitonin therapy. One third of the patients from the calcitonin group referred pain relief which was obviously independent from the therapy effects of the polychemo or hormonal treatment. Since only subjective criteria for sceletal pains were used and no therapy arm with calcitonin alone was studied, the results of our study have to be critically analyzed together with other reports about calcitonin and bone metastases in the literature.

Lifelong dietary restriction beginning at 3 to 6 weeks of age in rodents is known to decelerate the rate of aging, increase mean and maximum life-spans, and inhibit the occurrence of many spontaneous cancers. Little is known about the effects of dietary restriction started in middle age. In the experiments now reported the food intake of 12- to 13-month-old mice of two long-lived strains was restricted by using nutrient-enriched diets in accordance with the concept of "undernutrition without malnutrition." The mice on the restricted diet averaged 10 to 20 percent increases in mean and maximum survival times compared to the control mice. Spontaneous lymphoma was inhibited by the food restriction.

Fifty patients with ovarian cancer had a second-look laparotomy performed after chemotherapy. Complete histologic response was confirmed in 12 patients, microscopic disease was found in 12 patients, and macroscopic disease was found in 26 patients. Fifteen patients with macroscopic disease had total tumor clearance at second look. Surgical response to chemotherapy was found to correlate well with disease volume at the onset of chemotherapy. The operation is a safe, accurate technique for assessing the ovarian cancer patient. The findings and outcome probably depend upon the timing of the procedure. Any survival benefit from the operation remains to be proved.
